8 Beginning November 5, 2014: Eligibility: >30 day wait You live >40 miles** You need to travel by plane or boat There is a body of water or a geologic formation that cannot be crossed by road in your way. Have you enrolled in VA healthcare? If yes, what care are you receiving? Have you talked to the VA about long term care, caregiver support, respite care etc? If not, why? If because of news coverage, talk about possible benefit to Veteran (reduced cost etc) If because the VA is far away, education on Veteran s Choice Act. Do you have/need dental care? Have you applied for service-connected disability (if applicable)? Are you eligible for any of the current registries? 8

9 Service-connected disability Non-Service Connected Pension (NSC) Age 65 or older, OR Totally and permanently disabled, OR A patient in a nursing home receiving skilled nursing care, OR Receiving Social Security Disability Insurance, OR Receiving Supplemental Security Income **Yearly family income must be less than the amount set by Congress Eligibility varies (based on NSC, service connection, distance etc). Talk to the Travel Office at local VA. Mileage or common carrier transport.
