1 1 Project Management Packet Project Name: Timeframe: Note: this packet must be completed in its entirety and submitted to CHS Web Team leadership upon completion of the project in order for group members to receive Student Service Learning (SSL) credit for the work they perform.

2 2 PHASE I: Planning (page 1 of 2) Prior to beginning work on this project, your group must collectively determine how to most effectively meet the need(s) it is addressing. By completing this worksheet, you will identify specific tasks to be completed and assign each task to a group member. Once you finish, obtain approval from a member of the leadership team before moving on to Phase II. General Information: 1. Write down the project description that your group was provided on the lines below. 2. Identify all group members by name on the lines below. Exchange addresses and/or phone numbers with each other so that you may stay in contact as necessary throughout the project. 3. Record this project s deadline date (as identified by the leadership team), as well as any other significant project-related dates, on the lines below. Write each date on a line to the left and then write its description on the corresponding line to the right. Task Assignments: As a group, identify the specific tasks you will need to perform in order to complete this project and record the appropriate information about them in the table on the next page. Assign a group member to each task. You may assign multiple tasks to a single group member, but list each task separately. Also establish and record a target completion date for each task, and list any additional details associated with each task (e.g. the name(s) and contact information of people from whom you will need to gather content/information for use in completing this project).

4 4 PHASE II: Project Completion/Publishing (page 1 of 2) Content Gathering/Preparation: 1. If necessary, coordinate with student and staff representatives of organizations within Clarksburg High School to discuss project plans and gather content (text, graphics, and/or other media). The types of content you will need to gather will depend on the nature of your project. Your group should have specified meeting with students or staff as a project-related task in the table in your Phase I documentation if contacting other members of the school community is necessary for your project. Discuss with a member of the leadership team about what information to obtain from such meetings if you are unsure. No more than one lunch meeting should be devoted to meeting with students or staff members. If your group is unable to gather the necessary content for your project during a single lunch period, you are expected to do so on your own time outside of our weekly meetings and come prepared with the content to the appropriate work sessions. Record any and all meetings with students and staff members on the lines below, including the name of each individual with whom you met, the date of each meeting, and a short description of the outcome of each meeting. Once all necessary content has been gathered, save all media (including text documents, images, and video/other multimedia content) to one group member s local drive (Clarksburg High School computer account). You may also choose to save content to other group members local drives or to other forms of portable storage for purposes of convenience. Carrying Out Assigned Tasks: During the designated work session(s), each group member is to complete the task(s) assigned to him or her. This may be accomplished by using graphic design software applications, word processors, code editors, or the Montgomery County Public Schools Tron content management system. At least one member of your group holds access credentials for Tron and has been trained in the use of the system. Should your group need additional support with using Tron, the leadership team is able to provide documentation and assist you as needed. While completing their assigned tasks, each group member should save all project-related files in an organized fashion (using logical

5 5 PHASE II: Project Completion/Publishing (page 2 of 2) folder structure) on his or her school computer account (and on a portable storage device if desired) and keep in communication with the rest of the group with regard to the status of his or her tasks. Should a group member be absent from a scheduled work session, it is his or her responsibility to contact a member of the leadership team to arrange for coverage on tasks as necessary. If any member of your group has a planned absence scheduled, he or she should inform both the team as a whole and a member of the leadership team as soon as possible. All group members should coordinate with their group s Tron account holder to arrange for publication of their project components upon completion of their assigned tasks. All content must be published to the Clarksburg High School web site by the project deadline. Task Completion Record: As your group members complete their assigned tasks, use the table below to record information about their work. This should include a description of each of the tasks completed, the name of the group member who completed it (and his or her initials), its actual completion date, and any relevant notes regarding its completion status. 6 6 PHASE III: Post-Project Report (page 1 of 2) Writing The Post-Project Report: Once your group has completed its assigned project, you need to submit a brief followup report on its status. It must respond to all of the points listed below. You may choose to write it in paragraph form or use a bulleted list, and you may either handwrite it on the back of this page or attach a separate sheet of paper. The purpose of this report is to justify each team member s involvement in the project for purposes of awarding Student Service Learning (SSL) credit and to inform the leadership team of the status of your project as well as of any follow-up actions that need to be taken. Once you complete this brief report, submit this entire completed packet to a member of the leadership team. You will be contacted should the leadership team feel that follow-up communication with your group is necessary. Points to be addressed: What were the objectives of your project? Who worked on the project and what, specifically, did each group member contribute to its completion? Please be detailed in your response to this point. Were you able to complete the project according to the specified timeframe, or did you encounter any problems which caused you to have to extend the timeframe or make any other changes to the original project plan? Were you able to meet all of the project objectives? Did you encounter any technical problems? Did you need to contact members of the Clarksburg High School community (either students or staff members) to gather information, and did you have difficulty contacting those individuals who were important to the progress of your project? Do additional steps need to be taken in order to follow-up on/revisit your project later on? Do you have any questions, comments, or concerns regarding your project?
