4 NSC SPEECH-LANGUAGE PATHOLOGY PROGRAM GUIDE Thank you for expressing interest in the SLP Program at Nevada State College! To confirm that becoming an SLP is the right decision for you, we request that you become familiar with the field, and the program at Nevada State College. Please check each box below as you complete these tasks, sign and date at the bottom, and submit this form with your application to the SLP program. I have visited the website: to learn about the field of speech-language pathology I have reviewed the document Eligibility Requirements and Essential Functions, Council of Academic Programs in Communication Sciences and Disorders (2007), and have considered whether I possess the necessary skills and abilities for this profession. I understand that I may seek accommodations and modifications with the Resource Center for Students with Disabilities (RCSD) I understand that the Master s degree is the preferred qualification and entry level to practice in the profession. I understand that the Nevada Department of Education currently allows those with a bachelor s degree in Speech-Language Pathology, or attaining the required endorsement are permitted to practice within the public schools in Nevada. There is no guarantee that this regulation will not change in the future, and that a master s degree will be required as the minimum entry requirement for all settings. I am aware that entry and completion of the SLP Program at Nevada State College does not guarantee admission into a graduate level program. I have visited the NSC Speech Pathology Program website, and understand the requirements of the program. I understand that membership in the Master Clinician Network ( ) is an ongoing requirement for program participation. I am aware that it is my responsibility to register as a Student and to pay any associated fees. I agree to maintain an ongoing registration in the Master Clinician Network while enrolled in the SLP program at NSC. If you have questions, or would like to further discuss your interest in the Speech Pathology Program at Nevada State College, please call (702) to make an appointment with an advisor in the SLP program. We would be happy to meet with you. Thank you for considering a career in Speech-Language Pathology. Printed Name: Student ID #: Signature: Date: Updated 10/15/14 ss
