South Country will not use any policy or practice that has the effect of such discrimination. Enrolling in this health plan does not guarantee you can see a particular provider on this list. If you want to make sure, you should call that provider and ask whether he or she is still part of this health plan. You should ask if he or she is accepting new patients. This health plan may not cover all your health care costs. Read your contract or Evidence of Coverage carefully to find out what is covered. American Indians: American Indians can continue or begin to use Tribal and Indian Health Services (IHS) clinics. For enrollees age 65 years and older, this includes Elderly Waiver (EW) services accessed through the tribe. If a doctor or other provider in a Tribal or IHS clinic refers you to a provider in our network, we will not require you to see your health plan primary care provider prior to the referral.

4 Welcome to South Country Health Alliance South Country Health Alliance (South Country) is a county-based health plan that serves twelve Minnesota counties. Through South Country, these counties have joined together to help improve how you receive your health care. Clinics, doctors, dentists, eye doctors, chiropractors, hospitals, nursing homes and home care agencies are working with us to see that you get the best care. The twelve counties in our service area include: Brown, Dodge, Freeborn*, Goodhue, Kanabec, Morrison, Sibley, Steele, Todd, Wabasha, Waseca and Wadena. We work closely with county health care providers, human services and county public health departments in these counties to support your wellness. South Country provides five different programs to meet members needs. Your county worker will help you find the program that best fits your needs. * The program choices in Freeborn County are Minnesota Senior Care Plus (MSC+), SeniorCare Complete (MSHO) and AbilityCare (SNBC). South Country s Five Programs Prepaid Medical Assistance Program (PMAP) PMAP is a managed care program that covers health care for the following people who have Medical Assistance: Children under the age of 21 Parents and caretakers of a dependent child Pregnant women Certain low-income adults without a dependent child Our PMAP program is called Medical Assistance. MinnesotaCare MinnesotaCare is a managed care program that covers health care for people who do not have access to affordable health care coverage. Some members may be required to pay a premium. Minnesota Senior Care Plus (MSC+) MSC+is a managed care program that covers health care and Elderly Waiver services for people who: are ages 65 and over and have Medical Assistance. If you are enrolled in MSC+, you may have another option available. Check with your county financial worker to see if you can enroll in the Minnesota Senior Health Options (MSHO) program. MSHO is a voluntary program. Minnesota Senior Health Options (MSHO) MSHO is a voluntary managed care program that covers health care and Elderly Waiver services for people who are ages 65 and over, have Medical Assistance, and have both Medicare Parts A and B. Our MSHO program is called SeniorCare Complete. If you choose to leave MSHO, you will be enrolled in an MSC+ plan for your Medical Assistance servies, if you are eligible to enroll. Special Needs BasicCare (SNBC) SNBC is a voluntary managed care program that covers health care for people with disabilities who: are ages 18 through 64, have Medical Assistance, and have either both Medicare Parts A and B or Medical Assistance only. Our SNBC program is called AbilityCare. AbilityCare also integrates Medical Assistance and Medicare Parts A,B and D for people who choose this option. ii Primary Care Network Listing

5 Important Information Clinics The clinic information printed in this listing was correct as of the date of printing (Sept. 2012). You need to choose a primary clinic when you enroll. Your primary care clinic can provide most of the health care services you need, and will help coordinate your care. Please confirm with Member Services at (toll free), or 711 (TTY) that your clinic is still a provider with our health plan before signing up. How do you choose your PCP? When you become a member of our plan, you must choose a Primary Care Provider (PCP) from our Primary Care Network Listing (or Provider Directory). You must write your choice on the enrollment form. If you cannot find a PCP listing for your area, or have questions, call Member Services. If you do not choose a PCP, the PCP nearest to you will be assigned as your PCP. Cost Sharing You may be required to pay an amount toward some health care services. This is called cost sharing. Some examples of cost sharing are copays or deductibles for certain services. You are responsible to pay your cost sharing amount to your provider. Members will receive an Evidence of Coverage with more information about cost sharing. Member Identification (ID) Card and Member Packet Members will get a health plan Member Identification (ID) Card. This card must be shown whenever you get health care. You will also receive a member packet after you become a South Country member. Your member packet will include an Evidence of Coverage that explains services covered. Also included in the packet is a Provider Directory listing the providers in our network. Service Accessibility If you need special access to get services from a provider, you may ask for information about accessibility by contacting Member Services at (toll free). Service Authorizations Our approval is needed for some services to be covered. The approval must be obtained before you get certain services or before we pay for them. You may call Member Services at the phone number listed on the inside cover of this booklet if you are concerned about a service being covered or not. Members will receive an Evidence of Coverage with more information about which services require authorization. Services Care Coordination (For MSC+, SeniorCare Complete and AbilityCare Members) Care coordination services may be available to help you with your health care needs, such as, understanding directions from your doctor, getting to and from your clinic, scheduling appointments with specialists, or getting a prescription filled. Your care coordinator may be able to assist you in accessing medical supplies like walkers and hospital beds or help you with transportation to your appointment. You can get more information on Care Coordination services by calling Member Services at (toll free). Chemical Dependency If you need chemical dependency services, you can see any provider in the South Country/ MMSI network without a referral. Call Member Services at (toll free) if you need help finding a chemical dependency provider. Members should call Member Services before treatment begins as some treatments may require prior authorization. Chiropractic Care South Country provides some chiropractic services. You will receive a list of chiropractors in your Provider Directory after you become a South Country member. If you wish to go to a chiropractor outside of our network, you will need a service authorization before you get the service. Call Member Services if you need a service authorization. You can get additional information on accessing chiropractic services by contacting Member Services at (toll free), or 711 (TTY). Member Services Toll Free: , TTY 711 iii

6 Dental DentaQuest provides dental services for South Country s members. You will receive a Provider Directory that lists the dental providers after you become a South Country member. The Customer Service number for DentaQuest is (toll free), or (TTY for the hearing impaired). Durable Medical Equipment (DME) You can get information on how to access Durable Medical Equipment by contacting Member Services at (toll free). Elderly Waiver Home and Community Based Services (For MSC+ and SeniorCare Complete Members Only) If you are age 65 or over and need to stay in your home, there is a program called Elderly Waiver (EW). EW offers more services than regular Medical Assistance. Services may include visits by a nurse, home care services, homemaker services, chore services, and more. Your Care Coordinator can arrange these services for you. If you would like more information, please call Member Services at (toll free). Elderly Waiver (EW) Obligation (MSC+ and SeniorCare Complete Members Only) If you get EW services and your income is over the income limit, you may have to pay a monthly amount. This is called EW waiver obligation. Your county will notify you of any EW waiver obligation when your eligibility is determined. Eye Care Services South Country members can receive eye care services. These services include eye exams and glasses. You will receive a list of eye care providers in your Provider Directory after you become a South Country member. The Evidence of Coverage lists the eye services covered by South Country. You can get more information on eye care service by calling Member Services at (toll free). Health Education South Country offers wellness programs to help members achieve their best personal health and wellness. This includes: Programs that encourage members to complete recommended preventive services such as mammograms, wellness checkups, blood lead tests, and immunizations. A program that supports participation in most Community Education classes. A tobacco cessation program through the Mayo Clinic Tobacco Quitline. A trained counselor can talk with you over the phone and provide information, support, and advice. You can call the Mayo Clinic Tobacco Quitline at (toll free) or (TTY for the hearing impaired). For more information on any of these programs visit our website or call Member Services at (toll free). Home and Community Based Waiver Services for People with Disabilities (AbilityCare Members Only) We do not cover Home and Community Based Services for People with Disabilities, but you may be eligible to get them through your county. Call your county to apply for these and other waiver services under the Community Alternatives for Disabled Individuals (CADI), Brain Injury (BI), Community Alternative Care (CACI or Developmental Disability (DD) Waiver programs. These waiver programs offer more services than regular Medical Assistance. Services include extended home care, assisted living, supported employment and more. Home Care Home Care services include skilled nursing, rehabilitation therapies (for example-speech, physical, respiratory, and occupational), home health aide, medical equipment and supplies, Personal Care Assistant (PCA) and Private Duty Nurse (PDN) services. We do not cover PCA or PDN services for AbilityCare members, but you may be able to get them through your county. Call your county to apply for these services. Hospitals Use the hospital(s) associated with your primary care clinic. There is a list of hospitals after the primary care clinic section in this booklet (beginning on page 34). If you need to go to a hospital that is not in the listing or is not connected with South Country, call Member Services toll free at before you get services. If you are admitted to an out-ofnetwork hospital, call Member Services within 48 hours of your admittance. 7 Interpreter Services Hearing and language interpreter services are available to help you get services. Oral interpretation can be given to you in your language. You may be able to get written information in your language. Call Member Services toll free at to get interpreter services. Mental Health If you need mental health services, you can see any provider in the South Country network without a referral. Call Member Services at (toll free) if you need help finding a mental health provider. Members should call Member Services before treatment begins as some treatments may require prior authorization. Nursing Home and Skilled Nursing Home Care (For MSC+, SeniorCare Complete and AbilityCare Members) For MSC+ and SeniorCare Complete members we will cover a total of 180 days of nursing home care. If you need nursing home care beyond the 180 days, the Minnesota Department of Human Services (DHS) will pay for your care. This applies unless you were already in the nursing home when you enroll in our plan. For AbilityCare members we will cover 100 days of nursing home care. If you need nursing home care beyond the 100 days, the Minnesota Department of Human Services (DHS) will pay for your care. This applies unless you were already in the nursing home when you enrolled in the plan. You can get more information nursing home services by calling Member Services at (toll free). Open Access Services You may choose to receive care from any physician, clinic, hospital, pharmacy, or family planning agency to get some services even if it is not in our network. The services are: family planning, diagnosis of infertility, testing, and treatment of sexually transmitted diseases, and testing for AIDS or HIV-related conditions. You can get more information on open access services by calling Member Services at (toll free). Pharmacy Members must use our network of pharmacies for drugs covered by our health plan. Remember to have your South Country identification card and your Minnesota Health Care Programs card available when you call to refill your prescription. Also, remember to show your identification card when picking up your prescriptions. If you are enrolled Medical Assistance, MSC+, or AbilityCare (Medicaid only) and have Medicare, you must enroll in a Medicare Prescription Drug Program (Medicare Part D) to get most of your drugs. You can get more information pharmacy services by calling Member Services at (toll free). Specialty Care You can see any of the specialists in our network without a referral. You will receive a Provider Directory that lists all the specialists after you become a South Country member. You can get additional information on to how to access specialty care by calling Member Services at the number listed on the inside cover of this booklet or by checking our web site at www. mnscha.org. Spenddowns AbilityCare members who have an income over a certain limit may have to pay an amount each month called institutional or medical spenddown. AbilityCare members with an institutional spenddown continue to pay their spenddown to the facility in which they live. Members who do not pay ther speddowns as required will be disenrolled from AbilityCare. People with a medical spenddown are not eligible to enroll in AbilityCare. Transition of Care When you join South Country we will help you continue to get necessary health care services that were authorized before you enrolled with us. Some examples of these services are: orthodontia, mental health, chemical dependency, at-risk pregnancy, or prescriptions. Transportation (For members of SeniorCare Complete, MSC+, Medical Assistance and Ability- Care) We are not required to provide transportation to your primary care clinic if it is over 30 miles from your home if a primary care clinic is available within 30 miles. 8 30 miles from your home can be made in certain circumstances. If you need a ride and a closer clinic in our network is not available, please call our Member Services at (toll free). Transportation may be provided to South Country members for medical and dental appointments if you do not have transportation, even if it is over 30 miles from your home. If you need help accessing transportation services, or have special transportation needs, call your county worker or call Member Services at (toll free). Urgent/After Hours/Emergency Care When possible, call your primary care clinic before going to the emergency room. They can help you decide if you need urgent or emergency care. Your primary care clinic s phone number will be on your member identification card. Also, South Country members have access to the Ask Mayo Clinic nurse advice line. Call the Ask Mayo Clinic nurse advice line if you need help in deciding if you need urgent or emergency care. Nurses are available 24 hours a day, seven days a week at (toll free), or (TTY for the hearing impaired). In an emergency that needs treatment right away, either call 911 or go to the nearest emergency room. South Country does not cover emergency care or other health care services received from providers located outside the United States and Canada. Member Advisory Committee South Country strongly believes in working with our mem bers to provide the best health care services possible. Join the South Country Member Advisory Committee (MAC) and make suggestions on how South Country can better serve their members. The MAC group meets every other month. Call Member Services at the number on the inside cover of this booklet to become a participant and learn more about the benefits of being a MAC member. South Country Community Care Connectors Unique to South Country is our Community Care Connectors (CCC). South Country has a team connector in each of the counties that we serve. The connectors help our members find the services and community support they need to stay healthy.
