2 WELCOME Indiana State University College of Health and Human Services, School of Nursing, Department of Advanced Practice Nursing faculty and staff are pleased with your decision to volunteer as a clinical mentor for a Doctor of Nursing Practice (DNP) student. The Department of Advanced Practice Nursing has a rich history of providing quality, offcampus clinical experiences to its students. It is indeed to the credit of our many fine mentors throughout the nation that such an enriching educational environment has developed. Please complete the brief Mentor Biographical Data Form attached or provide a recent resume or curriculum vitae along with proof of licensure and certification, when applicable. Please provide the month/year the mentorship will begin and the month/year the agreement will end. If you have already been designated as a mentor or are renewing your agreement, simply sign the Mentor Agreement (first page with names/dates) and return it to the student who is responsible for returning it to the Department of Advanced Practice Nursing. For your information and review, we have included the DNP Clinical Guidelines document in Appendix A. Appendix B is a list of key departmental contacts. Appendix C includes a sample of the DNP Clinical Hours Log. On behalf of the Department of Advanced Practice Nursing at Indiana State University College Health and Human Services, we extend our sincere appreciation for your willingness to serve as a mentor. Students: All you need to return from this clinical packet is the actual completed mentor agreement with signatures and the necessary copies of license and board certification verifications. You need to scan and all required pages as a multiple page pdf attachment to the program director. You must receive approval before doing any clinical hours in the program. You must wait until the semester officially starts to begin your clinical rotation all courses. 3 Mentor Functions & Responsibilities: 1. Mentors the DNP student as a leader and advanced practice provider. 2. Serves as an exemplary role model, host, sponsor and teacher to the DNP student. 3. Provides orientation to the student of the organization as appropriate. 4. Provides temporary office space, when applicable, for the student. 5. Facilitates selection of appropriate experiences for the student to meet specified outcomes based on the student s scholarly project. 6. Provides the student with experiences, as appropriate, to meet the course requirements/objectives and specific educational expectations. 7. Appraises the student s knowledge and growth during the designated mentorship period. This may include the completion of formal evaluations. 8. Maintains communication with the faculty as needed; discuss/evaluate student progress. 9. Notifies faculty of areas of concern regarding the DNP student.

4 DNP Student Responsibilities 1. Responsible for understanding and practicing within the scope of advanced nursing practice as regulated by the Nurse Practice Act in the state in which the clinical experience occurs. Students are responsible for determining any state requirements that may hinder their ability to complete clinical experiences in their states. 2. Provides Indiana State University and/or mentor/clinic with a current license to practice professional nursing prior to the clinical experience. Provides proof of current immunization status, CPR certification and Federal Background check upon request. 3. Submits the signed mentor agreement to the Department of Advanced Practice Nursing, Indiana State University, College of Health and Human Services prior to the beginning of the DNP scholarly project course and has received approval from DNP advisor or faculty. 4. Negotiates goals/objectives with the mentor for fulfilling clinical requirements, course objectives, and ultimately achieving a gradual progression toward mastery of the student learning outcomes by graduation. 5. Provides the mentor with written course objectives for the clinical experience and negotiates educational and clinical experiences for their attainment based upon the goals set forth in each DNP scholarly project course. 6. Maintains a collegial relationship with the mentor. 7. Submits Clinical Agency Form if required by the agency. It is maintained accurately and kept up-to-date at all times it is initialed daily by the student and mentor. 8. Evaluates attainment of course objectives/student learning outcomes and seeks clinical experiences for those objectives which were not fulfilled. 9. Assumes responsibility for arranging site visits with the faculty and mentor at the stakeholder sites when within driving distance of ISU. Notifies mentor of any absences prior to scheduled date of experience. 10. Assumes responsibility for individual learning needs recognizing own limitations and strengths.

Acknowledge the breach of the Student Academic Integrity Code with falsification of any clinical documents, hours, and patient data will result in immediate dismissal from the DNP program and from ISU. 13. Must dress professionally and appropriately according to the setting. A lab coat or appropriate attire fitting to the setting and name pin identifying the student as an ISU Doctor of Nursing Practice Student should be worn while in the clinical setting. 14. All students are required to notify the Program Director via or personal phone call within 72 hours of the commission of any act that: (a) may put their professional license in jeopardy, (b) resulted in a critical incident involving patient safety, or (c) could result in your failure to possess an unencumbered professional license in the future. This would include any notification of pending judgment or action taken against the student s professional license(s), any traffic offense involving illegal drugs or alcohol or if any person was killed or hospitalized as a result of an accident in which you were charged with a traffic violation or in the event that criminal or civil charges are filed against you in a court of law. You do not need to disclose information about minor traffic violations. A written explanation of any incident and copies of all explanatory documentation must be submitted to the Program Director within a reasonable time frame after the initial phone call to the Program Director. The director may be reached urgently at

6 DNP Faculty Responsibilities 1. Provides clarification of clinical educational requirements and objectives with the mentor as desired or deemed necessary. 2. Assesses the adequacy of clinical experience and appropriateness of experiences for the successful completion of student learning outcomes and individual course objectives. 3. Critiques student s mastery of the student learning outcomes via examination of clinical logs, course assignments, mentor feedback/evaluations, student s self-evaluation, student/mentor and student/faculty conferences, as well as communication via or when practical, site visits. 4. Maintains open dialogue regarding the student s progress with the mentor and student. 5. Facilitates and enhances the student - mentor- faculty relationship by providing continual, constructive feedback and other information as appropriate. 11 Appendix A COLLEGE OF HEALTH AND HUMAN SERVICES SCHOOL OF NURSING DEPARTMENT OF ADVANCED PRACTICE NURSING DOCTOR OF NURSING PRACTICE PROGRAM Clinical Guidelines Under the guidance of the faculty advisor, the project advisory committee (PAC) and designated mentor, the student will develop and implement a specific outline of clinical experience concerning an area of interest. The outline will identify specific opportunities for meaningful engagement with experts from nursing, as well as other disciplines The Doctor of Nursing Practice (DNP) scholarly project is a process to address a clinically relevant problem in nursing. This requires competence in knowledge development activities such as the translation of research into practice, the evaluation of practice, activities aimed at improving the reliability of healthcare practice and outcomes, and participation in collaborative research (DePalma & McGuire, 2005). Clinical experiences are guided by the DNP competencies (AACN, 2006), student learning outcomes and the individual course objectives. Emphasis should be placed on the application and synthesis of new learning while building upon previous knowledge. The clinical experiences can include a wide variety of sites where the DNP student achieves additional competencies for the practice doctorate (NONPF, 2010). According to NONPF, residency provides additional hours in autonomous practice, leadership, practice inquiry, and policy as part of the fabric of professional preparation for the NP with a practice doctorate (NONPF, 2010). The line between current employment and clinical scholarship hours and project must remain clear to the organization, the mentor, the DNP Project Advisory Committee and the DNP student. The DNP student must be able to demonstrate the achievement of the DNP competencies and student learning outcomes, regardless of whether they are in their current place of employment or a different clinical setting (NONPF Board of Directors, 2010). Mentors will work to facilitate these achievements. Student Learning Outcomes: 1. Integrate scientific knowledge to influence health policy and decisionmaking through leadership, collaboration, and inter-professional action at the organizational, local, regional, national, and global levels. 2. Develop and evaluate care delivery approaches that meet current and future needs of patient populations based on scientific findings in nursing

12 and other clinical sciences, as well as organizational, political, and economic sciences. 3. Use analytic methods to critically appraise existing literature and other evidence to determine and implement the best evidence for clinical practice. 4. Design, select, use, and evaluate programs that evaluate and monitor outcomes of care, care systems, and quality improvement including consumer use of health care information systems. 5. Advocate for social justice, equity, and ethical policies within all healthcare arenas. 6. Employ effective communication and collaborative skills in the development and implementation of practice models, peer review, practice guidelines, health policy, standards of care, and/or other scholarly products. 7. Synthesize concepts related to clinical prevention and population health in developing, implementing, and evaluating interventions to address health promotion/disease prevention efforts, in care of individuals, aggregates, or populations. 8. Demonstrate independent advanced practice clinical judgment, systems thinking, and accountability in designing, delivering, and evaluating evidence-based care to improve patient outcomes. Hours: a. Indiana State University requires 1200 post-baccalaureate hours of clinical experience for the doctor of nursing practice degree; b. Supervised clinical hours, completed in a Master of Science program or Post-Master s Certificate program, may be applied toward the hour requirement. A maximum of 700 post-baccalaureate clinical hours may be applied. Students must have completed the verification of post-baccalaureate hours form in order for these hours to be applied. Students who have not completed 700 post-baccalaureate hours may be credited a maximum of 500 hours toward the requirement. Students must complete the portfolio process to be considered for the hour credit; c. The number of hours to be applied will be jointly determined by the student, the committee (comprised of nursing faculty) and the Program Director; d. Documentation of post-baccalaureate hours, as well as the Independent Practice Verification Form/portfolio, should be sent to the DNP Program Director as part of the final application packet. These documents are linked from the application worksheet on the DNP website; and, e. It is required that each student will do a minimum of 500 clinical hours f. Table 1, provides some practical guidance on clinical.

13 Activities: a. Early in the program, students will discuss their ideas/plans for clinical with their advisor, mentor, and/or instructor; b. Identified activities should be experiences that will contribute to the student s career and are beyond/different from normal everyday compensated work activities; c. Documented hours done for the student s project may be included as clinical hours as long as they are new experiences for the student, are well documented, mentored, and deemed appropriate by the advisor/course instructor; d. The student, with the help of their advisor/instructor, will identify an appropriate mentor; e. Mentors should be doctorally prepared, or master s prepared, advanced practice nurses or experts in the student s area of interest. Students and mentors will complete the mentor agreement and send this along with a CV to the advisor or program director. The advisor or program director will determine that the mentor has the necessary qualifications and, if so, will approve their mentor status; f. The student will complete an outline of clinical experiences, in collaboration with the mentor and advisor, prior to logging any clinical hours. The outline is a dynamic document that will be altered to ensure that the student s needs are met each semester. The outline will evolve over the course of the program based upon evaluations and feedback from the mentor and advisor; g. Objectives, as well as the means of achieving the goals, will be developed with the mentor and approved by the advisor; h. Accomplishment of goals and clinical hours will be documented on the clinical hour log form and signed by the mentor and advisor; and, i. The completed clinical hour log should be sent to the advisor and mentor at the completion of each semester (submission via assignment submission link). j. All assignments for the DNP Scholarly course series must be met to progress in the DNP program Mentors: a. Ideally, mentors should be doctorally prepared and/or have evidence of expertise in the area. Master s prepared mentors will be considered if the student is able to provide documentation confirming that the mentor is an expert in their area of interest. Exceptions to this should be discussed with the advisor and/or the program director.

If you have a health condition or you are pregnant or have been hospitalized you must be of sound health to return to the healthcare setting. 15 Name Tags All students must wear a name tag identifying themselves as an ISU student during any on-site clinical time. Name tags are ordered in advance of entering the clinical setting. Students should identify themselves as a graduate DNP student when speaking to the vendor.

17 Appendix C The clinical hour log below should be uploaded when completed to scholarly clinical course site and the DNP Portfolio Site. It is recommended that you keep an ongoing log throughout the program in an Excel sheet and upload this file to the DNP Portfolio site at the end of each semester. This will help us to track hours for accreditation purposes and also prevent you from losing your work should you have a computer/hard drive failure. Falsification of clinical hours may result in dismissal from the DNP program and/or Indiana State University in congruence with the ISU Student Academic Integrity Code. An Excel version of this log is provided and is an acceptable alternative. The most important part of this log secondary only to tracking the hours is linking each experience to one or more of the eight program outcomes.
