1 PROGRAM LEARNING OUTCOMES: MARIAN UNIVERSITY SCHOOL OF NURSING AND HEALTH PROFESSIONS Master of Science in Nursing Family Nurse Practitioner (FNP) 1. Demonstrate strong critical thinking and decision-making skills. 2. Critically and accurately assess, plan, implement, and evaluate educational experiences of learners (nursing students, staff, and clients) to improve learning and health outcomes. 3. Critically and accurately assess, plan, implement, and evaluate educational programs available to nursing students, staff, and clients to improve delivery of education and to promote health outcomes. 4. Communicate effectively, both orally and in writing, with professional and non-professional individuals. 5. Analyze, synthesize, and utilize knowledge acquired throughout the course of study. NUR 623 PRACTICUM I: FAMILY NURSE PRACTITIONER AND ROLE DEVELOPMENT The graduate student begins to implement the role of the family nurse practitioner. Using critical thinking, the student develops decision-making and clinical management skills in family-centered health when caring for individuals and their families across the lifespan. The student utilizes data-gathering techniques and diagnostic reasoning in order to enhance, maintain, and restore health. The focus of the first practicum is on practicing advanced health assessment skills, establishing therapeutic relationships with clients, the differential diagnostic process, and collegial relationships with other health care providers. Health education is emphasized. The student manages acute and chronic health problems, as well as integrating health protection, health promotion, and disease prevention while under the direction of a health care preceptor provider. Discussion of practicum experiences with a focus on the client case presentations and role development are integrated throughout the course. Employee and occupational health will also be addressed. valuing, decision making, and communicating to: 1. Use comprehensive assessment skills to determine the client diagnosis under the supervision of the health care preceptor provider. 2. Plan comprehensive management plan and drug therapy for clients with acute and chronic health needs under the supervision of the health care preceptor provider. 3. Propose treatment plans that include strategies for health promotion, maintenance, and restoration under the supervision of the health care preceptor provider. 4. Identify ethical and legal issues encountered during the practicum experience. 5. Identify competencies needed by the advanced nurse practitioner. 6. Identify current trends and on-going issues related to the advanced nurse practitioner role. 7. Interact therapeutically with clients and collegially with preceptors and other health care providers. 8. 2 NUR 632 PRACTICUM II: FAMILY NURSE PRACTITIONER AND ROLE DEVELOPMENT The graduate student continues to implement the role of the Family nurse practitioner. Using critical thinking, the student continues to develop decision-making and clinical management skills in caring for individuals and their families. The student utilizes data-gathering techniques with analysis in order to enhance, maintain, and restore health. Health education is emphasized. The student manages acute and chronic health problems in cooperation with a health care preceptor provider. Discussion of the practicum experiences with a focus on the client profile and role development occurs weekly. The areas of radiology, office procedures, cardiac emergencies and electrocardiograms will be addressed. valuing, decision-making, and communicating to: 1. Use comprehensive assessment skills for clients with acute and chronic health needs in cooperation with the health care preceptor provider. 2. Plan comprehensive management plan and drug therapy in cooperation with the health care preceptor provider. 3. Implement educational strategies for health promotion, maintenance, and restoration. 4. Discuss ethical and legal issues encountered during the practicum experience. 5. Discuss competencies needed by the advanced nurse practitioner. 6. Discuss current trends and on-going issues related to the advanced nurse practitioner role. 7. Implement care for clients that are respectful and cognizant of individual cultural and spiritual beliefs with the health care preceptor provider. 8. Demonstrate knowledge of advanced practice phenomena, interventions, and outcomes of care through presentations of client profiles to faculty and fellow students. NUR 642 PRACTICUM III: FAMILY NURSE PRACTITIONER AND ROLE DEVELOPMENT: Graduate students synthesize the role for the family nurse practitioner. Using critical thinking skills, the graduate student synthesizes decision-making and clinical management skills for individuals and their families. The graduate student utilizes data-gathering and diagnostic reasoning techniques with analysis in order to enhance, maintain, and restore health of complex clients and families. Health education and program planning are emphasized. Students manage acute and chronic health problems in collaboration with a health care provider and demonstrate the ability to recognize less common health issues. Discussion of practicum experiences with a focus on the client case studies and role development occurs regularly. The course focuses on greater responsibility and accountability in managing care of acute clients as well as family groups. Collaborative management of acute and chronic problems for the complex client and family with multi-system problems is explored. valuing, decision making, and communicating to: 1. Uses comprehensive assessment skills for clients with acute and chronic health needs. 2. Plan a comprehensive clinical management plan and drug therapy for clients with complex acute and chronic health needs. 3. Evaluate treatment strategies for health promotion, maintenance, and restoration. 4. Analyze ethical and legal issues that affect advanced nursing practice. 7/28/15 GMK 2/16/16 MF 2 of 5

3 5. Meet minimal competencies needed to meet the certification requirements to practice as a family nurse practitioner. 6. Analyze current trends and issues related to the advanced nurse practitioner role. 7. Demonstrate the role of the advanced practice nurse as advocate and initiator of primary care approaches for vulnerable clients. 8. Present client case studies to faculty and fellow students. 9. Collaboratively expand clinical judgment and manage care of complex clients with multi-system diagnoses and advanced or terminal disease. 10. Demonstrate increasing autonomy and accountability in managing the diagnosis, therapy, education, counseling, and consultation roles of the advanced practice nurse. RULES AND GUIDELINES FOR FAMILY NURSE PRACTITIONER (FNP) STUDENT PRACTICUM PLANNING *Marian University s FNP program prepares students to work as nurse practitioners in primary care only. As such, ALL practicum experiences must focus on the provision of primary care to pediatric, adult and geriatric patients in outpatient settings. RULES: 1. Students may use Graduate Nursing Program faculty as preceptors only if the faculty member is not teaching the practicum course. 2. Students may have no more than two (2) preceptors per term and may not add preceptors after the third (3rd) week of the term. These requirements will be strictly enforced unless a major hardship occurs. 3. Students may not spend more than 225 hours with the same preceptor unless it is deemed necessary for the student s success or progression by the Graduate Nursing Program Chair or FNP Program Director. 4. Students should distribute practicum hours evenly throughout the semester; practicum hours may not be completed more than 2 weeks before the end of the semester unless prior arrangements are made with the program director and instructor for an extenuating circumstance. 5. There is not a restriction to the numbers of hours a student can use for a clinical day. There is also no limit to the amount of practicum hours a student can perform in a week. 6. Students can still only use DIRECT PATIENT CARE hours. No hours for travel, prep, etc. 7. Specialty areas that cannot be utilized are emergency departments and any specialty area where a limited range of diagnoses will be seen. 8. Practicums need to occur primarily in outpatient settings. With few exceptions, inpatient settings are not appropriate. [Exceptions to this include a hospitalist rotation (see page 2 of this document), any specialty in which rounds are occasionally made in a hospital (however, at least 70% of the practicum needs to be outpatient), and an extended care facility in which the preceptor performs routine physicals and provides primary care to residents.] 9. Preceptors may be NP, MD, DO or PA. All preceptors must have at least one year of experience. 10. At least one semester s practicum (or 200 hours) must be done with a Nurse Practitioner. 11. Students may use their current employer as a practicum site as long as the practicum setting is not in the area in which the student is currently employed as a RN. 12. The Graduate Nursing Program Chair will give final approval for all practicum sites and preceptors. 13. Practicum placements will be confirmed by the Graduate Nursing Program Chair or Program Secretary via Students may not use immediate family members (spouse, parent, grandparent, sibling, children or inlaws) as a preceptor. 7/28/15 GMK 2/16/16 MF 3 of 5

4 CLINICAL AREAS TO CONSIDER FOR PRACTICUMS: Collaborative clinical practice Migrant clinics Community clinics Nurse-managed clinics Extended care facilities (non-acute) Occupational health settings Group clinical practice/hmos Parish clinics Medically underserved areas Prisons Rural health clinics Specialty practices Urgent care FAMILY NURSE PRACTITIONER PRACTICUM GUIDELINES: NUR 623 Practicum I (150 total practicum hours including 16 hours of classroom time) Acceptable placements for Practicum I include Urgent Care/Quick Care, Family Practice, Internal Medicine NUR 632 Practicum II (225 total practicum hours including 16 hours of classroom time) Based on the student s experience and success in Practicum I, acceptable placements include Urgent Care/Quick Care, Family Practice, or Internal Medicine. A specialty area can be added as a second Practicum experience but it is highly recommended that students complete most Practicum II hours in Urgent Care/Quick Care, Family Practice, or Internal Medicine (see excluded specialties above). NUR 642 Practicum III (225 total practicum hours including 16 hours of classroom time) Based on the student s experience and success in Practicum II, acceptable placements include Urgent Care/Quick Care, Family Practice, or Internal Medicine. Specialty areas except those excluded above are also acceptable for this practicum. ADDENDUM: Students who are in their second or third practicum may do a hospitalist rotation. The primary purpose is for students to be able to participate in the continuum of care and engage in an interprofessional experience. Placement in a hospitalist experience is limited to no more than one-half of the required practicum hours for that semester and this placement requires prior approval from the Graduate Nursing Program Chair. NOTE: The MSN/FNP program at Marian University does not prepare students to work as hospitalist NPs since this type of practice is considered to be acute care and not primary care. FACULTY, STUDENT, AND PRECEPTORT ROLES NURSE PRACTITIONER AND STUDENT CLINICAL PRACTICUMS: The graduate nursing faculty in the Family Nurse Practitioner (FNP) program are committed to the education of advanced practice nurses and view the education process as a collaborative effort that involves the student, the faculty, and the preceptors. A description of the roles and responsibilities of faculty, students and preceptors is provided. Marian University utilizes the core competencies set forth by NONPF (National Organization of Nurse Practitioner Faculty). 7/28/15 GMK 2/16/16 MF 4 of 5

5 Description of Practicum Roles: Faculty Member 1. Assume responsibility for all courses, including the clinical courses. 2. Develop the clinical course syllabus, identify appropriate clinical content, and arrange clinical placement with student input. 3. Provide preceptor with course outlines, clinical objectives, student assignments and evaluation tool. 4. Work closely with students and preceptors to assist students in achieving course objectives. 5. Develop evaluation tools for student performance and learning. 6. Monitor student progress and be responsible for clinical evaluation in coordination with the evaluation by the preceptor. 7. On-going preceptor communication via written correspondence, scheduled clinical supervision of students, conference time and additional time as necessary. 8. Validate student health requirements, liability insurance, and licensure and CPR certification. Student 1. Work with the faculty to arrange clinical placement. 2. Meet with faculty and preceptor to develop and review clinical objectives and arrange the specific clinical schedule. 3. Keep the preceptor informed of required specific educational experiences. 4. Be prepared and punctual for the clinical experience and contact the preceptor if unable to be in the clinical area for any reason. 5. Complete mid-term and final self-evaluations utilizing the NE Student Clinical Evaluation. Student is to build competency during each practicum and from one level of practicum to the next. Learning objectives should be established for each practicum. (forms Learning Objectives) 6. Maintain clinical log. 7. Maintain patient confidentiality 8. Maintain liability insurance, maintain RN licensure, and maintain current CPR certificate. Preceptor and Clinical Practice Sites 1. Serve as host, sponsor, teacher and role model for student at site. 2. Assist the student in searching for learning experiences that will benefit the student in achieving the course objectives. 3. Assume responsibility for the clinical supervision and management of patients receiving primary care from the nurse practitioner student. 4. Negotiate dates and times for clinical experiences. 5. Arrange a substitute preceptor or experience as necessary. 6. Contact the faculty member if a problem occurs with a student. 7. Assist in the evaluation of the NP student s clinical performance. (forms Student Clinical Evaluation Tool) 8. Provide evaluation and feedback regarding the nurse practitioner program, specific courses, student assessments and faculty support. 9.
