1 August Attend Nursing In-service when scheduled by District RN. Place blank copies of Teacher s Clinic referrals (HEA-F037) in teachers mailboxes. Be sure that a Nursing Parent Letter is sent home with each child. Also, be sure that one goes to each new child that registers at your school. Talk with Principal about preferable dates for WellChild, if the program will be at your location. Receipts must be maintained if your school is awarded a year-end gift card. Make copies of every packet that is turned in by student/ parent before giving them to WellChild. Middle School Nurses: Priority is September 15th cut-off date for incoming 7 th grade immunizations. No appointment cards will be accepted after that date. Send home letters to parents of children with lacking immunizations / make telephone calls about lacking immunizations. Document all attempts. Be sure to offer each staff member the chance to take Medication Training (do not forget about Food Service, Custodial, and Educational Assistants). There must be a minimum of two trained per location. Discuss with your Principal. Many of them are asking that all staff be trained. Once they sign up, have them complete the online training in PLAN and check them off once they bring their check-off form to you. Use this for inhalers and EpiPens as well. Diastat training is completed at Central Office by the District RN. Input immunizations in PowerSchool. Be sure to check the box if the immunizations are in compliance (K- Vision / hearing screening dates scheduled for 2 nd and 6 th graders. Start preparations for this (gather volunteers and speak with HS Health Science Teachers for student volunteers- communicate your volunteer information to District RN). Spreadsheets to CSH Supervisor- only 1 spreadsheet per school; school nurse to coordinate with PE Dept for BMI and BP info. Follow timeline provided by District RN. Print / daily schedule to District RN by August 14th. Include Daily medications (and procedures) and PRN medications (and procedures). HEA-F098 and HEA-F099 are to be used. Be sure to include in Notes Section whether or not the child needs Nursing Assistance. Students with Medical Conditions- send home HEA-F107 to each student. Follow the protocol of HEA-W001 if action When you receive IHP s from District Registered Nurse, make a copy and place in a folder in ABC order by last name, Gather list of employees at your school willing to be trained in Diastat administration. Turn list in to District Registered Nurse. Gather carb count listing for students with diabetes. List to be provided by District RN. Post sign-up list for Flu Vaccination for School Staff (do not forget about Classified Staff, including our Food Service, Custodial, Ed Assts, etc). Turn in list to District RN. Please check all expiration dates prior to students returning to school on the half day. Ask your Principal for a copy of the completed SAF-F017 (Faculty and Staff Roster) to attach to the outside of all GoKits at your location. Verify location of all GoKits at your building. Be sure all staff know the location of all GoKits in building. Post a sign-up sheet for Flu Vaccinations (staff) once received from District RN Flu vaccine delivery dates and administration of vaccine dates are to be announced at a later date. Send School Nurse Monthly Clinic Totals Form to District RN prior to the working day of the month. Student Accident Forms- send the completed ones to Risk Management/Safety Dept. after putting them in ABC order by month. Medication disposal- follow protocol. There shouldn t be any leftover medications in your clinic. Biohazard- be sure all biohazard is disposed of prior to leaving for the school year. 5 Send home Asthma, Seizure, Diabetes, and/or Severe Allergy packets home to all students who may have these identified in PowerSchool to use for the next school year. Send home HEA-F062 with children with medication, to use for the next school year. Send home Self- Medication forms as well for students that self-medicate (HEA-F059).
