4 Introduction The (CHIS) was created by NH state statute to make health care data available as a resource for insurers, employers, providers, purchasers of health care, and state agencies to continuously review health care utilization, expenditures, and performance in and to enhance the ability of consumers and employers to make informed and cost-effective health care choices. The statute also required that the New Hampshire Insurance Department (NHID) and the NH Department of Health and Human Services (NH DHHS) partner on the project. The same legislation that created the CHIS also enacted statutes that mandated that health insurance carriers submit their de-identified health care claims data and Health Employer Data and Information Set (HEDIS) data to the state. NH DHHS, Office of Medicaid Business and Policy, after a competitive bid process, contracted with Milliman in June 2012 to assume maintenance of the CHIS. Under the contract, Milliman is acting as DHHS s agent for the collection of claims data, and is providing a series of reports and studies for DHHS that examine the NH Medicaid program in concert with using the commercial data for benchmarking, and is hosting this website. The purpose of this document is to explain the process and data requirements for submitting data to Milliman on behalf of the CHIS program. 5 Background State of statutes mandate that all health insurance carriers and third party administrators (health care claims processors) submit electronic claims data that pertain to members or subscribers who receive their benefits under a policy or plan issued in. A policy that is issued in New Hampshire shall include any policy that provides coverage to the employees of a employer that has a business location in. An out-of-state employer's branch location in shall be considered a employer, and the carrier shall submit a claims data set for all members who are employed at that branch location. Third party payers that write less than $250,000 in accident and health insurance premiums in New Hampshire on an annual basis shall not be required to submit their health care claims data sets. Third party administrators that administer health insurance plans covering fewer than 200 lives in total are also not required to submit their health claims data. Each health care claims processor shall also submit all health care claims processed by any subcontractor on its behalf. In instances where more than one entity is involved in the administration of a policy, the health carrier shall be responsible for submitting the claims data on policies that it has written, and the third party administrator shall be responsible for submitting claims data on self-insured plans that it administers These claims data must be submitted in accordance with Insurance Department (NHID) rules, Chapter Ins 4000 Uniform Reporting System for Health Care Claims Data Sets. The rules should be read carefully in order to fulfill the regulatory requirements of the NHID and to have the data submission process operate most efficiently. The rules define the reporting requirements (thresholds, exclusions, filing frequencies, file formats, data element definitions and codes, etc.) for claims data submitted by all licensed health care claims processors. Patient identifiable data such as member names, specific postal addresses, social security numbers, etc. will automatically be hashed by the NHpreprocessor to de-identify prior to data submission The DHHS has entered into an agreement with Milliman to collect and process the data. Milliman is strictly prohibited from collecting any social security numbers or direct identifiers and from releasing or using data or information obtained in its capacity as a collector and processor of the data for any purposes other than those specifically authorized by the agreement. The agreement provides that Milliman shall transmit all data that it collects and processes to the NHID and the DHHS. 7 Registration Requirement Registration Any health care claims processor meeting NH s health care claims data submission requirements must register with Milliman prior to submitting any data files. All health care claims processors currently submitting data files must re-register with Milliman by March 15, 2013, and by March 15th of each year thereafter. Each health care claims processor and each carrier shall submit a registration form to the NHID and the DHHS, or Milliman. Some of the required fields are outlined below: Company name; NAIC code; Mailing address; Information about whether the company conducts health insurance related business; Number of members covered; Information about claims adjustments; Information about plan service carve outs; Identification of any missing required data elements; and Name, address and address of the persons, submitting data, involved in data compliance and completing the form. 8 Overview of the Data Submission Process: Follow the following steps: 1. Complete and submit the on-line registration form. 2. Generate data extracts including all required data elements and formats in accordance with New Hampshire Insurance Department (NHID) rules, Chapter Ins 4000 Uniform Reporting System for Health Care Claims Data Sets. These requirements are included in subsequent sections of this manual. 3. Process extracts through the NHpreprocessor. This application and the associated user guide will be ed to you after you complete and submit the registration form. If you have not received the application, please send a request to [email protected]. 4. Submit data processed through the preprocessing application to Milliman, preferably through the secure file transfer server using your SFTP account. This account will be ed to you when you complete the registration form. If you need assistance, please send an to: [email protected]. Alternative methods for submitting data are also provided in the Directions for Submitting Data section of this document. 5. Milliman will conduct data quality audits on the submitted data and complete the processing for the NH CHIS data warehouse. If any issues are found with the submitted data that prevent complete processing and integration into the NH CHIS data warehouse, Milliman will contact the designated contact via . 9 Processing Data Through the NHpreprocessor Milliman will the NHpreprocessor application and user guide when you have completed and submitted the on-line registration form. If you have not received the application, please send a request to [email protected]. The user guide will be ed with the application, but the contents of the user guide are also included here. Introduction The NHpreprocessor is used to hash ASCII files that contain health care claims data that will be submitted to the state of, Department of Health and Human Services (DHHS). The utility hashes the specified ASCII files, creating an output ASCII file and a zip file. Non-ASCII files are not supported. System Requirements This utility is intended for use on the Windows operating systems listed below. Regardless of the operating system, the system must have the most recent service pack installed. Also, the utility requires that the system have the Microsoft.NET Framework 4.0 installed. Windows XP Windows Server 2003 Windows Server 2008 R2 Windows 7 Installation Instructions The user must run the setup program using an account that has local administrator rights. If the system does not have the.net Framework 4.0 installed, the setup program notifies the user to install that framework before allowing the user to continue. Once the.net Framework 4.0 is installed, the user may follow the following steps to install the utility. 1. Copy the NHpreprocessor setup program to a local drive. 2. Right-click on the program, waiting for the context-menu to appear. Confidential & Proprietary 6
10 3. Launch the program with Run-As-Administrator rights. 4. The User Account Control dialogue box appears. Click on the Yes command button. 5. The Setup NHpreprocessor dialogue box appears. Click on the Next command button. 6. The next dialogue box allows the user to specify where the utility is installed. Once you have specified the folder, click on the Next command button. 11 7. When the next dialogue box appears, click on the Install command button. 8. When the final dialogue box appears, click on the Finish command button. Using the Client Utility The user may run the client utility using an account that has standard user rights. The program may be launched by clicking on Start-> All Programs -> MedInsight -> NHpreprocessor. Once the utility has been launched, the user is presented with the following form. 1. Click on the Select File to Be Encrypted command button to specify the input file. 2. Click on the Select Output Directory to specify the folder where the output file and log file will be created. Note that the utility will fail if the user does not have modify rights on the folder selected. 3. Click on the Encrypt File command button to process the file. Confidential & Proprietary 8
12 4. When the utility successfully processes the file and creates the zip file, it presents the user with the dialogue box shown below. 5. The utility names the output files using the following naming convention: FileTypeProjectNaicBegin_EndVersion. (e.g., MCNHABCDEVGH200901_ txt). Data files are given a.txt extension and zip files are given a.zip extension. 13 Using the Batch Utility If the user wants to automate the creation of the output file and zip file, the user may run the batch utility NHpreprocessbatch.exe. The user may run the utility using an account that has standard user rights. The utility takes three parameters: the name of the input file, the full path where the input file is located, and the full path where the utility must create the output files. The utility returns 0 if it completes the hash process successfully. Otherwise, it returns a non-zero value. The following is an example of a batch command: NHPREPROCESSBATCH NHMCTEST1.TXT C:\PROGRAMDATA\MEDINSIGHT\INPUT C:\PROGRAMDATA\MEDINSIGHT\OUTPUT Trouble Shooting The client utility presents the user with an Error Processing File dialogue box if the data in the file fails one or more data checks. The information presented in the dialogue box can be used to trouble shoot the data issue. Also the client utility and the batch utility create a log file with each run, which is stored in the output folder specified by the user. The log file contains additional information that may be useful in trouble shooting data issue. 14 Directions for Submitting Data Secure SSL Web Upload Interface Accessing MedInsight Secure Transfer Server via Internet Browser All registered data suppliers will receive a letter from Milliman with their username. Passwords will be provided in a separate communication. Although most browsers are supported (including Internet Explorer, Firefox and Chrome), Internet Explorer is the preferred browser. From an Internet browser, go to the address and log in with your username and password: Once logged in, you will be within your home directory. You can either stay there or navigate to other directories (if available). To transfer (download) files from the server to your local computer, simply find and double-click the file you would like to transfer. You can also multi-select files by holding the CTRL button and selecting files individually, or holding SHIFT and clicking on the first and last file you would like to transfer. To transfer (upload) files to the server, click the Upload button at the top of the page and click Browse, find the file you would like to upload, click OK and then click the Upload button: Confidential & Proprietary 11
15 Electronic Transmission through a File-Transfer Program Accessing MedInsight Secure Transfer Server via FTP Client As an alternative to the Internet browser method above, an FTP client that supports SFTP can be used. This may be desirable in cases where transfers need to be automated or when more flow control is needed (such as the ability to resume a transfer if it failed for some reason). Additionally, transfer speeds are generally better when using a client. All registered data suppliers will receive a letter from Milliman with their username. Passwords will be provided in a separate communication There are many different FTP clients available that suppor SFTP. FileZilla is one example and it is free. 16 CD-ROM or DVD-ROM Mailing Information Files on physical media can be mailed to: Natasha Rosenblatt Milliman 1301 Fifth Avenue Suite 3800 Seattle, WA External Label Carriers and health care claims processors submitting data on physical media shall affix an external label to CD-ROM or DVD on which data are sent that includes: 1. Health plan name; 2. NHID submitted code; 3. Contact person name; 4. Contact person address; 5. Contact person telephone number; 6. Contact person address; 7. Shipping date; 8. Unique tracking identifier number for each file; 9. Period beginning date; 10. Period ending date; 11. Record count; 12. Date proposed; 13. Submission date; and 14. Any date of resubmission. Transmittal Sheet All data file submissions on physical media shall be accompanied by a hard copy transmittal sheet containing the following information: 1. Identification of the health care claims processor; 2. File name; Confidential & Proprietary 13
17 3. Type of file; 4. Data period(s); 5. Date sent; 6. Record count(s) for the file(s); and 7. Contact person with telephone number and address. The information on the transmittal sheet shall: Match the information on the header and trailer records; and Conform to the following layout: NHID Data Transmission Form Payer Name: NHID Submitter Code: Contact Person Name: Address: Telephone: File Name Eligibility Medical Prescription Drugs Period Beginning Date Period Ending Date Record Count Date Processed Original Submission Resubmission Testing of Files At least 30 days prior to the initial submission of the files, each carrier or health care claims processor shall submit to the NHID and the DHHS, or Milliman, a data set for determining compliance with the standards for data submission. The size, based upon a calendar period of one month, or quarter of the data files submitted shall correspond to the filing period established for that health care claims processor. Health care claims processors using non-conforming local/homegrown CPT and/or diagnosis codes must submit those codes with descriptions in MS Excel format prior to the first data submission. to: [email protected]. 18 After the files are loaded, a validation process is performed on each data file, with results being generated within five days. Load threshold levels for individual data elements are validated against those defined by the NH Department of Health and Human Services (NH DHHS). File submissions failing to meet one or more of the thresholds will be rejected by the system. An will be generated notifying the health care claims processor that their submission has been rejected and listing the data files and elements that did not meet the load thresholds. A listing of all default load threshold will be available on the NH CHIS website in the near future. Note there have been no changes in the thresholds from prior submissions. Status of Data Submissions Within five days of submittal of the data files, a report that provides detailed results of the validation process will be created for each health care claims processor. The report will indicate which files have passed and which files have failed. If a file has failed, the report shall also indicate the specific data element(s) that caused the failure, including an explanation for the failure and a suggested solution, where appropriate. The status of your data submissions will be available on the NH CHIS website in the near future. Rejection of Files Failure to conform to the requirements for submission shall result in the rejection and return of the applicable data file(s). All rejected and returned files shall be resubmitted in the appropriate, corrected form to the department and Milliman, within 10 days. If it is determined that a health care claims processor may not be able to meet one or more of the load thresholds and/or quality edits due to the data being excluded from the claims transaction process, an element level threshold variance may be granted. All variances must be approved by the NH DHHS. Filing Periods The filing period for each claims data file listed shall be determined by the total number of covered lives who are residents for whom claims are being paid or processed by each carrier or health claims processor. For those carriers or health care claims processors having 2,000 or more New Hampshire covered lives, data shall be submitted monthly. For those carriers or health care claims processors having fewer than 2,000 covered lives, data shall be submitted quarterly. Replacement of Data Files No carrier or health care claims processor shall replace a complete data file submission more than one year after the end of the month in which the file was submitted unless it can establish exceptional circumstances for the replacement. Any replacements after this period shall be approved by the NHID. Individual adjustment records shall be submitted with a monthly data file submission. Confidential & Proprietary 15
19 Compliance with Data Standards Compliance Milliman shall evaluate each member eligibility file, medical claims file and pharmacy claims file to determine compliance with the following data reporting requirements: Note: If exceptions were approved for specific data elements on your previous data submission by the State of New Hampshire or the previous data collector, those exceptions will continue to be approved until further notice. 1. The applicable code for each data element shall be included within the eligible values for the element; 2. Coding values indicating data not available, data unknown, or the equivalent shall not be used for individual data elements unless specified as an eligible value for the element; 3. Member sex, diagnosis and procedure codes, and date of birth and all other data fields shall be consistent within an individual record; and 4. Member identifiers shall be consistent across files. Notification Upon completion of the evaluation, the NHID and the DHHS, or Milliman shall promptly notify each carrier or health care claims processor whose data submissions do not satisfy the standards. This notification shall identify the specific file and the data elements that do not satisfy the standards. Response Each carrier or health care claims processor notified of a non-compliant data submission shall respond within 60 days of the notification by making the changes necessary to satisfy the standards. 20 General Data Requirements The following are the general data requirements: 1. Adjustment records. Carriers and health care claims processors shall report adjustment records with the appropriate positive or negative fields with the medical and pharmacy file submissions. Negative values shall contain the negative sign before the value. No sign shall appear before a positive value. 2. Capitated services claims. Claims for capitated services shall be reported with all medical and pharmacy file submissions. 3. Data fields. Carriers and health care claims processors shall make every effort to report the data fields outlined in these requirements if the data field is present in any part of their data systems. Carriers shall submit data fields even in circumstances where the data is integrated from multiple systems. However, if a field is not available for submission, or cannot be derived reliably from other information available on the carrier's transaction system, the health plan shall notify the NHID and the DHHS, or their designee, and shall identify the field that cannot be provided. After notification, the carrier shall not be required to populate that data field in its reports. The carrier shall report on an annual basis its efforts to populate this field, and the expected date as of which this field will be available, if there is such a date. 4. Claimant and member records. Claims records and member records for medical and pharmacy claims shall be reported if either selection criteria are met: enrollees that are residents of New Hampshire and for all members who receive their benefits under a policy or plan issued in New Hampshire. A policy that is issued in shall include any policy that provides coverage to the employees of a employer that has a business location in New Hampshire. Claim records. Records for medical and pharmacy claims file submissions shall be reported at the visit, service, or prescription level. The submission of the medical, and pharmacy claims shall be based upon the paid dates and not upon the dates of service associated with the claims. 6. Code sources. Unless otherwise specified, the following code sources are to be utilized in association with the member eligibility file and medical and pharmacy claims files submissions. The required source codes are found in Appendix I. If codes specified in these rules are updated by the code source, whether the update includes new codes or a modification of descriptions, the changes provided by the source preempt the definitions and descriptors provided in these rules. 7. Member Identification Codes. Carriers and health care claims processors must provide a unique identifier for each of their covered members. The NHpreprocessor application will hash all member/subscriber identification codes and names before data are transmitted to Milliman. To ensure consistent hashing, member identifiers should not be encrypted or hashed on the initial extract loaded into the preprocessor. If a health care claims processor does not collect the social security numbers for its members, the health care claims processor shall provide the social security number of the subscriber and assign a discrete two digit suffix for each member under the subscribers contract using the following criteria: Confidential & Proprietary 17
21 a. If the subscriber's social security number is not collected by the health care claims processor, the subscriber's certificate or contract number shall be used in its place(this data element will be de-identified by the NHpreprocessor application). b. The discrete two digit suffix shall also be used with the certificate or contract number (this data element will be de-identified by the NHpreprocessor application). c. The certificate or contract number with the two digit suffix shall be at least 11, but no more than 30 characters in length (this data element will be de-identified by the NHpreprocessor application). d. The unique member identification code assigned by each carrier and health care claims processor shall remain with each member for the entire period of coverage for that individual. 8. Specific/Unique Coding. With the exception of provider codes and provider specialty codes, specific or unique coding systems shall not be permitted as part of the health care claims data set submission. 9. Co-insurance/Co-payment. Co-insurance and co-payment are to be reported in 2 separate fields in the medical and pharmacy claims file submission. 10. Coordination of Benefit Claims. Claims where multiple parties have financial responsibility shall be included with all medical and pharmacy claims file submissions. 11. Version Number. When more than one version of a fully-processed claim service line is submitted, each version of a claim service line shall be enumerated sequentially with a higher version number (MC005A) so that the latest version of that service line is the record with the highest version number (MC005A) and the same claim number + line counter. a. Fully-Processed Claim Lines: Only fully-processed claim service lines that have gone through an accounts payable run and been booked to the health plan ledger shall be included on medical and pharmacy claims data submissions. b. Denied Claims. Denied claims shall be excluded from all medical and pharmacy claims file submissions whenever possible so that when a claim contains both fully-processed paid service lines and partially processed or denied service lines, an effort shall be made to include only the fully-processed, paid service lines as part of the health care claims data set submittal. c. Subsequent Incremental Claims. Subsequent incremental claims submissions shall include all reversal and adjustment/restated versions of previously submitted claim service lines and all new, fully-processed service lines associated with the claim, provided that they have paid dates in the reporting period: i. Each version of a claim service line shall be enumerated sequentially with a higher line version number (MC005A); and ii. Reversal versions of a claim service line shall be indicated by a claim status code = '22' (Field MC038). Confidential & Proprietary 18
22 d. Global Payment Arrangements. If a claim contains service lines that have been denied because their costs are covered on another line of the claim line, such as under a global payment arrangement, those denied line(s) shall be: i. Included in the data submission; and ii. Clearly indicated by a claim status code = '04' (Field MC038). e. Exclusions of Denied Claims or Service Lines. Carriers and health care claims processors that are unable to exclude denied claims or service lines without compromising the completeness of their claims submission may submit all versions of fully-processed paid and denied claims service lines, provided that lines and versions thereof are clearly indicated by a claim status code = '04', and the line version number is sequentially noted on any reversal and adjustment versions of those lines to clearly indicate the order in which all changes to these lines were processed 12. Prepaid Amount. Any prepaid amounts shall be reported in a separate field in the medical and pharmacy claims file submissions. Confidential & Proprietary 19
23 General File Specifications File Formats Each data file submission shall be an ASCII file, variable field length, and asterisk delimited. When asterisks are used in any field values, they shall be enclosed in double quotes. Header and Trailer Records Each member eligibility file and each medical claims file, and pharmacy claims file that is submitted shall contain a header record and a trailer record. The "Header record" means the first record of each separate file that is submitted and the "Trailer record" means the last record of each submitted file. The header and trailer record format shall conform to the following record specifications: 1. Record Specifications. Health care claims processors and carriers shall use the following record specifications in submitting their claims records: a. The file header record layout shall be submitted using the following data elements: i. HD001. This element is named "record type". The data type of this element is text. Its length is 2. ii. HD002. This element is named "payer". The data type of this element is text. Its length is 8. Carriers and health care claims processors shall code according to payer submitting payments, NHID submitter code. iii. HD003. This element is named "National Plan ID". The data type of this element is text. Its length is 30. Carriers and health care claims processors shall code according to CMS National Plan ID. iv. HD004. This element is named "type of file". The data type of this element is text. Its length is 2. Carriers and health care claims processors shall code according to ME member eligibility, MC medical claims, PC pharmacy claims, DC Dental claims. v. HD005. This element is named "period beginning date". The data type of this element is integer. Its length is 6. Carriers and health care claims processors shall code according to CCYYMM, beginning of paid period for claims, beginning of month covered for eligibility. vi. HD006. This element is named "period ending date". The data type of this element is integer. Its length is 6. Carriers and health care claims processors shall code according to CCYYMM, end of paid period for claims, end of month covered for eligibility. vii. HD007. This element is named "record count". The data type of this element is integer. Its length is 10. Carriers and health care claims processors shall code according to total number of records submitted in this file, with the header and trailer record excluded from the count. 24 viii. HD008. This element is named "comments". The data type of this element is text. Its length is 80. Carriers and health care claims processors shall code according to their own option. b. 48 Detailed Medical Claim File Requirements General Medical Claim File Requirements Notes 1. If exceptions were approved for specific data elements on your previous data submission by the State of or the previous data collector, those exceptions will continue to be approved until further notice. 2. Carriers and health care claims processors must provide a unique identifier for each of their covered members. The NHpreprocessor application will hash all member/subscriber identification codes and names before data are transmitted to Milliman. To ensure consistent hashing, member identifiers should not be encrypted or hashed on the initial extract loaded into the preprocessor. 3. Medical Claims File Exclusions. Claims for stand-alone insurance policies shall be excluded if the stand-alone coverage is provided for the following types of services: a. Specific disease; b. Accident; c. Injury; d.
