3 Program Overview Delta Rural Access Program (DRAP) was established by Tombigbee Healthcare Authority (THA ) and its consortium members to address the health care challenges and diminishing resources experienced by residents residing in rural Alabama. To address these challenges, the THA sought and received funding in 2007 from Health Resource Service Administration (HRSA) to develop and implement the Delta Rural Access Program (DRAP) initiative and began addressing the unmet heath care needs in 18 of Alabama's rural Delta counties. Because the of the essential need for collaboration in a era where healthcare continue to change, THA formed a partnership with several key organizations who showed compassion and commitment to serve their communities to assist THA in developing and executing innovative projects that would; 1) provide preventative services for individuals at-risk of developing chronic health diseases; and 2) increase access to prescription drugs for the medically indigent. Since the program inception, we accomplished much, but became aware that we could have a greater impact on the health of our residents by intensely focusing our efforts on chronic health diseases prevalent among our target population. Recognizing that diabetes in the Delta counties is increasing at an alarming rate, the DRAP community of partners united in their desire to address diabetes utilizing a comprehensive diabetes prevention and management program.
4 PROGRAM GOALS AND OBJECTIVES The overarching goal of DRAP 2013 is to reduce the disease and economic burden of diabetes, and improve the quality of life for all persons who have or are at risk for developing type 2 diabetes. The objectives is to support these individuals in making life-style changes to prevent or reduce the impact of diabetes through education, case management, and pharmacy assistance.
5 TARGET POPULATION The target population will include residents of the Delta Region of Alabama who are between the ages of 19-65: who are at-risk of developing Type 2 Diabetes as indicated by their family history, social behavior, or who are overweight and inactive; and who present to the hospital in Marengo and Monroe Counties with type 2 diabetes complications that require case management. These Delta counties are rural and sparsely populated with two contiguous counties as far as 30 miles apart; largely minority (83.0% black), (13.0% Caucasian) and (4.0% Hispanic).
6 Geographic Coverage Area The DARK Shaded areas on the map below represents the Alabama Delta Regions coverage areas According BRFSS, a data collection system supported by the CDC, the prevalence of diabetes in Alabama Delta Counties increased in 2010 when compared to 2004 data. 7 Program Approach What Will We Do DRAP will provide residents in the Alabama region access to diabetes prevention classes, case management patient navigation services, prescription drugs access, and health screenings Where Will We Do It Diabetes prevention classes will be offered in a setting like churches, schools, health care provider, and business, as well as in community base organizations. Who Will Do It 9 partners consisting of hospitals, mental health agencies, county extension offices, and communitybased organizations will provide the services in 18 of Alabama s counties.

10 Evidence Base Curriculum Selection 1 Diabetes Prevention Power to Prevent: A Family Lifestyle Approach to Diabetes Prevention was developed to help bring diabetes prevention and control to African American communities. National Diabetes Education Program (NDEP) ) 2 Case Management Stanford School of Medicine Diabetes Self-Management Program Curriculum (DSMPC), a nationally recognized DMP proven to redirect the course of the diabetes epidemic and build confidence that will yield reduced rates of disease in the future. 3 Pharmacy Assistance Rx Assist Plus: An electronic mechanism, funded by Robert Wood Johnson, sustained and made available to the nation via web based access. Rx Assist Plus is to designed to locate medication at little to no cost, with features that effectively manage and track data for reporting.
