2 A Case Report on Treatment of Ovarian Cyst via Faradarmani Introduction Ovarian cysts are fluid filled sacs in ovaries. Most of them are functional (non pathologic) and recover through easy treatments. Functional cysts include two groups; luteal and follicular. If the cyst is large, resistant to treatment or painful; surgery is necessary. Sometimes surgery leads to total ovariectomy. For pain relief oral analgesics are used and to prevent new cyst formation, OCP (oral contraceptive pills) are used. Usually cysts disappear within 8 weeks or by two menstrual cycles. If the cysts persist they may be pathologic rather than physiologic. Regarding the fact that ovarian cancer is a major cause of death in women, the probability of malignancy is always a concern where ovarian cysts exist. Ovarian tumors can be seen in sonography in the forms of cystic or multi-vacuolated. The probability of malignancy increases when the inner septums of cysts are thick. Case Report The patient is a 23 year old single girl with a history of irregular painful menstrual periods and hair loss which began since 2003 (1382). After sonograghy of uterine and ovaries she was diagnosed as a P.C.O (polycystic ovarian syndrome) case and was treated with Cyproterone compound. On 29/08/2004 (1383/06/08) sonograghy; the endometrium thickness and dimensions of uterine were normal, ovaries were slightly enlarged and multiple cysts were observed in peripherals. Based on these results PCO was again confirmed (Appendix 1). After few months, she sought medical advice again with the same symptoms and diagnosis, the same medication was prescribed by her physician. Through this evaluation on 26/05/2005 (84/03/05) the uterine and adnexa s size were normal and only in left ovary a simple cyst was seen with dimensions of 30*18 mm (Appendix 2). In the next assessment on 17/04/2006 (85/01/28) the size and thickness of endometrium were normal again but the right ovary contained a cyst with the size of 36*30 mm and the left ovary contained a 18*16mm follicle (Appendix 3). Para clinical findings such as CBC, hormonal and thyroid function tests were all normal with no evidence of anemia (Appendix 4). Unfortunately as described above; for 3 years this scenario was repeated over and over again without any definite treatment; that is to say the patient was referred to sev- 1

3 eral physicians and they took the same approach (cyst diagnosis, a treatment period and then sonography) until 2008 (early 1387) when the cysts and symptoms yet remained untreated. Because of the treatment failures and being tired of recurrent referrals to physicians, being reluctant to take more medication and fear of surgery; the patient stopped taking her medication. In April 2008 (Ordibehesht 1387) she was introduced to Faradarmani and about one month after attending the classes and trying Faradarmani, the symptoms disappeared, her menstrual periods became regular, hair loss and pain ceased and she became totally healthy. For further evaluation another sonograghy was done on 16/12/2008 (87/8/26) in which no cyst or tumor was seen, this indicates that the former cysts disappeared (Appendix 5) and now she has no symptom or pain and is in good state of health. Discussion In the view that in this patient non- hemorrhagic ovarian cysts have been resistant to the classic medical treatment, and also considering the large size of cysts; surgery is the only definite treatment method, and also because in drug resistant cysts the probability of malignancy exists in long term, assured treatment of such patients are mandatory. Fortunately in this patient after disappointments from classic medical treatment methods, successful treatment was easily achieved through Faradarmani without any side effect. Conclusion This report shows that Faradarmani can also be effective in treating chronic diseases in a short period of time. Just month after beginning this treatment, the whole clinical symptoms and para-clinical tests became normal and the patient was relieved from tension and anxiety. 4 Radiography and Sonography Clinic Date: 29/08/2004 (1383/06/08) Age: 19 Sonograghy of the Uterine and Adnexa The dimensions of Uterine are 70*33*39 mm with normal size and echogenicity. There is no lesion in parenchyma. Endometrium thickness is 6mm and normal. There is no [pathologic] lesion in adnexa. The ovaries are slightly large and multiple small cysts are observed in peripheral regions. Right ovary dimensions are 35*19 mm, left ovary dimensions are 40*19 mm. Comment: Sonograghic view of ovaries is consistent with PCO. Appendix 1 Sonography Centre Date: 26/05/2005 (84/03/05) Age: 20 Sonograghy of the Uterine and Adnexa The dimensions of uterine in mid-sagital section were measured as 75*31 mm. The shape and echogenicity of the uterine parenchyma is normal. The thickness of endometrium is 10 mm. The size and echogenicity of Right ovary is normal. The dimensions of Right ovary are measured as 34*15 mm. In the left ovary there is an image of a simple cyst with dimensions of 30*18 mm. There is no free fluid in pelvis. Comment: Left ovarian cyst. 5 Radiography and Sonography Clinic Date: 17/04/2006 (85/01/28) Age: 21 Sonograghy of the Uterine and Adnexa The uterine with dimensions of 36*85 mm has normal size and echogenicity without any pathologic lesion in the parenchyma. The thickness of endometrium is 8mm that is normal; there is no lesion in adnexa. Right ovary contains a cyst with the dimensions of 30*36mm. The left ovary is 35*26mm which has normal shape and echogenicity and contains a 16*18mm follicle. Comment; Right ovarian cyst. The dimensions of uterine are within normal ranges (45*37*76). The External margins of uterine are smooth and there is no space occupation lesion in myometrium. The echogenicity of myometrium is uniform. The thickness of endometrium is 7mm and normal. The ovaries have normal echo and dimensions. There is no tumor or cystic lesion. There was no fluid in cul-de-sac. Comment; Disappearance of the former ovarian cyst. Appendix 5 Appendix 6 5
