1 Montgomery County Office of Developmental Disabilities Transition Guide The purpose of this Guide is to inform individuals with a diagnosis of Intellectual Disabilities, and families, how to access supports and services. The following are the essential steps to begin the process: The Office of Developmental Disabilities Registration Information Contact the Montgomery County Intake Coordinator at for more information about eligibility requirements for services. Individuals, family members or legal guardians must contact the Intake Coordinator. Schools or other agencies can not register an individual. Registration may start as early as 3-years old. Supports and Services After you are determined eligible, you may choose a Supports Coordinator Organization (See page 5) to help you identify what supports best meet your needs. Some of the services that are available include Supports Coordination, Employment Supports, Habilitation, Respite, Day Services, Transportation, Residential Services and Vehicle Adaptations. Services are delivered by service providers. Providers, services and county contact information may be accessed at County Assistance Office (CAO) Application and Financial Eligibility Information Qualification for medical assistance is important to qualify for different types of funding through The Office of Developmental Disabilities. To learn if you qualify for medical assistance and other benefits, contact the County Assistance Office (CAO) at Other Benefits/Resources Social Security Administration (SSA) Application Information: To learn if you qualify for social security disability income (SSDI) or social security income (SSI), contact the Social Security Administration (SSA) at The Office of Developmental Disabilities publishes a Newsletter to provide information about the DD System; in addition, Community Outreach events are held throughout the County at various locations. pgolden@montcopa.org for current information.
