9 Description: A Community-Based Residential Facility (CBRF) is a state-licensed*, group-living facility where five or more unrelated adults reside and supportive services are provided. These facilities most commonly provide assistance with personal cares, such as: bathing, dressing, meals, housekeeping, laundry, medication monitoring, and social activities. They serve a variety of different groups of people, including adults who are older, have Alzheimer's disease & dementia, a developmental disability, or a mental illness. Payment: Insurance, Medicare, and Medicaid do not pay for room and board at CBRF's. In Brown County, some adults may be eligible for as a payment option. To learn more about the program and eligibility, contact ADRC of Brown County. Please note: Acceptance of funding is at the discretion of each facility. There is no guarantee facilities listed as accepting have beds available at all times. Mandatory Services: 1. Program statement developed and available to consumers, detailing the program goals of the facility that are consistent with the needs of the residents to be served, plus a full description of care & services to be provided and by whom. 2. Develop, with the resident's input, an individualized service plan that outlines the resident's needs and wants, with goals, indicating the services and activities to be provided that will achieve these goals. 3. Monitoring resident health status and reporting changes in condition to appropriate health providers, and obtaining physician orders for care, medications, rehabilitation services, therapeutic diets and treatments. 4. Store and administer medications, according to physician orders, unless the resident chooses to and is capable of doing this her/himself. 5. Three meals a day served family or restaurant style, plus snacks. 6. Promote in-house and community leisure time activities. 7. Transportation options (provided or arranged) to community activities, employment, medical appointments, education programs, and religious services. 8. Trained staff (minimum of 45 hours of initial training that includes: first aid, infection control, medication management, fire safety, resident rights, working with specific residents that are served, etc.). 9. Cannot provide more than three hours/week of hands-on, skilled nursing care provided by a Registered Nurse (RN). Advocacy: The Wisconsin Board on Aging & Long-Term Care Ombudsman Program provides advocacy services to residents 60 years of age and older. 10 Licensing: CBRF's are licensed, classified, and regulated by the Bureau of Nursing Home Resident Care and Wisconsin Department of Human Services (DHS). Licenses are renewed every 24 months (fee collecting process with brief review of license renewal application). Facilities are inspected onsite approximately every two years (unannounced and separate from license renewal). They are required to investigate complaints of a regulatory nature and residents rights are protected in the regulations. There are six different classifications, but all of the facilities on this listing are classified at the highest level: Class C Non-Ambulatory (CAN). They serve residents who are ambulatory, semi-ambulatory, or non-ambulatory, but one or more of whom are not physically capable of responding to an electronic fire alarm & exiting the facility without any help (verbal and physical prompting). Find inspection summaries and Statements of Deficiencies (SODs) for CBRFs across the state of Wisconsin (search by zip code): Dementia- Things to Consider 1. Does the program statement indicate the facility serves people with dementia? 2. Does the facility call itself dementia specialists? What makes them so special? 3. Does the facility seek guidance from the Alzheimer's Association? 4. What type of training has the staff received concerning dementia? Has all of the staff been trained? 5. Is the staff able to handle difficult or challenging behaviors? 6. How many staff members are there? How many residents have dementia? (What is the staff: resident ratio?) 7. Is staff awake at night? 8. Are physical or chemical restraints used? 9. Is the environment safe for a person with dementia? 10. Are the activities appropriately structured for individuals who have dementia or who experience "sundowning" (awake at night)? 11. At what point will the facility refuse to keep the person?
