3 SCOPE AND APPLICATION This program applies to the control of entry into and work within confined spaces at all University of Nevada properties. The purpose is to establish basic requirements to protect employees and students from the hazards involved with confined spaces, thus preventing injury or death to University personnel, students, or contractors. This program adheres to 29 CFR , permit-required confined spaces. ROLES AND RESPONSIBILITIES Buildings and Grounds (Facilities Management) 1. Obtains initial confined space training from EH&S approved curriculum and provider. Obtains refresher training as necessary from EH&S. 2. Completes and maintains Permit-Required Confined Space Permit for each permit-required entry. 3. Provides personnel to perform entry supervisor, attendant, and authorized entrant duties as outlined in this program. 4. Maintains and calibrates confined space monitor for use in all permitrequired confined space entries. Environmental Health and Safety Department (EH&S) Contractors 1. Oversees overall maintenance of the Confined Space Program. 2. Provides technical support and guidance for the Confined Space Program. 3. Provides air monitoring support for Building and Grounds personnel when primary confined space monitor is non-functional or when additional contaminants are anticipated. 4. Provides refresher training to Buildings and Grounds personnel as necessary. 1. Submits contractor confined space program to EH&S for review and approval prior to permit-required confined space entry. 2. Coordinates with Buildings and Grounds on all permit-required entries in accordance with this program. UNR Permit Required Confined Space Program Page 3 of 18 02/10/06

4 City of Reno Fire Department 1. Provides emergency and confined space rescue services. 2. Meets rescue requirements of 29 CFR PERMIT SYSTEM Entry into identified permit-required confined spaces requires completion of the entry permit located in this document. The permit is valid for one uninterrupted entry and must be posted at the entry site during entry. After entry is complete, a copy of the permit is to be filed with Buildings and Grounds. Both the permits and the confined space entry program will be reviewed and evaluated by the Environmental Health & Safety Department on an annual basis or when potential deficiencies are recognized. The following are general procedures to serve as guidance in filling out the permit PRIOR to making an entry: 1. Fill in location and/or confined space number, work to be performed, and scheduled start and finish times (not to exceed an 8-hour shift). 2. Complete appropriate signature blocks. Each entry should at a minimum involve an entry supervisor, one attendant, and one authorized entrant. At least one attendant is required outside the permit space for the duration of the entry. The entry supervisor may serve as the attendant if the person is properly trained and equipped (see training section). 3. Atmospheric monitoring must be conducted prior to entry and continually during entry into any permit-required confined space. Should monitoring results exceed acceptable values the entry must be immediately terminated. Additional precautions, such as powered ventilation, may be required. Perform atmospheric monitoring and record results in the following order: oxygen content (19.5% to 23%) flammable gasses and vapors (<10% of lower explosive limit) carbon monoxide (<25 ppm) hydrogen sulfide (<10 ppm) Contact the Environmental Health & Safety Department when: atmospheric monitoring results are elevated, outside of acceptable ranges, or indicate a potentially hazardous environment the potential exists to generate additional contaminants (such as painting, coating, cleaning, welding, or torch cutting activities) UNR Permit Required Confined Space Program Page 4 of 18 02/10/06

5 the potential exists for other contaminants to be present in the confined space (such as natural gas, organic vapors, chorine, asbestos, or combustion products) if ventilation of the permit space is necessary prior to any sewer entry or entry connected to a sewer system 4. Check (or write-in) the potential hazards and the safety precautions that will be taken to minimize the hazards in the confined space. 5. Post the permit at the space during the entire entry duration. File a copy at Building and Grounds. ENTRY SUPERVISOR DUTIES 1. Know hazards that may be faced during entry into the confined space. Understand safety precautions necessary to minimize or eliminate hazards in the confined space. 2. Verify that all tests specified by the permit have been conducted and that all procedures and equipment specified by the permit are in place before signing the permit and allowing the entry to begin. 3. Verify that a functional means (such as a phone) for summoning rescue services is readily accessible. In the event of an emergency, direct phone contact with the rescue service is always preferable to two-way radio. If a two-way radio must be used, a person who is trained to use the radio to obtain rescue services must be available at all times during the entry. 4. Provide pre-entry briefing to all entry participants which summarizes terms of the permit, potential hazards, protective measures, and any additional specific issues related to the entry. 5. Ensure entry operations are conducted in accordance with terms of the entry permit and acceptable entry conditions are maintained. 6. Transfer responsibility, when necessary, for an entry operation only when it does not increase risk to authorized entrant(s). 7. Terminate the entry and cancel the permit when operations are completed or a condition not allowed under the entry permit arises in or near the permit space. 8. 6 ATTENDANT DUTIES 1. Know hazards that may be faced during entry into the confined space. Understand safety precautions necessary to minimize or eliminate hazards in the confined space. 2. Remain outside the permit space during entry operations until relieved by another authorized attendant or until completion of entry. 3. Continuously maintain an accurate count and identity of authorized entrants in the permit space. Prevent unauthorized personnel from entering the confined space area. 4. Do not perform duties that will interfere with the primary duty of monitoring and protecting the authorized entrant(s). 5. Maintain constant communication with and monitor authorized entrant(s) for signs of unusual behavior that may indicate undue stress or exposure to hazardous atmosphere. This may include verbal, visual, or other prerehearsed means of communication such as rope-tugging. 6. Monitor activities inside and outside the space to determine if it is safe for entrant(s). Evacuate attendants from the permit space immediately under any of the following conditions: a condition arises which is not in accordance with the permit entrant(s) exhibit signs of unusual behavior that may be indicative of exposure to a hazardous atmosphere or an environment which is immediately dangerous to life and health (IDLH) a situation outside the space arises that could endanger the authorized entrant(s) the attendant cannot effectively and safely perform all required duties any atmospheric monitoring alarm is observed 7. Summon rescue or other emergency services if authorized entrants need entry assistance to escape from the confined space. The authorized attendant never enters the confined space for rescue. AUTHORIZED ENTRANT DUTIES 1. Know hazards that may be faced during entry into the confined space. Understand safety precautions necessary to minimize or eliminate hazards in the confined space. 7 2. Properly use the required equipment. Examples of some equipment atmospheric testing and monitoring, ventilation, communication, personal protective equipment, lighting, barriers, shields, ladders, emergency and rescue equipment, and any other necessary equipment. 3. Communicate with the attendant to enable the attendant to monitor their status and be alerted of the need to evacuate the space. 4. Alert the attendant when: any warning sign or symptom of exposure to a hazardous environment or atmosphere is detected (odor, irritation, dizziness, headache, etc.) a condition arises which is not in accordance with the entry permit any instrumentation alarm occurs 5. Exit from the permit space as quickly as possible when: the attendant or entry supervisor requests evacuation the entrant recognizes any warning signs or symptoms (odor, irritation, dizziness, headache, etc.) of exposure to a hazardous environment, atmosphere, or dangerous situation a condition arises which is not in accordance with the entry permit any instrumentation alarm occurs EMERGENCY PROCEDURES Only non-entry rescue measures may be taken by attendants. To facilitate non-entry rescue, retrieval systems (such as a full-body harness with retrieval line attached at the center of the entrant s back near shoulder level, or above the entrants head) should be used when authorized entrants enter a permit-required confined space. The other end of the retrieval line should be attached to a fixed point outside the permit space. A mechanical device should be used to retrieve personnel from vertical type permit spaces more than 5 feet deep. Attendants are not permitted to enter the confined space to perform or assist in rescue activities that require entry. For rescues that require entry, or for other emergency situations, dial and state that a "confined space rescue" is required. Upon arrival of rescue service the entry supervisor or attendant should: 1. Inform the rescue service of the potential hazards which they may encounter in the confined space. 2. 8 CONTRACTOR PERMIT-REQUIRED CONFINED SPACE ENTRY When employees of another contractor must perform work in a UNR permit-required confined space the contractor must submit a copy of their confined space program for approval by the Environmental Health and Safety Department prior to entry. This may be submitted as part of the contractors safety and health plan or as a separate document. The contractors program must at a minimum meet the OSHA requirements in 29 CFR Prior to entry the contractor: TRAINING 1. Notifies UNR Buildings and Grounds when and where an entry will be made. 2. Obtains available information regarding hazards and associated protective measures for the permit-space. This information may be obtained from previous entry permits filed with Buildings and Grounds and/or from knowledgeable Buildings and Grounds personnel; appropriate individuals from Buildings and Grounds should be assigned to provide this information for contractors entering UNR permit-required confined spaces. 3. Coordinates entry operations with UNR when both contractor and UNR employees will be working in or near permit-required confined spaces. 4. Conducts atmospheric monitoring with contractor-supplied instrumentation. In some instances, special arrangements may be made to utilize UNR instrumentation. 5. Posts permit (either approved contractor permit or UNR permit) during entry. 6. Notifies UNR Buildings and Grounds upon completion of entry. 7. File a copy of permit with UNR Buildings and Grounds. Training must be provided for all employees who will potentially engage in entry supervisor, attendant, or authorized entrant duties. The training should enable the individual to acquire the understanding, knowledge, and skills necessary to safely perform their assigned duties. Initial training is required prior to assigning individuals to the above duties, additional training is required whenever there is a change in permitspace operations, procedures, or anticipated hazards. The training must establish employee proficiency in each of the required duties and be fully documented. Emergency Procedures non-entry rescue obtaining rescue services DEFINITIONS Attendant: An individual stationed outside one or more permit-required confined space(s) who monitors the authorized entrants and performs all duties outlined by the confined space program. This individual must have current confined space training. Authorized Entrant: An employee who is authorized to enter a permit-required confined space. This individual must have current confined space training. Blanking or Blinding: The absolute closure of a pipe, line, or duct by the fastening of a solid plate (such as a spectacle blind or a skillet blind). This closure completely covers the bore and is capable of withstanding the maximum pressure of the line without leakage. Confined Space: A space that is large enough for an employee to enter and perform the assigned work; has limited or restricted means of entry or exit (for example, tanks, vessels, silos, storage bins, hoppers, vaults, and pits); and is not designed for continuous employee occupancy. 10 Emergency: Any occurrence or event internal or external to the permit-required space that could endanger entrant(s). This includes any failure of hazard controls or monitoring equipment. Engulfment: The surrounding or covering of a person by a liquid or flowable solid which may result in death or injury by suffocation, strangulation, constriction, or crushing. Entry: Breaking the plane or passing through the opening of a permit-required confined space. Entry Permit: A document which allows entry into a permit-required confined space. This document provides guidance that will provide for safe entry and work in a permitrequired confined space. See the "Permit-Required Confined Space Permit" for additional information. Entry Supervisor: The person responsible for determining if acceptable entry conditions are present at a permit space. Additional responsibilities include entry authorization, supervising entry operations, and terminating entry operations. Hazardous Atmosphere: An atmosphere that presents the risk of death, injury, illness, incapacitation, or which may inhibit self-rescue. Any one of the following presents a hazardous atmosphere: 1. flammable gas, vapor, or mist in excess of 10% of its lower flammable limit (LFL) 2. airborne combustible dust at a concentration that meets or exceeds its LFL; NOTE: This concentration may be approximated as a condition in which the dust obscures vision at a distance of 5 feet or less. 3. oxygen concentration below 19.5% or above 23.5% 4. any hazardous substance measuring more than the accepted threshold limit value (TLV) 5. any other atmospheric condition that is immediately dangerous to life or health. NOTE: Contact the Environmental Health and Safety Department (x5040), refer to Material Safety Data Sheets, published information, and internal documents to help provide guidance in establishing acceptable atmospheric conditions. Hot Work Permit: Written authorization to perform operations producing a source of ignition (examples: riveting, welding, cutting, burning, and heating). Immediately Dangerous to Life or Health (IDLH): Any condition that poses a threat to life, causes irreversible health effects, or interferes with an individual s ability to escape unaided from a permit space. 11 Lower Explosive Level (LEL): The lower limit of flammability of a gas or vapor at ordinary ambient temperature expressed by a percentage of the gas or vapor in air (by volume). Oxygen Deficient Atmosphere: An atmosphere containing less than 19.5% oxygen by volume. Oxygen Enriched Atmosphere: An atmosphere containing more than 23.5% oxygen by volume. Permit-Required Confined Space (Permit Space): A confined space that has one or more of the following characteristics (see page ten for further guidance in assessing potential permit-required confined spaces): contains, or could contain, a hazardous atmosphere; presents an engulfment hazard; has an internal configuration that could entrap or asphyxiate an entrant; the configuration could be inwardly converging walls or a steeply sloped floor which tapers to a smaller cross-section; contains any other recognized serious safety or health hazard. Rescue Service: Personnel designated to rescue employees from permit-required spaces. Retrieval System: The equipment used for non-entry rescue of persons from permit spaces. (examples include a retrieval line, chest or full-body harness, wristlets, or lifting devices). Testing: The process of identifying and evaluating the hazards of a permit space. Testing enables a means to devise and implement control measures for the protection of authorized entrants.
