4 WHO CAN APPLY FOR COVERAGE? You are eligible to apply for health care coverage offered under the Medavie Blue Cross Seniors Health Program if you, or a qualifying spouse, are a resident of New Brunswick registered with Medicare and are age 65 years or older. 1

5 WHAT BENEFITS ARE OFFERED? You may choose from prescription drug coverage and various health care benefits to create the health care program best suited to your needs. Four (4) unique plans are offered: PLAN A: Medavie Blue Cross Seniors Prescription Drug Program PLAN B: Medavie Blue Cross Base Extended Health Plan PLAN C: Medavie Blue Cross Enhanced Extended Health Plan PLAN E: Medavie Blue Cross Hospital Reimbursement Plan These plans are supplemental to government health care programs. Medavie Blue Cross will not provide coverage for any benefits available through government-funded hospitals, agencies or providers. LATE APPLICANTS: There is a one-year waiting period for certain benefits under Plans B and C if you do not apply within 60 days following your 65th birthday, or within 60 days following the termination date of other extended health benefits, or within 60 days following gaining eligibility for NB Medicare as a new resident. 2

6 PLAN A MEDAVIE BLUE CROSS SENIORS PRESCRIPTION DRUG PROGRAM This program is available to residents of New Brunswick and qualifying spouses who are 65 years of age or older and currently registered with New Brunswick Medicare. The program provides coverage for those drugs purchased in New Brunswick that have been approved by the New Brunswick Department of Health. Under this plan, a co-payment fee must be paid to the pharmacist every time a prescription is filled or refilled. Who is eligible for the Medavie Blue Cross Seniors Prescription Drug Program? You are eligible to apply for this program if: you are 65 years or older and have a valid New Brunswick Medicare card, and you are not receiving the Guaranteed Income Supplement, and you do not qualify for government coverage based on your income, and you have no other insurance for prescription drug coverage. 3

7 When should I apply for the Medavie Blue Cross Seniors Prescription Drug Program? Knowing when you should apply is very important. You will be accepted into the Medavie Blue Cross Seniors Prescription Drug Program if: you apply within 60 days following your 65 th birthday, or you have reached age 65 and you apply within 60 days following the cancellation of a previous prescription drug plan, or you have reached age 65 and you apply within 60 days following gaining eligibility for NB medicare as a new resident. Late applicants are NOT automatically accepted, so be sure to complete your application within the time limits described above. How do I apply for the Medavie Blue Cross Seniors Prescription Drug Program? In your enrolment package you will find everything you need to apply. So, within the 60 days limits described above, you must: complete the Medavie Blue Cross Seniors Health Program application form, and return the form to the Medavie Blue Cross Seniors Health Program following the 4

8 directions provided in your enrolment package. What happens if I missed the dates or forgot to apply within the 60-day limits described on page 4? If you did not apply to the Medavie Blue Cross Seniors Prescription Drug Program within the 60-day limits described on page 4, you may apply as a late applicant. As a late applicant, you are required to complete a medical questionnaire. You may or may NOT be accepted, based on your medical history. The first step is to apply for New Brunswick Medicare coverage. When you receive your Medicare card, check the date that your Medicare coverage becomes effective. 5

9 To guarantee your acceptance, you must apply for the Medavie Blue Cross Seniors Prescription Drug Program within 60 days of your Medicare effective date. If you do NOT apply within 60 days following your Medicare effective date, you will be considered a late applicant. You may or may NOT be accepted, based on your medical history. When does my drug program become effective? If you apply within 60 days following your 65 th birthday, coverage becomes effective on the first day of the month of your 65 th birthday. If you apply within 60 days following cancellation of another prescription drug plan, coverage becomes effective on the first day of the month following the cancellation date. If you apply within 60 days following gaining eligibility for NB Medicare, coverage becomes effective on the first day of the month following the gain of NB Medicare. If you are a late applicant, your coverage becomes effective the first day of the month following acceptance of your application. Acceptance as a late applicant is based on your medical history. 6

10 Which drugs are covered under the Medavie Blue Cross Seniors Prescription Drug Program? The program covers an extensive list of drugs that have been approved as insured drugs by the New Brunswick Department of Health. These drugs must be prescribed by a health care professional in the course of treatment. Your pharmacist may substitute an equivalent generic drug for the brand name drug prescribed by your doctor. If you are not willing to accept the lower cost generic drug, the pharmacist may dispense another drug of the same type, but you will have to pay the difference in price. Certain other drugs not covered as regular benefits may be available through the Special Authorization process. How do I apply for Special Authorization when I need a drug that is not normally covered under the Medavie Blue Cross Seniors Prescription Drug Program? Certain drugs not offered as benefits may be available through the Special Authorization process. Your physician must: Submit a request in writing, giving your Medicare number 7

11 and date of birth, as well as specific clinical and diagnostic evidence supporting the use of the drug, the dosage form and strength, and the expected duration of therapy. The request should be addressed to: New Brunswick Prescription Drug Program Special Authorization Unit 644 Main Street, PO Box 220 Moncton NB E1C 8L3 Does it matter where I get prescriptions filled for drugs covered under the Medavie Blue Cross Seniors Prescription Drug Program? Eligible drugs MUST be dispensed in New Brunswick. All pharmacies in New Brunswick are currently recognized under the Medavie Blue Cross Seniors Prescription Drug Program. How does the Seniors Drug card work when I go to the pharmacy? Present your Medavie Blue Cross Seniors Prescription Drug Program card along with your prescription(s) to the pharmacist in a New Brunswick pharmacy. You will have to pay a co-payment fee to the pharmacist every time a prescription is filled or refilled. 8

12 How much is the co-payment fee? Co-payment amounts change from time to time. To find out the current co-payment fee, call the Medavie Blue Cross Seniors Prescription Drug Program toll free at If I am planning a trip outside New Brunswick, will my drugs be covered? Only drugs dispensed in New Brunswick are covered under the program. Your pharmacist can dispense up to a 90-day supply before you leave New Brunswick to ensure you have medication for the trip. If I m moving outside New Brunswick, can I still get my drugs covered? The Medavie Blue Cross Seniors Prescription Drug Program is for residents of New Brunswick only. If you are planning to move outside the province of New Brunswick, you must advise Medicare and the Medavie Blue Cross Seniors Prescription Drug Program of your moving date and your coverage will be cancelled accordingly. Although you can t take your New Brunswick drug plan with 9

13 you, you can obtain a 90-day supply of your medication before leaving New Brunswick, to cover the period until you can obtain coverage in your new province of residence. You should apply as soon as possible for coverage under the government sponsored health plan (medicare program) in your new province of residence. And you may wish to contact the Blue Cross office in your new province of residence for information on the prescription drug plans that may be available for you. PLEASE NOTE: If you are moving - within New Brunswick OR out of the province - please contact the Seniors Drug Program by mail or phone and advise us of your new address. 10

14 PLAN B MEDAVIE BLUE CROSS BASE EXTENDED HEALTH PLAN All benefits are reimbursed at 80 per cent of the eligible expense with maximums noted in the benefit description. CHIROPRACTIC SERVICES Medavie Blue Cross will reimburse 80 per cent of the cost, to a maximum of $12 per visit, for chiropractic treatment by a licensed chiropractor. Medavie Blue Cross will also reimburse 80 per cent of the cost of one (1) X-ray per calendar year, to a maximum of $20. The combined maximum for this benefit is $100 per calendar year. DIABETIC SUPPLIES Medavie Blue Cross will reimburse 80 per cent of the cost, to a maximum of $320 in a calendar year, for diabetic supplies prescribed by a doctor. This includes blood glucose testing materials (meter strips and lancets), and urine testing materials for glucose and ketones (single test strips, tapes and tablets). 11

15 Medavie Blue Cross will reimburse 80 per cent of the cost, to a maximum of $180 in a calendar year, for diabetic needles and syringes - either disposable or re-usable. Blood glucose monitors are not covered. (Late applicant provision applies to the Diabetic Supplies benefit - see page 2 for details.) FOOT CARE Medavie Blue Cross will reimburse 80 per cent of the cost for treatment of corns, bunions, callouses and care of nails by a registered podiatrist and/or chiropodist. The maximum Medavie Blue Cross will reimburse for these services is $16 per visit for five (5) visits per calendar year. GRADIENT PRESSURE SUPPORTS Medavie Blue Cross will reimburse 80 per cent of the cost for made-to-measure gradient pressure supports including: burn pressure garments, lymphodema sleeves, anti-embolism stockings and surgical stockings, to a combined maximum of two (2) per calendar year. Note: This benefit does not include support stockings or hosiery. 12

16 HEARING AIDS Note: Before being fitted for a hearing aid, you need PRE-APPROVAL from the Medavie Blue Cross Seniors Health Program in order to be reimbursed for this benefit. If you have a hearing loss, and an otolaryngologist or otologist prescribes (or you have a written recommendation from a registered audiologist) that you buy a hearing aid, Medavie Blue Cross will reimburse 80 per cent of the cost. There is a maximum of $320 every five (5) consecutive calendar years for this benefit. Batteries and the cost of exams are not included in this benefit. (Late applicant provision applies to the Hearing Aids benefit - see page 2 for details.) ORTHOTICS/ORTHOSES (Braces) When a doctor prescribes custom-made, custom-fitted or over-the-counter cervical collars, splints, braces and/or moulded arch supports for the treatment of a musculoskeletal deficiency, Medavie Blue Cross will reimburse 80 per cent of the cost to a maximum of $200 per calendar year. As well, Medavie Blue Cross will reimburse 80 per cent of the cost for custom-made, custom-fitted ankle-foot braces up 13

17 to a maximum of $300 per calendar year when prescribed by a doctor. OSTOMY SUPPLIES Medavie Blue Cross will reimburse 80 per cent of the cost for essential ostomy supplies including: drainage pouches, stoma caps, irrigation supplies, skin barrier blankets, pastes, powders and rings, skin care and gel products, adhesives and adhesive removers, belts and clamps, and disinfectants used only for ostomy supplies. Appliance covers and deodorants are not covered. (Late applicant provision applies to the Ostomy Supplies benefit - see page 2 for details.) PROSTHETIC SUPPLIES When your doctor prescribes a prosthetic (artificial) limb or a replacement limb, Medavie Blue Cross will reimburse 80 per cent of the cost once every seven (7) consecutive calendar years. As well, supplies such as stump socks (maximum six (6)), suspension belts, upper limb end devices (hand, hook, etc.) for amputees (maximum three (3)) and other devices necessary to use the prosthesis will be reimbursed at 80 per cent of the cost per calendar year. If maintenance and/or repairs are necessary, you are eligible to 14

18 have 80 per cent of the cost to a maximum of $80 reimbursed by Medavie Blue Cross in each calendar year. If there has been a change in your medical condition or atrophy, Medavie Blue Cross will reimburse 80 per cent of the cost for a replacement socket for your existing prosthetic limb once every two (2) consecutive calendar years. Breast prosthesis, following mastectomy, is an eligible benefit under this policy. Medavie Blue Cross will reimburse 80 per cent of the cost up to a maximum of $160 every two (2) consecutive calendar years. Hair prosthesis, due to an underlying pathology or its treatment (radiation, chemotherapy, or alopecia areata), is an eligible benefit under this policy, and will be reimbursed at 80 per cent of the cost, to a maximum of $240 per lifetime. (Late applicant provision applies to the Prosthetic Supplies benefit - see page 2 for details.) TRACHEOTOMY SUPPLIES Tracheotomy supplies including cannulas, ties, dressings, sponges (trach), suction catheters and syringes, heat and moisture exchangers, saline solution, surgical jelly (lubrication), and antiseptic cleansing agents will be reimbursed at 80 per cent of cost. 15

19 VISION CARE The costs associated with your eye exam, and the costs for your eyewear spectacles (including lenses and frames) or contact lenses when services are prescribed by a registered or licensed optometrist or ophthalmologist, will be reimbursed by Medavie Blue Cross at 80 per cent of the cost to a maximum of $64 every two (2) consecutive calendar years. For members having cataract surgery, the two (2) consecutive calendar year waiting period will be waived. To receive reimbursement, provide Medavie Blue Cross with a completed Vision Care Claim Form and an original paid-in-full receipt. Low Vision Aids - In addition to the Vision Care benefit, members who receive a prescription from an optometrist or ophthalmologist for monoculars and stands, aspheric lenses and/or magnifiers, to correct visual acuity that is less than 20/70, Medavie Blue Cross will reimburse 80 per cent of the cost to a maximum of $60 per calendar year. Please note: Safety glasses, sunglasses, or glasses for cosmetic purposes that do not require a prescription lens are not benefits under this policy. (Late applicant provision applies to the Vision Care benefit - see page 2 for details.) 16

20 PLAN C MEDAVIE BLUE CROSS ENHANCED EXTENDED HEALTH PLAN By selecting Plan C, you will be eligible to receive an enhanced extended health benefit plan that includes all benefits described in Plan B PLUS enhancements to that plan as listed below. The additional benefits that have been added to Plan C are prefaced with an asterisk (*). As well, any enhanced benefits have the enhancement explained. All benefits are reimbursed at 80 per cent of the eligible expense with maximums noted in the benefit description. *ACCIDENTAL DENTAL Should you receive a direct external accidental blow to the mouth that results in damage to natural teeth or a jaw injury, Medavie Blue Cross will reimburse 80 per cent of the charges from a dental practitioner to repair the damage up to a lifetime maximum of $7,000. To receive reimbursement for this benefit, you must submit a Dental Claim Form to Medavie Blue Cross within 180 days of the 17

21 accident, with the dentist s description of the complete details of the required services. If treatment is delayed, an explanation must be provided in writing by the attending dentist. *AMBULANCE AND AMBULANCE ATTENDANT Should you develop an acute unexpected illness or have an accident that requires an ambulance to transport you on a stretcher to the nearest hospital for inpatient treatment within Canada, Medavie Blue Cross will reimburse 80 per cent of the cost to a maximum of $400 per calendar year. When medically necessary, travel expenses of an accompanying registered nurse (not a relative) may also be reimbursed under this benefit, within the maximum noted above. These benefits are available only in Canada. CHIROPRACTIC SERVICES See *Other Practitioners, page 23. DIABETIC SUPPLIES Medavie Blue Cross will reimburse 80 per cent of the cost, to a maximum of $320 in a calendar year, for diabetic supplies prescribed by a doctor. An emergency prescription drug is considered to be a drug necessary to treat an unexpected or sudden injury or illness or a change in medical condition that requires immediate treatment. *EQUIPMENT RENTAL Medavie Blue Cross will reimburse 80 per cent of the cost for renting a wheelchair and/or hospital-type bed (including 19

23 mattress and safety side rails), when prescribed by a doctor. If, due to extended illness or disability, the need for these items will be long term, Medavie Blue Cross, at its sole discretion, may approve the purchase of these items. (Late applicant provision applies to the Equipment Rental benefit - see page 2 for details.) FOOT CARE See *Other Practitioners, page 23, for a description of the increase in Podiatrist and/or Chiropodist benefits. See also *Nursing Services, page 22. INCONTINENT/CATHETER PRODUCTS When prescribed by a doctor, Medavie Blue Cross will reimburse 80 per cent of the cost of essential incontinent/catheter products including urinary catheters, fecal incontinence collectors, urinary kits, urinary leg bags, urinary adhesive skin protectors, extension tubing with connector, leg bag straps, holders and liners. These items must be required as a result of a medical condition. Please note: Disposable incontinence briefs, incontinence underpads, chair pads, bed pads and IV catheters are not benefits under this policy. 21

25 *NURSING SERVICES (in the home) Medavie Blue Cross will reimburse 80 per cent of the charges for a Licensed Practical Nurse (LPN), Registered Nurse (RN), or a Registered Nursing Assistant (RNA), when the nurse is not a relative of the patient, for home nursing services, or for treatment given at a foot clinic in Canada, to a maximum of $250 per calendar year. *ORTHOPEDIC SUPPLIES Charges for custom-made orthopedic shoes and shoe modification supplies, when prescribed by an orthopedic surgeon, physiatrist, rheumatologist, podiatrist or doctor, will be reimbursed at 80 per cent of the cost, to a maximum of $100 per calendar year. ORTHOTICS/ORTHOSES (braces) When a doctor prescribes custom-made, custom-fitted, or over-the-counter cervical collars, splints, braces and/or moulded arch supports for the treatment of a musculoskeletal deficiency, Medavie Blue Cross will reimburse 80 per cent of the cost up to a maximum of *$400 per calendar year. In addition, Medavie Blue Cross will reimburse 80 per cent of the cost 22

26 for custom-made, custom-fitted ankle-foot orthotics/orthoses up to a maximum of *$400 every calendar year, when prescribed by a doctor. OSTOMY SUPPLIES Medavie Blue Cross will reimburse 80 per cent of the cost for essential ostomy supplies including: drainage pouches, stoma caps, irrigation supplies, skin barrier blankets, pastes, powders and rings, skin care and gel products, adhesives and adhesive removers, belts and clamps, and disinfectants used only for ostomy supplies. (Late applicant provision applies to the Ostomy Supplies benefit - see page 2 for details.) *OTHER PRACTITIONERS Charges from a chiropractor, chiropodist and/or podiatrist, clinical psychologist, massage therapist, osteopath, physiotherapist, or speech therapist will be reimbursed to a maximum of $200 per practitioner, to an overall maximum of $400 per calendar year. In combination with these maximums, Medavie Blue Cross will also reimburse for one (1) X-ray, to a maximum of $20 per calendar year. 23

27 *OXYGEN EQUIPMENT & GASEOUS OXYGEN (for mobility purposes) Charges for the rental of mobility oxygen equipment and gaseous oxygen prescribed by a doctor may be reimbursed as described below. If, due to extended illness or disability, the need for these items will be long term, Medavie Blue Cross, at its sole discretion, may approve the purchase of these items. Mobility Oxygen Equipment: Charges for oxygen conservers, portable cylinders and cylinder carts (excluding concentrators) will be reimbursed to a maximum of $1,600 every three (3) consecutive calendar years. Gaseous Oxygen: Charges for gaseous oxygen to be used with mobility oxygen equipment will be reimbursed to a maximum of $1,200 per calendar year. (Late applicant provision applies to the Mobility Oxygen Equipment & Gaseous Oxygen benefit - see page 2 for details.) *PHYSICIAN SERVICES (out-of-province) The usual and customary charges of a doctor, licensed to practice where services are rendered (in Canada only), are reimbursed at 80 per cent of the cost. 24

(Late applicant provision applies to the Prosthetic Supplies benefit - see page 2 for details.) *RESPIRATORY DEVICES Charges for portable aerosol compressors, when medically necessary, will be reimbursed to a maximum of $400 per three (3) consecutive calendar years. When the member is admitted for active treatment, Medavie Blue Cross will reimburse 80 per cent of the cost, to a maximum of $50 per day, for a maximum of 90 days per calendar year. The hospital must be paid for the service and a paid-in-full receipt submitted to Medavie Blue Cross.
