1 Instructions: Coordinated Program Application for Post-Graduate Students Completing CP Requirements 1) Applications must be TYPED and all requested information must be submitted by October 1st or February 1st. All applicants MUST be currently enrolled in the graduate program in the Department of Dietetics & Nutrition at the time of the application to the Coordinated Program. Incomplete applications will not be considered for review for the Coordinated Program. Students may only apply twice. 2) Official Transcripts: Required from all colleges or universities (including FIU) from which you have taken courses applicable to the Coordinated Program (CP). Official transcripts are to be sent from college/university directly to the Program Assistant. All transcripts from foreign countries must be evaluated by an outside agency approved by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) and the Academy of Nutrition and Dietetics (AND) and a course equivalency to a US Regional Accredited Degree. 3) Eligibility for Application as Post Graduate Students Completing CP Courses Only All applicants must meet with their advisor to determine their eligibility to apply: a. Evaluation of all dietetic courses completed compared to coursework requirements of the CP. b. Graduate courses: Have < 9 credit hours remaining. c. Master's thesis/project requirement: A first draft of project/thesis must be given to Major Professor. d. Signature of your Major Professor is required on the application form. Professor's signature denotes that the graduate course and thesis/project requirements are met. 4) Applicants should have an overall GPA of 3.0 or above at the time of application to the CP and prior to the start of the supervised practice. 5) Personal Statement: a. Should be 12-pt. font and 1.5 spacing. It will be reviewed as part of the application process by the CP admission's committee. b. Content of the Personal Statement should include: i. Your professional goals ii. How previous work and/or volunteer experience has prepared you for this program iii. Skills and knowledge that you can contribute to this program iv. Your reasons for aspiring to become a Registered Dietitian 6) Recommendation Forms: Select two individuals to complete the Recommendation Forms*. These persons should be familiar with your academic ability and work performance. They cannot be related to you. The CP Director or Committee members will not write letters or recommendations. *The writers of your letters of recommendation may choose to send a personal letter but the Recommendation Form Rating Checklist must be completed to be considered by the Program Director of the CP program. Each form must be in a sealed envelope with the writer's signature on the seal. 7) Complete Background check, FBI level 2 and Fingerprinting: a. The CP applicant must submit with the application a copy of their COMPLETED Level II FBI Background Check including the Finger-Printing completed by American DataBank. The student is responsible for any costs that this requirement incurs. (NOTE: THIS PROCESS CAN TAKE UP TO 3 WEEKS.) Checklist: 1. Typed completed CP program application form 2. Official transcripts sent to the Program Director 3. List of the remaining graduate courses by semester 4. Signature of Major Professor 5. Personal Statement 6. Two completed Recommendation Forms 7. Panther Degree Audit 8. 2 Coordinated Program Application for Post-Graduate Students Completing CP Requirements Cover Page I understand the following policies and procedures regarding this application: (Student Name and PID#) 1. IN ORDER TO BEGIN THE SUPERVISED PRACTICE EXPERIENCE, STUDENTS MUST: a. Have completed ALL their didactic course requirements. b. Have completed ALL the MS course requirements. c. Complete the MS thesis/project requirement, including presentation of the results in Graduate Seminar (DIE 6937). 2. The Coordinated Program will not consider applicants who: a. Have received lower than a C in any course taken in the Department of Dietetics and Nutrition or its transfer equivalent. b. Have received lower than a C in any science course required for the CP. 3. The student will be responsible for his/her own transportation to the supervised practice site. Students assume all liability for safely traveling to and from assigned areas. 4. The CP is designed for FIU degree-seeking students only. 5. Before supervised practice begins, each student will need to provide documentation: a. Vaccinations, including varicella titer, PPD X 2, MMR, and tetanus vaccines. b. Level II FBI background check including finger-printing. c. Active adult CPR certification (only by American Heart Association (AHA)). d. A 10-panel drug test could be required by the placement facility. i. The Supervised Practice Instructor will let you know if a 10 panel drug test is required. If required, you will need to complete it within 30 days of beginning the practicum. The student is responsible for any costs that the above requirement incurs. Details and appropriate forms will be provided to the student by the Department. 6. The CP applicant must submit with the application a copy of the COMPLETED Level II FBI Background Check including the Finger-Printing completed by American DataBank. The student is responsible for any costs that this requirement incurs. The CP applicant understands that final admission to the CP is contingent on the outcome of their background check. 7. There is an additional fee of $100 per credit hour assessed for supervised practicum classes, for the purpose of meeting the expenses associated with the supervised practice education. 8. Students must abide by the Academy of Nutrition and Dietetics Code of Ethics for the Profession of Dietetics. Students must also abide by the standards as discussed in the FIU CP Handbook. Students will not be admitted into the CP if their conduct does not meet these codes. During the Supervised Practicum, any student who does not perform in accordance to applicable policies and procedures as outlined in the CP handbook may be removed from the Coordinated Program. 6 Written Communication Skills: Autobiographical Statement Provide a brief personal history including your background, personal and professional goals, your reasons for aspiring to become a Registered Dietitian, and the influences (e.g., people, situations) that have contributed to your decision. Include a summary of how previous employment or volunteer experiences prepared you for this program and the skills and knowledge you would bring to this program. This written statement will also be evaluated for spelling, grammar, clarity of thought, in addition to content. A maximum of two typed pages with 12 point font and 1.5 spacing is required Page 6 of 7
7 CRITERIA FOR SELECTION OF STUDENTS Applications are due in the department of Dietetics and Nutrition on the assigned date (October 1st or February 1st) - no exceptions will be made. Should the due date fall on a weekend, the application will be due the Friday before the due date. The admission criteria are specified and weighted in the following point system. Faculty Evaluation of Applicants Department faculty members who have taught, advised, or supervised applicants will be invited to complete the Recommendation Form Rating Checklist. The CP Program Director or Committee members will NOT write letters of recommendation for students applying to the program.
