At the time of interview you are requested to produce the originals of the following documents along with the attested photocopies:- a) School Leaving Certificate indicating date of birth. The candidate, who has not submitted earlier the age proof mentioning date of birth, must produce it in original at the time of the interview. b) Diploma/Degree/Post Graduate/Ph.D degree certificates, mark sheets of the examinations passed along with the attempt certificate. c) Certificates/documents in support of experience claimed, duly signed by Head of Institution (s)/ HOD with rubber stamp, clearly indicating the period with date, month & year of employment and the nature of work performed. Experience will be reckoned up to the last date of receipt of applications i.e d) Employed candidates who have not already submitted No Objection Certificate from their employer(s) or whose applications are not forwarded by the employer(s), are required to bring the same at the time of the interview, failing which they may not be interviewed. e) Candidates applying for the reserved vacancies are required to bring, in addition to the above, SC/ST/OBC certificate (as the case may be) in the prescribed form. The OBC certificate should be issued in the last one year with certificate regarding creamy layer. f) Copies of publications/articles/research, if any. g) The Medical qualifications possessed by the candidates should have the recognition of the Medical Council of India. The proof thereof may be attached. h) In so far as Post-Graduation Medical qualifications awarded by Indian Universities are concerned, they should also find a place in the Schedules to Indian Medical Council Act, 1956, as amended from time to time. The proof thereof may be attached. i) Holders of speciality Board of USA qualifications should complete the entire requirements of the Board concerned. P.T.O.

3 - 2 j) Completion of compulsory Rotating Internship Certificate, Registration Certificate, an official document showing name of College / Institution from where degree / diploma has been done and official document showing name of the University and College from where teaching experience has been gained to be furnished. k) The experience gained should be recognized by the MCI or the statutory body concerned with the system of medicine as valid teaching experience for teaching posts and certificate thereof or any other valid proof in support thereof should be submitted. Teaching experience in any other post like the post of General Duty Medical Officer / Medical Officer shall not be considered for eligibility purpose for recruitment to the teaching posts. l) In the case of qualifications in Super-Specialties (M.Ch./ D.M.) except in the case of holders of D.M./M.Ch. of five years duration, the holders should have obtained M.D. (General Medicine) or M.S. (General surgery) or an equivalent qualification as prescribed by the said council in its recommendations on post graduate medical examination. m) In the case of holder of D.M./ M.Ch. qualification of five years duration, the period of Senior Post Graduate residency rendered in the last part of the said D.M./ M.Ch. shall be counted towards requirement of experience. 2. No TA will be paid for attending the interview, nevertheless, out-station SC/ST candidates who are unemployed and are not in receipt of any kind of stipend/emoluments etc., will be reimbursed actual traveling expenses limited to 2 nd Class Mail/Express Rail fare both ways by the shortest route subject to production of the original tickets. SC/ST candidates who are not permitted to appear in the interview on account of non-production of original certificates will not be entitled to such reimbursement. 3. Candidates are requested to bring this letter with them at the time of interview. 4. Please note that no request for change in the date, time and venue of interview will be entertained. 5. Your candidature is provisional subject to fulfilling of the eligibility criteria up to the satisfaction of the appointing authority.
