All suppliers furnishing components for MoDOT projects shall e qualifie as herein escrie. All components will e suject to inspection y the engineer at the source, intermeiate shipping terminal, or at estination. The engineer shall e allowe unlimite access to all facilities an recors as require to conuct inspection an sampling in accorance with Sec 106 unless specifie elow Application for Placement on Qualifie List. To ecome qualifie, a written request shall e sent y the supplier to Construction an Materials. The request shall inclue a Pre-Qualifie Section 1040, 1043, an 1044 Supplier Inclusion Certificate an Guarantee Statement an a guarantee that all material to e use in farication will e in accorance with MoDOT specifications an pre-approval for any source of material will e receive prior to use Maintaining Qualification. To maintain qualification, the supplier shall maintain quality control ocumentation. The require ocumentation for each shipment of material use in prouction shall e kept on file for three years. The supplier shall notify Construction an Materials at least 24 hours prior to each shipment Disqualification of a Supplier. A supplier may e isqualifie to provie components for use on MoDOT projects ase on the iscretion of Construction an Materials, for reasons incluing, ut not limite to, not maintaining require ocumentation, failure of material to consistently meet specifications, falsification of any ocumentation, misraning of components, unsatisfactory performance in the fiel, or for other reasons inicating lack of consistent material quality A supplier will not e consiere for reinstatement until after one year from the ate of removal for falsification of ocuments Three notices of failure to meet specification requirements within a 12-month perio will e cause for isqualification of the supplier for one year, effective from the ate of the thir notice A supplier isqualifie within one year of the en of a isqualification may e suject to permanent removal, with no application for reinstatement accepte for a perio of three years.
