1 (ACSM) GENERAL INFORMATION The purpose of the Undergraduate Level ACSM Leadership & Diversity Training Program (LDTP) is to promote the establishment of ACSM membership and involvement for undergraduate students of ethnically diverse backgrounds. The program includes $ of expenses to be applied to membership of national ACSM and its Southeast Chapter (SEACSM) and attendance at the SEACSM annual meeting (February 12-14, 2015 in Jacksonville, FL). Awardees will also be paired with a mentor throughout the meeting. This program is funded by the American College of Sports Medicine. Eligibility for Undergraduate Level Program: Current FASEB-MARC Award Recipients are NOT eligible. All applicants must meet the following general criteria: 1) Be a member of a racial or ethnic minority group, as defined by the National Institutes of Health: American Indian or Alaska Native, Black or African American, Hispanic or Latino, and Native Hawaiian or Other Pacific Islander 2) Be a citizen, citizen national, or permanent resident of the U.S. or Canada 3) Be enrolled in a Bachelor level program in a sports medicine, exercise science, activity epidemiology, or a related field at a college or university in the Southeast Chapter ACSM. 1

2 REQUIRED APPLICATION MATERIALS Required Application Materials For the Program: 1) Your completed and signed application form will be evaluated for professional appearance. We will be looking for neatness, accuracy, and professional presentation. This part of the application is worth ten (10) points. 2) EXACTLY TWO letters of recommendation, one of which is from the student s advisor or from another professor, department chair, academic dean, or individual who is familiar with the applicant s capabilities and work ethic. At least one of the letters must be from a current Southeast ACSM member. This section of the application is worth ten (10) points five (5) points for each letter. 3) A description (1200-word maximum) of how you intend to exhibit your dedication to the goals and long-range activities of ACSM. Examples might include committees on which you would like to serve, participation in Southeast regional chapter activities, or anything else that describes your desired future ACSM participation. Also address your professional goals. Convince us that you are worth the investment and please be as specific as possible. We will also be looking for information about how you intend to advance diversity within ACSM. This section of the application is worth ten (10) points. 4) A summary of participation in professional development activities including campus and community activities and especially those associated with service to ACSM. Applicants with high levels of participation in regional or national ACSM-related activities (meeting attendance, presentations, student/professional activities, certifications, etc.) may be given higher consideration. Applicants with high levels professional-development activities within other organizations will be viewed favorably. This section of the application is worth ten (10) points. 5) A current CV or resume. We will be looking at your ability to present yourself professionally on paper, your academic achievements, and your professional development activities related to the typical ACSM student members areas of interest. This section of the application is worth ten (10) points. Note: Since many applicants provide outstanding recommendations, the candidate s written description serves as a key discriminator among closely qualified applicants.
