1 REQUEST FOR QUOTATION (RFQ) Project # 16/0013 THIS IS NOT AN ORDER THE HARRIS CENTER LINEN LAUNDRY SERVICE The Harris Center for Mental Health and IDD (Formerly known as MHMRA of Harris County) requests a quotation for Linen Laundry Service. The supplier will provide a quote for laundry service of tablecloths and other linens located at 9401 Southwest Freeway, Houston, TX The successful supplier will provide a total cost solution for linen laundry service. INSTRUCTIONS TO BIDDERS 1. Provide a copy of Policy coverage for commercial general liability, workers compensation as applicable and your W9 form. 2. Include a statement that the Quotation represents an irrevocable offer for one hundred twenty (120) days from the Quotation date. 3. Please provide at least two (2) references. Scope of work should include: Cost of laundering tablecloths: a. Price of seventy (70) tablecloths b. Price per tablecloth c.
