1 CONTINUATION PROGRAM PLAN for Current EXISTING Teacher Centers Only SUBMISSION DIRECTIONS ELIGIBLE APPLICANTS: This Continuation Program Plan format is intended for use by current "Existing" Centers operating a Teacher Center in accordance with Education Law 316. Current "Established" Centers may submit an abbreviated Continuation Program Plan for "Established" Centers, available from the Teacher Center Program Office (Room 467 EBA, Albany, NY 12234). New applicants should contact the Program Office for additional information and application materials. All application formats may be found on the NYSED and Teacher Center web sites. FUNDS AVAILABLE and NUMBER OF AWARDS TO BE MADE: The legislative allocation for of $31 million supported 126 Teacher Centers throughout the State. Since the State Budget is not yet in place, the allocation is unknown at this time. If the statewide Teacher Center allocation remains at $31 million, continuation applications will be awarded the same funding level as If the State allocation is increased, continuing centers will receive a proportionate increase, provided the applicant's past performance is satisfactory, and all program requirements have been satisfactorily met. If the State allocation is decreased, continuing centers will receive a proportionate decrease. Centers will have to revise their budgets should the State allocation change. Grants to Centers may not exceed $2,000,000 (except for New York City) and may not be less than $20,000. APPLICATION REVIEW: Recommendations for continued funding are determined by the quality and extent of the program as reflected in the program plan, past performance, and compliance with program requirements, Education Law 316, and Commissioner's Regulations (81.2). In accordance with the Legislation, recommendations for continued funding are reviewed by the Professional Standards and Practices Board. SIGNATURE and INFORMATION SHEETS Due by May 31 (SEPARATE SECTION, Pages 1-5) Commissioner's Regulation (a) Compliance with the requirements set forth in Education Law section 316. Commissioner's Regulation (d) The extent to which activities or planned activities fit one or more of the purposes set forth in subdivision(1)(a) of section 316 of the Education Law. Teacher Center Standards: (A) Governance and (B) Management Include fully completed set of original Signature and Information pages. All required Signature and Information sheets must be completed and included. Policy Board composition must be in compliance with existing law. At least one statutory purpose must be addressed. Applications without the proper policy board composition will not be considered for funding. II. ABSTRACT: CENTER and PROGRAM DESCRIPTION Due by May 31 Teacher Center Standards: (A) Governance, (B) Management, (C) Professional Development, (D) Program, (E) Evaluation Provide a concise description (three pages maximum) of the Center and its proposed program for the upcoming year. This abstract should serve as an Executive Summary, a brief, stand-alone description of the Center. Include the following information: brief description of the Center and its program unique characteristics of the school community that impact on Teacher Center program. Include program implications engendered by the special needs of the unique population served by the Teacher Center. statement of the Center's mission goals for the Center expected measurable outcomes for participants for the upcoming year proposed key activities, including those which show they: are generated by specific local need are aligned with the New York State Teacher Center Standards reflect State and national initiatives for educational improvement reflect collaborative efforts with other agencies are being included/modified because of evaluation results and policy board reflection address the New York State Learning Standards and/or positively impact teaching and learning. EVALUATION PROCESS Due by May 31 Teacher Center Standards: (E) Evaluation A Teacher Center's evaluation process should contain a three-tiered plan for assessing the Center's success on the following three levels. 1 ACTIVITIES: The process the Center will use to evaluate program activities. Include the methods to be used (survey, questionnaire, interview, etc.), a description of the data to be collected, and criterion referents against which their success will be measured. 2 OUTCOMES: The process the Center will use to determine the extent to which expected outcomes for participants have been met. Include techniques to be used, the criteria against which outcomes will be assessed, and the rationale for their selection. 3 IMPACT: The process the Center will use to evaluate the overall effectiveness of the Center in terms of its stated mission, and its probable impact on teaching and learning. Describe the Center s evaluation process. Describe the plan for measuring the extent of your Center's success in each of the following areas. Include specific supporting evidence that will be collected to substantiate that: 1 teachers and other participants are satisfied with the program and services provided by the Center. Evidence may include: a summary of results of activity evaluations or other feedback. 2 each of the Center's stated expected outcomes were satisfactorily met. Evidence may include: a summary of responses gained through various evaluation strategies, demonstration of knowledge and skills gained by participants, or other feedback. 3 the Center is meeting its mission and goals, and is operating according to the Teacher Center Standards. Evidence may include: results of interviews and other evaluation strategies, demonstration of increased recognition and support of the Center, or other feedback. 4 Teacher Center program and activities contribute to improved teaching/learning. Evidence may include: reported changes in teacher behavior, classroom implementation of new or different strategies and skills, results of action research, improved classroom performance, or other feedback. Describe the Policy Board's role in determining the evaluation process. Describe the process the Policy Board will use to review the evaluation results and adjust Center program based on their consideration. Include information on who will be responsible, and when evaluation activities will occur. Include samples of evaluation instruments. Page 3

PROGRAM PLAN NARRATIVE See next page for format The Continuation Program Plan Narrative should address each of the Teacher Center Standards (column one), describe the Center s accomplishments in light of last year s continuation plan (column two), indicate proposed program, activities, and plan for continuous improvement (column three). The description should be no longer than 25 pages. Appendices, if used, should consist only of items that directly relate to and support the narrative. Do not append raw data (i.e., needs assessment data, individual activity evaluations, attendance rosters, etc.) USE THE CHART ON THE NEXT PAGE AS A TEMPLATE FOR SECTION IV. V. BUDGET (FS10) and BUDGET NARRATIVE Due by May 31 Commissioner's Regulation (h) The reasonableness of the proposed cost in relation to the expected outcomes. Teacher Center Standards: (A) Governance and (B) Management Submit the original plus two copies of a thoroughly completed FS10 to the Teacher Center Program Office with this program plan and budget narrative. (Include required detail (calculation of cost, unit price, etc.) The FS10 must be for the same amount as the award. A revised budget will be required if the statewide allocation is different from this year's $31 million funding level. Include a budget narrative outlining specific expenditures and their relation to the program. Include a copy of the Policy Board minutes showing their approval of the budget. VI. END OF YEAR PROGRAM DOCUMENTATION Due by July 31 Teacher Center Standards: (B) Management and (E) Evaluation NOTE: Section VI (End of Year Report) is to be submitted separately from the foregoing Continuation Program Plan (Sections I through V). All required end of year documentation must be submitted by July 31, and before a continuation project can be approved. Page 4

9 SIGNATURE AND INFORMATION SHEETS LEA & TEACHER CENTER STATEMENT OF ASSURANCES As part of the conditions and requirements for accepting a grant under the Teacher Resource and Computer Training Center program, the following must be agreed to by the LEA s Chief School Officer, its Teacher Association President, the Teacher Center Policy Board Chair, and the Director of the Teacher Center. 1. Operating the Teacher Center in accordance with Legislation (Education Law Section 316) and Commissioner s Regulations (Part 81). 2. Enabling the Policy Board as the governing body of the Teacher Center, exercising all rights and responsibilities afforded by the Law. 3. Engaging a Director to administer the program. 4. Complying with all reporting requirements in a satisfactory and timely manner, including, but not limited to: Submission of an FS10 approved by the Policy Board and signed by the LEA s Chief School Officer. Submission of timely Budget Amendments, approved by the Policy Board and signed by the LEA s Chief School Officer, as appropriate. Periodic, regular submission to the SED Program Office of Policy Board meeting minutes and program materials. Responding to Readers Comments for areas deemed to be unsatisfactory or in need of improvement. Responding to requests for information from the SED Program Office. Submission of End of Year reporting documents. 5. Assuring Center representation at Statewide Teacher Center meetings, including the Annual Meeting, the Spring Retreat, and the Summer Institute. 6. Assuring regular attendance at regional Teacher Center network meetings. 7. Assuring regular participation in the Teacher Center telecommunications network (Caucus).
