1 PUBLIC NOTICE REQUEST FOR PROPOSALS FOR BEHAVIORAL HEALTH SERVICES FOR BREMEN TOWNSHIP FUNDING I. INTRODUCTION The Bremen Township Board of Trustees is requesting proposals for the purposes of contracting with one or more individuals, public entities, non-for-profit community-based organizations or private companies providing mental health and/or substance use disorder treatment services to residents of Bremen Township as outlined within this document. Funds allocated by the Bremen Township Board shall be used to contract for mental health and substance use disorder services to address behavioral health treatment needs of Bremen Township residents, with a high priority focus on services for youths and families. Adult services are not precluded from funding under this RFP. Additionally, funding in this RFP may be used for purchase of psychiatric services for initial evaluation, assessment, treatment plan, outpatient treatment, psychiatric medications and medication monitoring. Funds may also be used for parent trainings and other approved educational behavioral health community forums. Any funds awarded in this contract or contracts must only be used for Township Residents. Bremen Township covers an area of 37.8 square miles. Bremen Township is bordered by Harlem Avenue on the west, 135th Street on the north, Western Avenue on the east and 183rd Street on the south. Oak Forest and Midlothian are entirely within Township boundaries. A number of other suburbs have portions within Township Boundaries which will require further verification against the map contained in the attached Exhibit. The contract will be awarded for a one year term with four (4) additional one year term renewals. Each annual contract is subject to appropriation and budget approval by the Bremen Township Board. Successful bidders must meet the minimum qualifications criteria set forth in Section II below. II. BIDDER QUALIFICATIONS To be considered a viable bidder for the RFP, the bidder organization must describe how it meets all of the following qualifications and attach appropriate evidence of same: Bremen Township RFP February 4, 2016 Page 1

2 1. The following corporate documents must be provided: a. For corporate entities i. Copy of Articles of Incorporation ii. Taxpayer ID iii. List of Board of Directors iv. If a tax exempt organization, copy of letter of exemption from IRS b. For individual practitioners: i. Taxpayer ID ii. Copy of registration with Department of Professional Regulation and/or State Medical Board 2. A copy of bidder s current Illinois license(s) to provide mental health and/or substance use disorder treatment. 3. If applicable, a copy of bidder s Medicaid certification to provide mental health and/or substance use disorder treatment. 4. Evidence of professional liability insurance in at least $1,000,000 per occurrence and $3,000,000 aggregate. 5. Summary of experience in serving the behavioral health population. 6. Copy of current program brochure. 7. Website address. 8. Bidder contact information (name, title, office address, address, office and mobile phone). 9. Bidder organization MUST have at least three (3) years of experience of providing behavioral health counseling services to individuals. 10. Bidder MUST have the capacity to assess and document that the individual served by the program is a resident of Bremen Township in accordance with Township requirements. Bidder must have the demonstrated capacity and willingness to track program outcomes and fees for services rendered to Bremen residents using such reporting formats or systems required by the Township. 12. Bidder must have the demonstrated capacity and ability to claim and appropriately allocate, collect, document and report third party revenue to ensure leveraging of resources and fair reporting of funding contract billings. III. FORMAT OF PROPOSAL A. Cover Letter The Cover Letter (2 page maximum) should include an introduction to the project, the name and address of organization submitting the proposal and the bidders proposal to address the behavioral health needs of Township residents and explain why you want to serve Bremen Township Residents. B. Detailed Project Description Describe the services, programs and projected fees per unit of services. A fee schedule may be submitted. Please indicate the fee the bidder organization intends to charge per unit of service to the contract. Please also include annual number of clients to be served and type of population served. Please include total projected annual request for contracted funds. For reference, in the past two years Bremen Township has provided youth and family services to approximately eighty (80) clients per year, providing between two and four counseling sessions per month. C. Documents in Bidder Qualification Section Include all required documents for the bidder qualification requirements. D. Proof of Financial Stability Copy of last year s annual audited financial statements, latest filed tax return and a list of other government organizations, not-for-profit organizations or foundations with which bidder has a funding relationship. TIMELINE On or before February 4, 2016 February 29, 2016 On or before March 3, 2016 March 7, 2016 April 1, 2016 RFP posted on the Bremen Township website Completed RFP due to Township Administrative Office by 4:30 p.m. Interviews with RFP finalists Township announces award recipient(s) Contract effective and services commence V. PROPOSAL SUBMITTAL REQUIREMENTS Two copies of the proposal should be delivered on or before February 29, 2016 by 4: 30 P.M. in a sealed envelope to the Attention of Supervisor Maggie Crotty, Bremen Township Administrative Office S. Kedzie Parkway, Markham, Illinois Any submissions received after that time will be ineligible for consideration. VI. ADDITIONAL BIDDER PROCEDURES a. Organizations may be selected for an interview with a committee prior to selection and awarding of the contract(s). b. All questions and answers will be posted to the Bremen Township website so all bidders have the same information. VII. GENERAL TERMS AND CONDITIONS A. Awards shall be made on the proposal that best meets the needs of Bremen Township for specifications, cost and compatibility. B. Minimum specifications are designed as a requirement of the bid. Minimum specifications are outlines to provide for a particular need or use by the Township, and are not meant to eliminate any particular vendor. 5 unable to be met by the vendor, the bidder, prior to the opening of the bid, shall contact the Township Purchasing Agent in writing to determine if a particular specification may be altered or accepted. C. Bremen Township reserves the right to accept or reject any or all bids. The right is also reserved to waive any minor irregularities in this bid, and to award the bid to the bidder whose offer is most advantageous to the Township from the standpoint of suitability to purpose, quality of service, previous experience and price, and to accept the bid that is in the best interests of the Township. D. The bidder hereby declares that all statements and representations made in the bid proposal are true and correct, and are made under the penalty of perjury under the laws of the State of Illinois. E. Insurance coverage required in the bid specification shall include at a minimum general liability, professional liability, workers compensation and automobile liability insurance coverages in sufficient amounts. F. Within ten (10) days after the notice of award is issued, the selected organization shall provide a copy of its existing liability insurance certificate naming Bremen Township and its Trustees, Officers and employees as additional insureds on said policies. Such insurance coverage shall be maintained in full force and effect for the duration of the Contract and must be in a form satisfactory to the Township. G. The selected bidder organization(s) will provide references from at least 3 other contracted government funders or managed care organizations. H. The selected bidder organization(s) will be asked to sign a contract that includes all the specifications in this proposal and such other provisions the Township deems necessary to establish the fee for service funding contract. I. The selected bidder will use it best efforts to maximize funding from third party payors even for eligible Bremen Township residents. Bremen Township RFP February 4, 2016 Page 5
