Critical as Medicare is to the health care of seniors, it does not completely remove the burden of health care expenses. Expenses that Medicare does not pay, including deductibles, co-payments and outpatient services, become your financial responsibility. The company has a long history of assisting policyholders in managing their health care costs and providing plans to help complete their health care protection programs. Our computerized system allows us to work directly with Medicare providers throughout the United States, in most cases reducing the time it takes to get claims paid and limiting your need to get involved in the process. 4 MEDICARE PART B EXCESS CHARGES In those instances when an assignment is not accepted for payment in full, this benefit will pay a percentage of Part B excess charges subject to charge limitations established by Medicare or state law. This benefit is available as part of Plans F and G. FOREIGN TRAVEL This benefit will pay 80% of costs not covered by Medicare for emergency medical care due to sudden and unexpected sickness or injury during the first 60 consecutive days of travel in a foreign country. This benefit is subject to a $250 annual deductible and a $50,000 lifetime maximum. This benefit is available as part of Plans F and G. MANAGING YOUR HEALTH CARE COSTS American National Life Insurance Company of Texas provides you with several Medicare Supplement plans that offer a variety of coverage levels. You may select from this series the plan that best serves your needs now and in the future. ANTEX MEDICARE SUPPLEMENT PLANS cover prior medical conditions immediately. There is no exclusion for pre-existing conditions. pay benefits regardless of any other insurance you have. are guaranteed renewable, for life, as long as you pay your premiums when due and there are no material misrepresentations. The company cannot cancel your policy, regardless of any changes in your health or the number of times you are hospitalized, but can change your premiums if it changes the premiums on all policies with the same form number as yours in the state where you live. There will never be more than one change during any 12 month period.

5 BENEFITS OVERVIEW MEDICARE PART A COINSURANCE This benefit will pay the Medicare Part A coinsurance amount for days of hospitalization, and for each of Medicare s 60 lifetime reserve days. Once all lifetime reserve days have been used, it will pay 100% of Medicare Part A eligible hospital expenses up to a maximum of 365 days. The cost of the first 3 pints of blood under Medicare Parts A and B are covered under this benefit. MEDICARE PART B COINSURANCE After meeting the Part B annual deductible, this benefit pays the coinsurance amount (generally 20% of the Medicare approved amount). This benefit is available as part of Plans A, F and G SKILLED NURSING CARE This benefit will pay the coinsurance amount for Medicare approved skilled nursing care for the 21st through 100th day in a Medicare approved facility, following a 3-day hospitalization. This benefit is available as part of Plans F and G. PART A DEDUCTIBLE This benefit will pay the Medicare Part A inpatient hospital deductible per benefit period. This benefit is available as part of Plans F and G. PART B DEDUCTIBLE This benefit will pay the Medicare Part B deductible. This benefit is available as part of Plan F.
