1 Objectives of Training in Plastic Surgery 2008 EDITORIAL REVISION This document applies to those who begin training on or after July 1 st, (Please see also the Policies and Procedures. ) DEFINITION Plastic Surgery is that branch of surgery whose focus is the management of complex composite tissue defects. The word plastic is derived from the Greek plastikos meaning to mould or to give form. GOALS Upon completion of training, a resident is expected to be a competent specialist in Plastic Surgery, capable of assuming a consultant s role in the specialty. The resident must acquire a working knowledge of the theoretical basis of the specialty, including its foundations in the basic medical sciences and research. The residents must obtain the competencies expected of a specialist in Plastic Surgery. These include Medical Expert, Communicator, Collaborator, Manager, Health Advocate, Scholar, and Professional. Residents must demonstrate the requisite knowledge, skills, and attitudes for effective patient-centered care and service to a diverse population. In all aspects of specialist practice, the graduate must be able to address issues of gender, sexual orientation, age, culture, ethnicity and ethics in a professional manner. 2 Medical Expert Definition: As Medical Experts, Plastic Surgeons integrate all of the CanMEDS Roles, applying medical knowledge, clinical skills, and professional attitudes in their provision of patient-centered care. Medical Expert is the central physician Role in the CanMEDS framework. Key and Enabling Competencies: Plastic Surgeons are able to 1. Function effectively as consultants, integrating all of the CanMEDS Roles to provide optimal, ethical and patient-centered medical care 1.1. Perform a Plastic Surgery consultation effectively, including the presentation of well-documented assessments and recommendations in written and/or verbal form in response to a request from another health care professional, including the recognition, diagnosis, management and appropriate counseling with respect to: All forms of acute and chronic wounds All benign and malignant skin and soft tissue tumours All issues relating to Plastic Surgery of the breast Altering aspects of aesthetic manipulation of the face and body, including all options of scalpel surgery, liposuction, alloplastic materials, etc The lower extremity, such as tibial osteomyelitis, and soft tissue defects of the foot All medical and traumatic conditions afflicting the hand and upper extremity Acutely ill patients who present with trauma, thermal or cold injury, or with acute local and systemic infections or injuries Head and neck deformities Pediatric Plastic Surgery, including congenital disorders of the head and neck, extremities and trunk 1.2. Demonstrate effective use of all CanMEDS competencies relevant to Plastic Surgery 1.3. Identify and appropriately respond to relevant ethical issues arising in patient care 1.4. Demonstrate ability to effectively and appropriately prioritize professional duties when faced with multiple patients and problems 1.5. Demonstrate compassionate and patient-centered care 1.6. Recognize and respond to the ethical dimensions in medical decision-making 1.7. Apply knowledge of the clinical, socio-behavioural, and fundamental biomedical sciences relevant to Plastic Surgery, including: The breast Aplasia, hypoplasia, hyperplasia, virginal hypertrophy of female breast and ectopic breast tissue The breast in Poland s Syndrome, pectus excavatum/carinatum and scoliosis of spine Gynecomastia Tumours of male and female breast The principles and role of breast reconstruction for aplasia with or without associated musculoskeletal anomalies, post mastectomy and post burns to the chest wall especially in the pubertal female Extravisceral soft tissues including: Effects of co-morbidities on skin and subcutis Systemic and local skin diseases amenable to surgical treatment Benign and malignant tumours of skin and its adnexa with emphasis on melanoma, basal cell cancer and squamous cell cancer Benign and malignant tumours of soft tissues including, fat, fibrous tissue, synovium, muscle, vessels including lymphatics and nerve Principles of tumour surgery, adjuvant therapies where appropriate, sentinal node biopsy and Moh s micrographic surgery Skin Fascia Muscle Tendons and ligaments The peripheral nervous system The peripheral circulatory system including lymphatics Bone, cartilage and soft tissue components of joints Grafts and flaps of the above tissue types or composites thereof Biomaterial and alloplastic tissue substitutes Wound healing and the effects there on of medical/surgical intervention The effects of development and aging on the above Pathology pertinent to Plastic Surgery of the above with emphasis on: Developmental anomalies Page 3 of 18
4 Disease processes Effects of medical/surgical intervention including radiotherapy, drug therapy and immunotherapy Surgical anatomy of the trunk (extravisceral), head and neck, upper limb including brachial plexus, lower limb, and external genitalia Patient and staff safety issues as they relate to: The use of energy sources for diagnosis and treatment such as cautery units, laser light, and diagnostic imaging modalities Drug delivery systems Use of biomaterials especially human blood products Universal precautions against communicable diseases The use of powered equipment Assess by all appropriate means the injuries, diseases, degenerative changes, aging changes and developmental abnormalities commonly treated by Plastic Surgeons Develop treatment plans including appropriate non-surgical and surgical approaches which: Recognize the potential psycho-social impact of both the condition and its management on the patient and family Identify post-surgical complications local and systemic, and formulate a treatment plan that includes the appropriate use of consultation from other services HAND SURGERY Common developmental abnormalities of the upper limb and their systemic associations Dupuytren s disease Other common acquired diseases of the hand and wrist Arthritis of the hand and wrist emphasizing osteoarthritis and rheumatoid arthritis Tumours of the hand and wrist Hand infections Nerve compression syndromes of the upper limb Upper limb trauma including the brachial plexus emphasizing the hand and wrist The burned and frostbitten hand Principles of fracture reduction and fixation (including dislocations of the hand and wrist) Page 4 of 18
5 Principles of reconstruction of composite tissue loss Principles of tendon transfers about the hand/wrist Principles of thumb reconstruction Principles of revascularization and replantation Objective testing methods applicable to upper limb pathology and trauma including diagnostic imaging, electrophysiological testing, ultrasound and laser Doppler, and assessment of muscle compartment pressures Rehabilitation of the hand and the role of multidisciplinary clinics in hand surgery LOWER LIMB SURGERY, including analogous topics as itemized for the upper limb excepting those areas, which fall within orthopedic practice such as management of arthridities, amputations, bone tumours and fracture/dislocations. The following represent problems that are unique in terms of their particular challenges in wound healing: The diabetic foot Diseases of the peripheral vasculature including the lymphatic system and principles of their management Principles of reconstruction of the traumatically compromised lower limb emphasizing soft tissue cover, sensory-motor function, segmental bone loss and vascular status Principles of reconstruction of lower limb defects secondary to debridement of acute and chronic osteomyelitis and resection of tumours recognizing the effects of adjuvant treatments on wound healing MAXILLOFACIAL TRAUMA Facial fractures and associated injuries to cranial nerves, the skull and the cervical spine Principles of managing trauma of the facial skeleton including the airway, bleeding, reduction and stabilization of facial fractures, simple dento-alveolar injuries and soft tissue injuries of the face including the ocular adnexa, upper aero-digestive tract and safe surgical approaches to the facial skeleton HEAD AND NECK SURGERY Principles of reconstruction of subtotal or total loss of the nose, the upper and lower eyelids, the upper and lower lips, the ear, and any of the aesthetic units of the face Tumours of the head and neck including the upper aero-digestive tract Principles of tumour surgery and adjuvant therapies Page 5 of 18
6 Effects of local and systemic diseases on facial features Pertinent infections of the head and neck Vascular and lymphatic malformations of the head and neck FACIAL AESTHETIC SURGERY The influences on patient perception of normalcy including ethnicity, age, peer pressure, and psychosocial circumstances Effects of sun-damage, nicotine, and other environmental factors on the normal aging process Methods of skin restoration including dermabrasion, chemical peels, laser treatment and use of retinoids Methods of ablating major crease lines including: Augmentation techniques such as dermal fillers and fat grafts Surgical excision of muscle/ nerve Use of botulinum toxin Methods of recontouring of facial features by: Augmentation with autogenous tissues including fat, dermis, fascia, cartilage and bone Augmentation with alloplastic materials Surgical redistribution of skin and subcutis, and platysma Direct excision of excess skin and subcutis, cartilage and bone Reposition and suspension of deeper structures Suction Assisted Lypo Non-surgical or medical modalities to improve appearance BURN SURGERY AND COLD INJURY Pathophysiology (local and systemic) of burn injuries including thermal burns (open flame, radiant, contact and scald), friction burns, electrical burns including lightening, chemical burns and burns secondary to electromagnetic radiation Inhalation injury Burn resuscitation and monitoring of the acute burn patient Burn wound care Principles of surgical debridement and wound closure Ethical issues surrounding life-threatening burns Psycho-social issues with burns including abuse particularly of children and the elderly and substance abuse or mental illness Page 6 of 18

Describe the CanMEDS framework of competencies relevant to Plastic Surgery 2.3. Apply lifelong learning skills of the Scholar Role to implement a personal program to keep up-to-date, and enhance areas of professional competence 2.4. Contribute to the enhancement of quality care and patient safety in Plastic Surgery, integrating the available best evidence and best practices 3. Perform a complete and appropriate assessment of a patient 3.1. 8 3.2. For the purposes of prevention and health promotion, diagnosis and or management, elicit a history that is clear, relevant, concise and accurate to context and preferences 3.3. Demonstrate effective, appropriate, and timely performance of diagnostic and therapeutic procedures relevant to the management of: All benign and malignant skin, soft tissue and bony tumors pertinent to Plastic Surgery practice All forms of acute and chronic wounds pertaining to plastic surgical practice Use all forms of wound reconstruction including grafts, flaps of all kinds, the transfer of all pertinent tissue types both locally and as distant tissue transfers in the management of wounds Debride complex wounds Directly close complex wounds where appropriate and primarily repair deep structures including muscles, nerves, tendons and vessels Harvest from appropriate sites, prepare the recipient bed or structure to receive, and apply or interpose autogenous grafts of skin, mucosa, fat, fascia, nerve, vein / artery, cartilage, tendon, bone Use biomaterials and alloplastic substitutes appropriately Raise, delay and inset flaps of one or composites of the tissues listed above Contour tissues with a combination of excision, reduction, augmentation and re-alignment Administer local anesthesia and conscious sedation Page 8 of 18
9 BREAST AND TRUNK SURGERY Demonstrate the ability to perform all diagnostic and surgical procedural skills related to all plastic surgical operations of the breast, including those originating from congenital, asymmetry, gynecomastia, oncogenic and radiation problems: Breast reduction, mastopexy and augmentation Nipple-areolar complex reconstruction Subcutaneous mastectomy of both male and female breast Breast reconstruction post-mastectomy both immediate and delayed using regional or distant flaps plus or minus implants Panniculectomy and abdominoplasty Body contouring with suction assisted lipoplasty ( SAL ) and autogenous and alloplastic augmentation Reconstruction of the trunk defects such as chest and abdominal wall loss, sternal osteomyelitis, perineal defects etc UPPER AND LOWER LIMB SURGERY Principles and practice of extremity replantation, skin and muscle reconstruction, escharotomy and fasciotomy Treatment of hand and wrist fractures and non-unions, Kienbock s disease, ligament injuries, and hand and wrist articular diseases and trauma Peripheral nerve surgery including brachial plexus, peripheral nerve surgery and repair, compression neuropathies and nerve tumours Tendons including repair, grafting and transfers MAXILLOFACIAL TRAUMA Cricothyroidotomy and tracheostomy Reduction and fixation of facial fractures with intermaxillary fixation, wires, plates and screws and external fixators HEAD AND NECK SURGERY Demonstrate the ability to repair and reconstruct all forms of post traumatic and post cancer resection deformities of the soft tissues and skeleton of the head and neck to include all fractures and oncologic defects related to Plastic Surgery practice Apply basic skills to reconstruct complex deficits secondary to trauma or tumour extirpation pertaining to loss of integrity of complex head and neck structures such as the ear, eyelid, nose, lip, cheek etc Dissection of the extra-temporal facial nerve Cannulate and repair the parotid duct Page 9 of 18
10 Cannulate, stent and repair lacrimal cannaliculi and lacrimal duct AESTHETIC SURGERY The expectation is that residents will gain experience by performing all parts of all common aesthetic procedures but in a segmental and sequential fashion and as with all procedures under appropriate supervision. Combined with the opportunity to be first assist at complete procedures and participating in postsurgical care, residents should be competent to establish their own aesthetic practices Be able to demonstrate the ability to perform aesthetic manipulation of the face and body with all significant options of scalpel surgery, skin resurfacing, tissue filling, liposuction, alloplastic materials, etc PEDIATRIC PLASTIC SURGERY Residents on completion of training should be competent to manage simple pediatric plastic surgery problems in a community setting provided appropriate facilities and support is available. This includes uncomplicated single system trauma and uncomplicated congenital and developmental problems Demonstrate the ability to repair common congenital problems related to pediatric plastic surgery including deformities of the head and neck, trunk, upper and lower extremities GENERAL COMPETENCIES Demonstrate the ability to deliver safe and effective local anesthesia and sedation where appropriate Safely use potentially dangerous equipment such as power tools, cautery, lasers and other devices when appropriate 5.2. Ensure informed consent is obtained for procedures 5.3. Ensure adequate follow-up is arranged for procedures performed 6. Seek appropriate consultation from other health professionals, recognizing the limits of their expertise 6.1. Demonstrate insight into their own limitations of expertise 6.2. Demonstrate effective, appropriate, and timely consultation of another health professional as needed for optimal patient care 6.3. 11 Communicator Definition: As Communicators, Plastic Surgeons effectively facilitate the doctor-patient relationship and the dynamic exchanges that occur before, during, and after the medical encounter. Key and Enabling Competencies: Plastic Surgeons are able to 1. Develop rapport, trust, and ethical therapeutic relationships with patients and families 1.1. Recognize that being a good communicator is a core clinical skill for physicians, and that effective physician-patient communication can foster patient satisfaction, physician satisfaction, adherence and improved clinical outcomes 1.2.
