3 1. Introduction First, we would like to express our appreciation for your participation in our Green Procurement Program. The Shindengen Group makes proactive efforts to totally abolish or reduce hazardous substances in our products through observance of various environmental regulations such as the Directive on the RoHS (restriction of the use of certain hazardous substances in electrical and electronic equipment) to improve the environmental quality of our products. Also we work on reducing the use and air emission of volatile organic solvents from the manufacturing process. The Shindengen Group will take dynamic business actions with minute attention to the harmony with the global environments. Thank you for your cooperation. 2. Purpose and Scope This Shindengen Environmental Management Document Guide is intended to explain how to complete the environmental management documents, namely, the Product Constitutive Substance Datasheet (Constitutive Substances List), the Warranty of Banned Substances-Free Product as Required by Shindengen Electric Mfg.; and important tutorials for the vendors and suppliers before entering the required information or data for these documents. * For the purpose of the Guide, Product Constitutive Substance Datasheet (Constitutive Substances List), Warranty of Banned Substances-Free Product as Required by Shindengen Electric Mfg., Warranty of Existence/Nonexistence of Restricted Substances by Shindengen Electric Mfg. are from time to time referred to as New Constitutive Substances List, Warranty of Banned Substances-Free Product and Warranty of Existence/Nonexistence of Restricted Substances, respectively. 3. Shindengen Environmental Management Documents (1) Product Constitutive Substance Datasheet (Constitutive Substances List) The Product Constitutive Substance Datasheet was changed from the old A4 portrait to the A4 landscape (See Fig. 1). The form will come with our request to declare the substances in your products by . You will answer Questions 1 to 3 and enter the date of issuance and the personnel in charge of the issuance. Then, you will return the form to us. * Make sure to complete the attached EXCEL files and send them to us by return . * For the entry methods, see Paragraph 7 (1), Entry Methods Product Constitutive Substance Datasheet (Constitutive Substances List). Fig1.[Product Constitutive Substance Datasheet (Constitutive Substances List)] (2) Warranty of Banned Substances-Free Product as Required by Shindengen Electric Mfg. The old form consisted of one A4 portrait page. The new Warranty of Banned Substances-Free Product contains two A4 landscape pages. The second page shows the thresholds, the currently exempted products, the JIS banned substances, and additionally banned substances by Shindengen Electric Mfg. This will also come with the request to declare the substances in your products by . You will complete any necessary information in the form, and then return it to us. * You may deliver the original hardcopy of the form by mail or in PDF file by . * For the entry methods, see Paragraph 7. This is the newly established Warranty form. In the cases where you enter Not disclose or Name unknown for any substance names in the new Product Constitutive Substance Datasheet, we will send you this Warranty form separately to have you declare the existence or nonexistence of the Shindengen Group-controlled substances. You will complete this form, and return it to us. * You may deliver the original hardcopy of the form by mail or in PDF file by . * For the entry methods, see Paragraph 7. (3), Entry Methods Warranty of Existence/Nonexistence of Restricted Substances by Shindengen Electric Mfg. * The recycle-recommended substances are excluded from the warranty. Fig.3. The list shows the substances which we want you to declare. * You do not need to return the list to Shindengen. To the person who may concern Fig. 4. 5 (5) Shindengen Group-controlled Substances The Shindengen Group-controlled substances are grouped into the following two levels, each of which covers different substances. For details, see the Shindengen Group-controlled Substances List. Level A: Banned substances (see Appendix 1, Shindengen Group-controlled Substances List (Level A Banned Substances)) Level B: Restricted substances (see Appendix 2, Shindengen Group-controlled Substances List (Level B Restricted Substances)) The Shindengen Group-controlled Substances List includes Appendix 3, Thresholds of Shindengen Group-controlled Substances and Exemptions from the Banned Substances, Appendix 4, Additional Requirements for Packaging/Packing Materials and Appendix 5, Shindengen Group-controlled Substances List (Other Chemicals). * The Shindengen Group-controlled substances are subject to periodic review and revision. We are planning to release such revisions in the substances in PDF file by or on our webpage (scheduled in around autumn 2008). 4. Deliverables You shall deliver the following: Product Constitutive Substance Datasheet (Constitutive Substances List), Warranty of Banned Substances-Free Product as Required by Shindengen Electric Mfg., ICP data and MSDS. You will provide us with the MSDSplus, if it all possible. 5. Confidentiality The deliverables will not be disclosed to any third parties but restricted within the Shindengen Group only. Notwithstanding the above, we may disclose any information or data provided in the deliverables as per the request to disclose by any competent authorities, their quasi authorities, or other specific customers makes an inquiry of the received information or requests. Your understanding and cooperation on such disclosure are most appreciated. 6. Others (1) In cases where the already received information or the 4M are changed, inform our Environmental Management Secretariat as set forth in Paragraph 8 of such changes with no delay. The 4M are: Man, Machines (equipment and tools), Materials and parts, Methods. (2) If there are any changes in your contact personnel including the company name, address, phone number, fax number, address and the like, inform our Environmental Management Secretariat of such changes. 4

In the green fields, you shall enter the information if applicable. Product Name, Product Model, Manufacturer, Product Weight and Note back 1) Check the fields of Product Name, Product Model and Manufacturer. If there is any wrong entry, please correct it and enter the proper one in the second line of the field of Note in the immediate right hand. 2) Enter the product weight. The weight is expressed in mg (milligram). If the weight exceeds a hundred grams, you may change it to gram. 3) If the data are the same as those in the New Constitutive Substances List already delivered to Shindengen, enter the relevant model name or the product series name in the cell immediately next right to the cell bearing Note: in the field of Note. We will fill out the Components List for the same series products. Enter the model/package name of the series product here (the cell immediately next right to the cell showing Note. Fig. 7. [Entry the required items in the Note field at the upper part of the Product Components Datasheet] If there is any wrong entry in the fields of Product Name, Product Model or Manufacturer, correct it and enter the proper one here (the 2 nd cell of the field of Note. 5
7 * Provide us with information/data for each substrate, when possible. In cases where it is collective substrate, describe it in the field of Note. * The field of Note bears up to 60 one-byte (30 two-byte) characters. Preview the page to confirm that any descriptions appear within the printable area of each line before you print the form out. Suppliers, Date of Issuance and Contact Personnel back 1) Enter contact information in the field of Supplier, including its address, contact department, phone number, FAX number, contact personnel and their address). In cases where we request you to declare the substances in more than two components, we will attach the sheets of the same number of components in one Excel file. Make sure to enter the necessary information in each sheet in a consistent manner. For your tips, highlight the green field2, and copy and paste the field in the other sheets. 2) Enter the issuance date in western calendar in the cell below the statement, The entered information and data are true and correct, and the name of the personnel who approved the issuance of this document in the field of Approved by. If the vendor/supplier and the company of such personnel are different, enter the personnel s company name in brackets after his/her name. [Entry Format of Date] * You may use the green field 2 in Fig. 6 for the sub-suppliers or sub-vendors. Existence/Nonexistence of Ozone Depleting Substances back Question 1: Check either of the radio buttons of Yes/No for any ozone depleting substances. If you choose Yes, refer to the Ozone Depleting Substances of the Shindengen Group-controlled Substances as listed in Appendix 1, and enter each proper name of the applicable substance. Supplier Declaration of Compliance with the Directive on the RoHS back Question 2: Click the Noncompliance immediately below the cell of Compliance/Noncompliance, and the pull-down button will appear on the right hand. Click the button and select Compliant or Noncompliant, whichever is applicable. In cases where you select Noncompliant, describe such noncompliant substance name and select Yes or No to answer the question about if any substitute. If it is Yes, describe the names of the substitute and the expected delivery period (for the applicable substances, see the Substance Group No. A05, A07, A09, A10, B02 and B03 of the Shindengen Group-controlled Substances as listed in Appendix 1). For the delivery period, if you already delivered the substitute, select Already delivered, or otherwise, if you are ready for the delivery, enter the date of delivery (enter the date in the form of year/month/date, e.g., 2008/5/1 as set forth above). If the product is in compliance with the Directive on the RoHS, make sure to select Compliant and provide us with the Warranty of Free Banned Substances Designated by Shindengen Electric Mfg. Question 3: Important how to complete the datasheet back 1) Read me As to Question 3, make sure to enter every constitutive substance in the product without omission. In cases where all the constitutive substances are not entered and the total content rate is not 100%, we will separately request you to re-check your answers or provide us with the Warranty of Existence/Nonexistence of Restricted Substances by Shindengen Electric Mfg. Please be forewarned. 6
8 Refer to the List of Data Required to Input in Product Constitutive Substances Datasheet, and enter the proper name of each component, purpose of inclusion, CAS No. and raw material name. 2) Entry of Components Refer to List of Data Required to Input in Product Constitutive Substances Datasheet included in the Product Constitutive Substance Datasheet, and enter the name of each component. * In cases where there are more than two components of the same name (for example, the color chords of the resistance and individual colors of the color bands are different), define the identifier for each such as Component 1, Component 2, in the field of Supplementary Note to enable us to identify each component. In cases where there are more than two components of the same name such as the color chords of the resistance, and their individual substance elements are different, define the component identifier for each such as Component 1 and Component 2 in the field of Supplementary Note. If there are more than two components of the same name which contain different substances respectively, define the component identifier for each such as Component 1, Component 2. Fig. 9. [Entry Method with more than two components of the same name which contain different substances respectively] 3) Entry of Total Substance Weight per Component Enter the total substance weight per component by one-byte numeric characters. * Click mg below the cell bearing Total Substance Weight per Component to change the unit as needed. When the unit for the Total Substance Weight per Component is changed, the unit of the product weight will be changed automatically. 4) Entry of Name of Substance/CAS No. Refer to the attached Shindengen Group-controlled Substances List, and enter the name of each substance and each CAS number. In cases where you do not disclose the names of substances, enter Not disclose in the field of Substance Name for each applicable component as well as the purpose of inclusion. However, such undisclosed content shall be below 10% of the total weight of each component, because the tolerance of such undisclosed substance shall be also registered to the IMDS (International Material Data System). If there is any substance over the tolerance or if the existence/nonexistence of the Shindengen Group-controlled substances is not explicitly declared, we will request you to redo the form and to submit the Warranty of Existence/Nonexistence of Restricted Substances by Shindengen Electric Mfg. Please be forewarned. 5) Entry of Content/Content Rate Enter the content and content rate in each product in the descending order (amount of used) by every component. When the content and the content rate is entered, the remaining weight and the remaining percentage will appear in the rows below the rows of Content [mg] and Content Rate [wt%] automatically; the former is calculated by deducing the entered content from the product weight or the latter by deducing the entered content rate from 100%. 7

[Content/Content Rate 6) Entry of Purpose of Inclusion of Substance Refer to the Available Choices for Purpose of Inclusion of Substance, List of Data Required to Input in Product Constitutive Substances Datasheet in the attached Product Constitutive Substance Datasheet, and enter the purpose of inclusion of each substance. 7) Entry of Name of Raw Material Refer to the Available Choices for Raw Material Name, the List of Data Required to Input in Product Constitutive Substances Datasheet in the attached Product Constitutive Substance Datasheet, and enter the name of each raw material. 8) Entry of Supplementary Note Enter any supplementary comments in the field of Supplementary Note. In cases where there are more than two components of the same name containing different substances, define the component identifier for each such as Component 1, Component 2 to enable us to identify. Information on Request for Declaration of Substances and Parts/Components back 1) Request No. means a serial number to each request for the declaration of substances designated by Shindengen. 2) Date of Request means the date when the datasheet is created by Shindengen. 3) Part/Component Code means the part/component codes given by Shindengen (if there are more than two applicable part/component codes, enter every single part/component code). 8

You may delete [Group II] and remove it from the scope of the warranty. If there are any errors in the product model names or manufacturers in the List of Substances, please let us know. 6) Group II includes the Additionally Banned Substances by Shindengen Electric Mfg. If the names of all substances and their content are disclosed, you may delete Group II and remove them from the scope of warranty. 7) A form of the Warranty of Banned Substances-free Product is a series of two sheets.
