Poster Session HRT11420 Innovation Awards November 2014 Melbourne your presentation to: Improving patient transport - Implementation of a pre transport Checklist Presenter: Ryan Midgley Chief Radiographer Kelly Preston CSC 1D, Emily Tilley CSC 2D Hospital Code Name:
Elevator Pitch- What is the outcome/significance of your presentation? Increased demand on Medical imaging transport, led to the need to review current system and process. With the introduction of a pre transport checklist the ammount of patient delays to medical imaging in the 3 Pilot wards was reduced by 72%, increasing productivity and reducing delays for Medical Imaging.
Presenters Summary Increased delays on wards for patients coming to Medical Imaging Incresae bed capacity at LMH +96 Need to reduce delays from the ward to Medical Imaging Group formed to reviewed the current system and process, we identified on a tracking exercise that 77 % of patients had at least 1 delay on the ward. We then agreed the best innovation would be to introduce a checklist to ensure prior to an orderly arriving to the ward, the patient would be prepared for transport. Result was a decrease in Delays from 77% - ~ 5%
KEY PROBLEM - One Slide Patient on the ward not being ready at time of collection which lead to significant delays, below is a Fish bone graph identifying the reasons for delay.
AIM OF THIS INNOVATION One Slide Decrease Delays experienced by Medical Imaging getting inpatients to examinations on time, increasing productivity.
BASELINE DATA - A manual tracking exercise demonstrated 17 of 22 patients from 3 pilot wards had at least 1 delay, minimum of 5 minute delay, and a maximum of 1 hr 77% patients had at least 1 delay
KEY CHANGES IMPLEMENTED Introduction of the Patient Transport Checklist Implementation of Electronic Bookings for all Medical Imaging transport jobs, and loss of domicile tranport staff, auditable and trackable system and more efficient. Introduction of workflow coordinator in Medical Imaging Holding Bay Introduction of Dect Phones for Nurse TL s on wards Introduction of large plastic Tubs for patient notes on the ward for patients going to Medical Imaging, allowing for quarantine and single point of collection of notes Ongoing Audits and review of transport reports
Patient Checklist MEDICAL IMAGING PATIENT TRANSPORT CHECKLIST This Form must be completed by the Nurse preparing the patient for a Medical Imaging Procedure If unable to completed within 30 min of call from Medical Imaging please call OR PATIENT ID GOES HERE WARD : BED: NURSE ISS ID CHECK CT NM US GEN FLURO MRI TIME CALL RECEIVED COLLECTION TIME BED W/C O2 INFECTIOUS PATIENT AWARE Y N DRESSED, ID, GLASSES Y N MEDICAL RECORD/CHART Y N OFFERED THE TOILET Y N EXAM PREP Y N MEDS IF REQUIRED Y N EQUIPMENT READY Y N JELCO Y N CHECKLIST COMPLETED BY REQUIRES ESCORT Y NURSE TO ESCORT This Checklist is required for all patient transport to Medical Imaging, Checklists to be forwarded to the Chief Radiographer

LESSONS LEARNT One Slide Single Workflow coordinator coordinating wards and imaging is vital. Single point of contact in wards for medical imaging is vital A system to quarantine notes is imperitive Electronic Bookings system allows analysis and reporting of data that is extremely important Use of a Checklist for patient transport for any area, decreases risk and increases efficiency. Well received by ward Staff, the use of the checklist is clear, and the goal and vision have been adopted by all staff. Hospital has avoided investment is additional transport staff
