1 REQUEST FOR PROPOSAL NO SECURITY SYSTEMS; TURNKEY SOLUTION FOR MULIPLE LOCATIONS 1 ADDENDUM #3 - RFP SECURITY SYSTEMS, TURNKEY SOLUTION FOR MULTIPLE LOCATIONS RFP Issued: August 31, 2012 Proposal Due Date: November 7, 2012 October 15, 2012 To all prospective bidders: The purpose of this addendum is to 1) change the technical requirements for Enterprise Access Control 2) Clarify the Fire Alarm Monitoring Requirements and, along with that clarification 3) change the Price Proposal Form to break-out Fire Alarm Monitoring and 4) provide a copy of the attendance sheet for the second site inspection. 1) Change the Technical Requirements for the Enterprise Access Control System as follows: Addendum#2, Attachment 2, Section A. Software: Item 4. Change the number of holiday dates from at least 4000 to at least 100 per year. Items 17, 22 and 23. Delete in their entirety 2) Clarification/Change. The furnishing of fire alarm systems is not part of this RFP; however, monitoring of all equipment currently being monitored is a part of this RFP. In as much as currently monitored equipment includes proprietary fire alarm systems, we are breaking out the monitoring of this equipment as a separate line item on the price proposal form. Bidders are allowed to no-bid this item if they are not authorized dealers of the fire alarm manufacturer. The Authority reserves the right to make award to more than one vendor on the basis of item, group or total bid whichever is in its best interest. 3) Addendum #2, Attachment 4-Price Proposal Form is DELETED in its entirety and replaced with the attached Price Proposal Form REVISED The price schedule has been changed to create a separate line item for fire alarm monitoring (see 2 above). We are requesting separate unit prices for burglar and fire on a monthly basis to be multiplied by months for a total 3 year price at each location. 4) Site Inspection #2, held on October 11, A listing of attendees is attached. All other terms and conditions of the original RFP remain unchanged. PERFORMANCE & PAYMENT BONDS: Payment and performance bonds in the amount of 100% of the bid award are due within 10 days of award. INSURANCE CERTIFICATE: Evidence of general liability, auto and workers compensations in minimum limits of $1,000,000 will be due within 10 days of award. (See Section of HUD Form 5370) DAVIS BACON WAGE DETERMINATION: The prevailing wage determination checked below, in effect at the time of award of the Contract, is attached and applicable to the Contract.
