3 Introduction The American School of Dental Assisting (hereinafter referred to as ASDA) offers affordable hands-on training to fully prepare students to succeed as Dental Assistants in the dental office. Dental assisting can be a high-paying career that is both rewarding and satisfying. The School is owned by Dr. Alex Knecht. The officers of the school are Dr. Alex Knecht, Director, and Hannah Knecht, Secretary. Faculty Members Alex Knecht, DDS, Director Dr. Knecht graduated from the University of Missouri, Kansas City School of Dentistry and is a member of the American Dental Association, Colorado Dental Association, and Academy of General Dentistry. Hannah Knecht, Secretary Hannah has a secondary education degree in teaching Biology from Emporia State University. She acts as the bookkeeper and maintains the finances for the ASDA. Program Offered Certificate Program: Dental Assisting 100 hours (combined lab, class, and internship hours) Occupational Objective: The graduate should be able to acquire a position as a Dental Assistant in a dental office. Program Cost Tuition $2895 Registration Fee $100 (non-refundable) Registration fee of $100 covers your textbook and seat reservation. TOTAL = $2995 Several payments plans are offered below, including long-term financing. There is a plan to fit everyone s budget. The cost of eleven (11) consecutive weeks of 8 am to 5 pm on Saturdays (or 6 pm to 10 pm Tuesday/Thursdays - depending on location) and 12 hour internship (Total 100 hours) includes all of the following: Textbook Modern Dental Assisting Dental X-ray certification On site CPR training and certification All dental supplies used in the clinical training

4 "Hands-on" training and experience using instruments and techniques in an actual dental office Certificate of Achievement and a letter of recommendation outlining your training and experience Tuition Payment Options Standard Financing ASDA Standard Plan: Tuition is due the day of class, after class finishes. 5 Special Financing 1. ASDA Extended Plan: For all payments directly to ASDA, we accept CASH, CHECK, VISA, DISCOVER, or MASTERCARD. A Co-Signer Agreement is required for all students paying ASDA directly (ASDA Standard Plan & ASDA Extended Plan) An Auto-Debit Contract is also required for all students paying ASDA directly. Each student must make payments according to their chosen plan. By signing the enrollment agreement the student agrees to follow the payment plan below and adhere to the financial policy of ASDA. Students may choose to finance their Tuition through the American School of Dental Assisting. Registration Fee of $100 may not be financed. ASDA offers two different financing options to choose from: 6 and 12 month financing. All payments are due on a monthly basis, based on when class begins. The first payment is due the 2 nd week of class, and following payments are on a monthly schedule thereafter. The 6-month plan has 6 equal payments, and the 12-month plan has 12 equal payments. An Auto-Debit Contract is required to automatically deduct the student s payment from their credit card account when the payment is due. If credit card does not process payment, then the student is responsible for any and all bank fees related to insufficient funds, and the student is expected to restore the appropriate funds that day, or Late Fees are enforced. The student is responsible for arranging on time payment if a credit card is declined. For every day a payment is delinquent, a $10/day Late Fee is added to the student s balance. Late Fee s may NOT be financed, and MUST be paid in FULL prior to the next payment due date. All accounts that are delinquent for 30 days or more will be sent to a Collections Agency, and the student will not receive their Dental Assisting Certificate. Students may not attend class if their account is delinquent. A Co-Signer Agreement is required for all financing terms with ASDA. Balance statements will be mailed or ed to each student once a month. Students shall not attend class until a financing contract has been signed. 6 2. CareCredit: CareCredit is a credit card that provides up to 24 months at 0% interest. If you enroll in CareCredit there is a Financing Charge added to the tuition depending on what term you select. Acceptance to this program is pending credit approval through CareCredit, a third party, and having a cosigner is recommended. Registration Fee of $100 can be financed (table below assumes this). Monthly Payment $ $ $ $ WIA Grant: Tuition and registration fee is paid in full up front by way of a grant through the Workforce Investment Act (WIA). In Colorado, call the CO Dept of Labor at (303) to contact the Workforce Center in your county. In Wyoming, call the WY Workforce directly at (877) WORK- WYO. We also accept any other grants that you can procure on your own (we do not accept federal grants). Late Payment Fees Your tuition payments must be made on the due dates according to your schedule, otherwise you will incur Late Fees. If you are paying by credit card, call to process your payment. If you are paying by check, please leave your check in the drop box (Ft. Collins students), or mail it to the address above (Denver, Aurora, Arvada, Westminster and Cheyenne students). Cash and Money Orders are accepted as well. You are responsible for making on time payments and keeping current with your balance. Listed below is our late/no payment policy as described in our course catalog and enrollment agreement: Late Fees $10/day payment is Late Student may NOT return to class until current with payments (including Late Fees) Late Fees may NOT be financed A student's account that is delinquent (where balance is not paid up to date) for more than 30 days will be sent to our Collection's Agency for recovery and the student will forfeit their Dental Assisting Certificate. The student is responsible for any and all bank fees related to a check that is returned for insufficient funds. The student is responsible for arranging on time payment if a credit card is declined. Auto-Debit Contract It is mandatory that every student on the Standard or Extended ASDA plan sign the Auto-Debit Contract. This option allows the student to have tuition payments automatically taken from their credit card account on the day the payment is due. This option eliminates the worry of any Late Fees being incurred, as long as the financing account has appropriate funds. (See separate form)

7 Co-signer Agreement Every student on the ASDA Standard or Extended Plan must have a Co-signer Agreement completed prior to acceptance into the school. This agreement is to ensure tuition payments are kept current, otherwise the student s co-signer will be required to make the student s payments. (See separate form) Course Schedule Saturdays 8 am 5 pm (with a 1 hour lunch break from 12 pm 1 pm) or Tuesdays/Thursdays 6 pm 10 pm (depending on location). A 12 hour internship is also required. ASDA operates on a continuous basis throughout the year. Starting dates are determined at the time of enrollment and are included as part of the enrollment contract. Sessions are held 3 or 4 times per year: winter, spring, fall, and summer. Classes are not held on the following holidays: New Years Eve New Years Day Memorial Day Independence Day Labor Day Thanksgiving Day Christmas Eve Christmas Day *When an unexpected closure occurs due to extraordinary conditions, such as inclement weather, students will be notified as soon as possible by phone. Entrance Requirements Each applicant will be considered based on 1) his or her of ability to benefit from the program, and 2) character recommendations by personal and professional references. ASDA does not discriminate on the basis of sex, race, religion or physical disability. Students must be at least 16 years of age. Enrollment Due to the "fast track" nature of the program, late enrollment is not permitted. Postponement of Start Date Whether at the request of ASDA or the student, a written agreement signed by the student and ASDA is required. The agreement must set forth: a. Whether the postponement is for the convenience of ASDA or the student, and: b. A deadline for the new start date, beyond which the start date will not be postponed. If the course is not commenced, or the student fails to attend by the start date set forth in the agreement, the student will be entitled to an appropriate refund of prepaid tuition and fees within 30 days of the deadline of the new start date set forth in the agreement, determined in accordance with ASDA s refund policy and all applicable laws and rules concerning the Private Occupational Act of 1981 Placement Assistance ASDA maintains a placement assistance service for their students consisting of job lead referrals and a letter of recommendation. It is understood, however, that the school cannot guarantee employment to any student or graduate.

8 Attendance Requirements A 100% rate of classroom attendance and punctuality will be required and ASDA reserves the right to dismiss those who do not abide by that standard. If a student is absent from a class, careful consideration will be given to the matter as to determine if the student can be excused (i.e. illness or death in the family). THIS IS A ONE HUNDRED HOUR COURSE Each student shall commit to the following in order to receive their training certificate and our letter of recommendation from Dental Assistant Training Academy: 1. Student CANNOT miss one day of the eleven-week session. 2. If student misses one day with an excused absence, student may make up that day at no charge. Student must make arrangements with instructor for date and time of make-up class. 3. If student misses one day and does not have an excused absence, student may be allowed to make that day up at the discretion of the instructor for a fee of $200/day, or $25/hour. 4. If student misses two days: the first day missed with excused absence, student may make that day up at no charge. Make ups must be before the 7th day after the missed class at an agreed upon date and time by the instructor and student. The second day missed, even if excused, may be made up at the discretion of the instructor for a fee of $200/day, or $25/hour. 5. If student misses three or more classes, there will be no make up available and certificate will not be issued. Tuition will be refunded according to the schedule of charges if applicable (see Schedule of Charges). It is very important that you make the commitment to your goal of receiving your certificate of training. Every class is important! Progress Policy Individual student progress is reviewed by the instructors. Dental Radiology certification, Final and CPR examinations will be graded on a class curve to ensure the material taught is being fully understood. As part of the final examination and dental radiology certification, a clinical evaluation is used to assess understanding and skill level. It is ASDA s policy to assist each student in every possible way to enable her/him to complete the course. When students do not achieve satisfaction by instructors review or cannot pass examinations, he/she will be notified and at which time possible withdrawal and refund will be considered. NOTE: ASDA does not guarantee transferability of our credits to another institution without a written agreement between ourselves and that institution. Grading System ASDA grades on a pass/fail system. Student will receive notification of a passing or failing grade on the day after the test for the mid-term and CPR examination, the following class for the dental radiology certification, and when graduation packets are delivered for the final exam. Conduct Policy All students are expected to act maturely and are required to respect other students and faculty members. Possession of weapons, illegal drugs, and alcohol of any kind are not allowed at any time on school property. Any violation of school policies may result in permanent dismissal from school.

9 Dismissal Any student may be dismissed for violations of rules and regulations of the school, as set forth in school publications. A student also may be withdrawn from classes if he/she does not prepare sufficiently, neglects assignments, or makes unsatisfactory progress. The Director, after consultation with all parties involved, makes the final decision. The Director may temporarily suspend students whose conduct is disruptive or unacceptable to the academic setting. After appropriate counseling, students who demonstrate a genuine desire to learn and conform to school standards of conduct, may be allowed to resume attendance. The Director will review each case and decide upon re-admittance. Student will not be re-admitted in the middle of a session. Student can reapply for a future session, but will be subject to pay entire tuition and registration costs for the new session. Students dismissed for not adhering to ASDA policies and regulations will not be allowed to reapply. Facilities ASDA s main office is 373 W. Drake, Suite 7, Fort Collins, CO We have more ASDA locations in Denver, Aurora, Arvada, Westminster and Cheyenne. Previous Credits Credits from another institution will be evaluated on a case-by-case basis. ASDA does not guarantee transferability of our credits to another institution unless there is a written agreement with another institution. Students are responsible for tuition fees proportional to how far into the program they transfer as well as registration and books/supplies fees. Student Complaints Student complaints should be brought to the attention of the Director to attempt resolution. The Director and student are to follow the grievance procedures according to school policy printed in the school catalog. If satisfactory resolution cannot be reached between the student and the school, the student may file a written complaint online with the Colorado Division of Private Occupational Schools at or by requesting a complaint form at (303) All student complaints submitted to the Division must be in writing and shall be filed within two years after the student discontinues training at the school. They can be sent to: Division of Private Occupational Schools of the Colorado Department of Higher Education 1560 Broadway, Suite 1600 Denver, CO Leave of Absence Since ASDA sessions are only eleven (11) weeks, ASDA does not permit leaves of absence by students. If a student needs to leave an ASDA session for any reason, the student will be refunded tuition according the Schedule of Charges. The student can reapply for a future session, but will be subject to pay entire tuition and registration costs for the new session.

10 Administration Policies Students must obey all rules of personal hygiene, sanitation, and sterilization while in the school. Gum chewing is not permitted. No smoking or eating is ever permitted in the dental treatment rooms. Although no formal dress code exists at ASDA, all students are expected to be well groomed and neatly dressed. Probation Policy Since ASDA students are graded on a pass/fail system, students are not put on probation. If student is not performing at a passing level, the student will be notified in writing at which time possible withdrawal and refund will be considered. Re-admittance Student will not be re-admitted in the middle of a program. Student can re-apply for a future session, but will be subject to pay entire tuition and registration costs for the new session. Students dismissed for not adhering to ASDA policies and regulations will not be allowed to re-apply unless corrections have been made. Refund Policy Refund Policy Students not accepted to the school are entitled to all monies paid. Students who cancel this contract by notifying the school within three (3) business days are entitled to a full refund of all tuition and fees paid. Students who withdraw after three (3) business days, but before commencement of classes, are entitled to a full refund of all tuition and fees paid except the maximum cancellation charge of $ or 25% of the contract price whichever is less. Also, the Registration Fee of $100 is not refundable unless student withdraws within the 3 business day time frame and returns all materials in new condition to the school. In the case of students withdrawing after commencement of classes, the school will retain the cancellation charge plus a percentage of tuition and fees, which is based on the percentage of contact hours attended in the Program/Stand Alone Course, as described in the table below. The refund is based on the official date of termination or withdrawal. 11 1. The student may cancel this contract at any time prior to midnight of the third business day after signing this contract. 2. All refunds will be made within 30 days from the date of termination. The official date of termination or withdrawal of a student shall be determined in the following manner: a. The date on which the school receives notice of the student s intention to discontinue the training program; or b. The date on which the student violates published school policy, which provides for termination. c. Should a student fail to return from an excused leave of absence, the effective date of termination for a student on an extended leave of absence or a leave of absence is the earlier of the date the school determines the student is not returning or the day following the expected return date. 3. The student will receive a full refund of tuition & fees paid if the school discontinues a course/ program within a period of time a student could have reasonably completed it, except that this provision shall not apply in the event the school ceases operation. 4. The policy for granting credit for previous training shall not impact the refund policy. Student Grievance Procedure American School of Dental Assisting does not discriminate on the basis of race, color, American origin, ancestry, religion, creed, sex, age, or handicap in its acceptance of students. Students may file a grievance when they believe they have been discriminated against in any of these areas. The purpose of these grievance procedures is to resolve differences as fairly and expeditiously as possible while preserving the right of students and staff members. Procedure: 1. Students shall first confer with the person who took the action or made the ruling to which they object no later than ten (10) days following the event that prompted the grievance. 2. If the difference is not satisfactorily resolved, the student may file a written statement setting forth the nature of the grievance to the Director of the ASDA. The grievance shall include a full description of the grievance, times, dates and pertinent facts and the remedy sought by the student. 3. The Director shall have a Student Grievance Panel (including the Director, the Secretary, and one student representative) which will have the power to make an appropriate investigation of the grievance and shall state the findings and make a recommendation 4. If the grievance is sustained by the panel, it will recommend appropriate action for relief of the grievance and communicate this in writing to the person(s) to whom the grievance was directed. If the findings of the panel do not sustain the grievance, the panel shall communicate this finding in writing to the student who filed the grievance. The ruling of the Student Grievance Panel is final. Schedule of Charges (see refund policy)

The school agrees to provide the occupational training in accordance with the provisions of the school s current Catalog Volume No. 2, dated 12/2009. Payment of all monies due shall be a condition of continuing enrollment. Upon satisfactory completion of all academic and skill requirements and when all financial obligations to ASDA have been met, ASDA will award the Certificate to the student. The student and ASDA understand that this Enrollment Agreement, WHICH INCLUDES THE REFUND POLICY, may not be amended except in writing and signed by both parties. **Please inital below. I understand that my total account balance is my responsibility, regardless of the financing option I have chosen. I understand that my account will be sent to a collections agency, I risk forfeiting my Dental Assisting Certificate, and I will not be allowed to return to class if I do not abide by the terms set in my contract with American School of Dental Assisting. If satisfactory resolution cannot be reached between the student and the school, the student may file a written complaint online with the Colorado Division of Private Occupational Schools at highered.colorado.gov/dpos or by requesting a complaint form at (303) All student complaints submitted to the Division must be in writing and shall be filed within two years after the student discontinues training at the school. They can be sent to:

17 Division of Private Occupational Schools of the Colorado Department of Higher Education 1560 Broadway, Suite 1600 Denver, CO Refund Policy Refund Policy Students not accepted to the school are entitled to all monies paid. Students who cancel this contract by notifying the school within three (3) business days are entitled to a full refund of all tuition and fees paid. Students who withdraw after three (3) business days, but before commencement of classes, are entitled to a full refund of all tuition and fees paid except the maximum cancellation charge of $ or 25% of the contract price whichever is less. Also, the Registration Fee of $100 is not refundable unless student withdraws within the 3 business day time frame and returns all materials in new condition to the school. In the case of students withdrawing after commencement of classes, the school will retain the cancellation charge plus a percentage of tuition and fees, which is based on the percentage of contact hours attended in the Program/Stand Alone Course, as described in the table below. The refund is based on the official date of termination or withdrawal. The student may cancel this contract at any time prior to midnight of the third business day after signing this contract. 2. All refunds will be made within 30 days from the date of termination. The official date of termination or withdrawal of a student shall be determined in the following manner: a. The date on which the school receives notice of the student s intention to discontinue the training program; or b. The date on which the student violates published school policy, which provides for termination. c. Should a student fail to return from an excused leave of absence, the effective date of termination for a student on an extended leave of absence or a leave of absence is the earlier of the date the school determines the student is not returning or the day following the expected return date. 3. The student will receive a full refund of tuition & fees paid if the school discontinues a course/ program within a period of time a student could have reasonably completed it, except that this provision shall not apply in the event the school ceases operation. 4. The policy for granting credit for previous training shall not impact the refund policy.

18 By signing below, the student agrees to pay the American School of Dental Assisting (ASDA) the total stated tuition and fees according to the financial plan I have chosen. The school agrees to provide the occupational training in accordance with the provisions of the school s current Catalog. Payment of all monies due shall be a condition of continuing enrollment. Student agrees to adhere to all attendance and conduct policies as stated in the school s current Catalog. Upon satisfactory completion of all academic and skill requirements and when all financial obligations to ASDA have been met, ASDA will award the Certificate to the student. The student and ASDA understand that this Enrollment Agreement, WHICH INCLUDES THE REFUND POLICY, may not be amended except in writing and signed by both parties.
