INTRODUCTION Increasing globalisation has meant a growing number of staff are being sent on overseas assignments. Ensuring that there is appropriate health care insurance in place goes a long way to reassuring employees that their specific needs as well as any accompanying family members, are being taken care of. This becomes of increasing relevance where the intended destination is a developed country where there is potential to incur significant medical costs, or a developing country where the standard or quality of healthcare available is questionable. In order to assist Australian companies with these exposures, Willis has in conjunction with our partners Ace Insurance Company of Australia Limited and BUPA Australia Health Pty Ltd, developed a Worldwide Healthcare Plan [the Plan] that offers: - Comprehensive Expatriate Health cover anywhere in the world. - Emergency Support & Medical evacuation. - Option to comply with Australia s Medicare system. - Options to extend cover. for:- Workers compensation replacement Benefits Income protection Corporate travel 2. OVERVIEW OF THE PLAN The Plan provides seamless cover between host and home country, with a choice of 2 key plans and 2 Policy Limits depending on region of residence,as detailed below: Region of residence Europe Asia Excl India, Africa, China,, Vietnam and Solomon Islands Region of residence India, Africa, China,, Vietnam and Solomon Islands Region of residence US/Canada/Japan/Hong Kong $1m Policy Limit $1m Policy Limit $1m Policy Limit Further details regarding coverage can be found in the Benefit and Compensation Tables included in the Appendix to this overview.
3 AN OVERVIEW HEALTH CARE PLAN 24 HOUR INTERNATIONAL EMERGENCY ASSISTANCE The Plan offers the best in Emergency Support services, accessed via a simple phone call to Ace Assist who arranges access to the following services free of charge, subject to the terms and conditions of the Policy) Ace Assist provides, through a professional assistance provider, access to a highly trained team of assistance personnel (including medical professionals) who are available on a 24/7 basis to assist you with medical emergencies and any other travel related problem you may experience overseas. How it works for you This additional benefit provides comprehensive and immediate advice in case you or your dependents are sick or injured and require hospitalisation whilst on your overseas assignment and the facilities to repatriate/evacuate you as necessary A phone call to the alarm centre number listed on your card will alert the Ace Assist service. The alarm centre will normally give immediate advice and arrange to put the member in touch with an English-speaking doctor. In an emergency the alarm centre can arrange the repatriation and/ or evacuation of you or your dependents to a suitable hospital for treatment. Alternatively the doctor may help to arrange treatment locally or ensure that any existing arrangements are satisfactory At all times the doctor has the full resource of Ace Assist at his disposal. Faced with an emergency without the aid of this additional benefit you could encounter many problems. Arranging medical treatment from thousands of miles away, and urgent repatriation, is time consuming, prone to delay and extremely expensive, even assuming that you know what arrangements need to be made. This valuable international benefit will take care of all the arrangements for you. And as the service could not be simpler to use we encourage you to utilise the service which include; Advising travellers on available medical resources overseas. Co-ordinating the planned aero medical transportation of incapacitated and critically ill patients back to Australia and from Australia to destinations overseas. Co-ordination of urgent aero medical transportation of critically ill patients from their current location to a more appropriate medical facility of first world standard. 24 hour medical monitoring 24 hour medical advise Providing a range of non-medical assistance services to travellers. Providing a message service to family members and employers or employees in emergency situations Providing guarantees of payment of hospital, accommodation, travel and additional costs Emergency Cash Advances Liaising with billing and accounts departments of hospitals Interpreting medical and technical reports or conversations Locating, booking and prepayment of flights, booking wheelchair and porter assistance, special arrangements for flights, e.g. booking oxygen, stretcher and obtaining
4 AN OVERVIEW HEALTH CARE PLAN medical clearance Co ordination of the preparation and repatriation of mortal remains Travel assistance Flight routing and details Referral to our International Network of: Medical Providers Embassy / Consulates Dental Providers Funeral Directors Aero medical Services Emergency Assistance Companies Legal Advisors If the local physicians are unable to provide the insured person with the best possible medical treatment, Ace Assist will convey the person to the nearest country better equipped to meet their needs. For total peace of mind, all medical expenses are taken care of directly or promptly reimbursed streamline and cut out unnecessary administration at Head Office. Claims Service to Members With particular emphasis having been given to quick and efficient claims settlement, systems are in place to achieve settlement within 10 working days of receiving all necessary documentation. Hospital in-patient treatment Ace Assist is able to arrange admission, guarantee of payment and direct settlement on discharge thereby avoiding out-of- pocket expenses. Outpatient treatment and services Options exist to arrange reimbursement in foreign currency and/or direct credit to foreign bank accounts (subject to domestic regulations) at no charge. In event of a serious injury/condition or contentious claim, Willis is able to offer assistance including in the negotiation of claims settlements. Effective administrative support At Willis Expatriate Risk, we provide dedicated administrators to ensure the smooth administration of the Plan. As part of the service, Individual members will be provided with a member kit containing: - A member booklet detailing information on Medicare eligibility and continuity of cover etc., - Claims procedures and Claim forms - Emergency Assistance cards. - Willis Contact details to respond on any specific query relating to the Plan. Where there is a number of staff on assignment, we can help develop procedures and billing arrangements that will help

OPTIONAL EXTRAS Depending on an individual s specific need, the following optional extras can easily be included to the core expatriate health cover Workers compensation replacement or top up benefits Accidental Death Disablement Benefits If within 12 consecutive months of an injury which results in death or specified events such as paraplegia/quadriplegia, loss of sight or loss of use of limbs etc, this cover will pay lump sum cash benefits. Income Replacement Benefits In event of injury or illness preventing an Insured Person from engaging in his/her usual occupation, the policy provides income replacement support for up to two years. 4. AUSTRALIAN HEALTHCARE SYSTEM All Australian expatriates eligible for Medicare are affected by the Australian National Health Act legislation including amendments introduced with effect from 1 July The risks of Not Getting It Right in relation to Medicare eligibility, continuity of health fund membership, Lifetime Health Cover (LHC), and the Medicare Levy Surcharge (MLS), can result in costly penalties for both the employee and employer. Connectivity to Australia Through the inclusion of BUPA/HBA Expatriate employees are able to enrol into a registered hospital table specifically designed to cover all legislative Medicare eligibility compliance issues and, to avoid any gaps in continuity of cover. This table is tailored to provide:- Authorised Business and related leisure Travel Insurance - Intended to compliment the benefits already received under the Expatriate Health Cover, cover applies whilst an insured person is travelling on authorised business travel including associated leisure travel. Cover is at a reduced premium and allows for up to 10 trips per annum with no single trip exceeding 60 days. Continuity of health fund membership for the duration of the assignment. Security of Lifetime Health cover Rating for duration of assignment Guarantees continuity of cover on permanent return to Australia with no waiting times for pre-existing conditions Medicare Levy Surcharge exemption. In essence, Australian Expatriates eligible for Medicare and who do not maintain or have never held Australian Health Insurance Cover, or those not insured under a Medicare compliant policy, may incur: - 12-month waiting periods for pre-existing conditions, incurred whilst on your overseas assignment, on permanent return to Australia.
6 AN OVERVIEW HEALTH CARE PLAN - Lifetime health insurance premium penalties if the correct cover or correct arrangements are not made or maintained whilst on your overseas assignment. - Possible MLS Tax penalties on lodgement of a tax return in Australia. Medicare Levy Surcharge (MLS) Under Australian Taxation Law, anyone who is eligible for Medicare AND is still required to lodge a tax return in Australia AND does not qualify for Non Resident status AND is not appropriately insured is subject to the Medicare Levy Surcharge (MLS). Personal circumstances vary and should be checked with a qualified accountant and/or the Australian Taxation Office. Further information can be found via the Australian Health Industry Association (AHIA) calculator: and ATO website: This levy is separate to the Medicare Levy and applies to gross income including exempt income earned overseas. Currently it is applicable to incomes over $50,000 per annum (for a single person) and over $100,000 per annum (for a couple/family).these limits are subject to change and are currently for review with the Senate Select Review committee. If prior to assignment an expatriate has private patient hospital cover and is considering cancelling or suspending cover while overseas, there may be Medicare Levy Surcharge implications if their income exceeds the relevant threshold and they are still required to lodge an Australian tax return. To retain or obtain an MLS exemption there are two options; - Retain membership of the existing health fund hospital table; or - Enrol into and Expatriate assist table which is specifically designed to eliminate the MLS tax (See Table 11 in the Schedule of Benefit and Compensation Tables in the Appendix). Under this arrangement, Members will be issued with a Private Health Insurance Statement to accompany their tax return.

INTRODUCING OUR PARTNERS Recognising that provision of an effective and responsive medical insurance provider plays an essential role in offering high quality support to employees whilst they are on overseas assignments, Willis has teamed up with partners who offer superior products and the resources required to support these services. Ace ACE Insurance Limited (Australia) is one of the ACE Group of Companies, a major global provider of insurance and reinsurance protection, with assets in excess of US$56 billion as at 31 December 2004 and gross premiums of over US$16 billion for the year ended 31 December ACE Insurance Limited has a financial rating of A Strong by Standard & Poor's A Excellent by A.M Best 6. OBTAINING A QUOTATION This overview is intended only to provide a description of the coverage afforded. All of the benefits described are otherwise subject to the limits, conditions and exclusions detailed in the relevant policy wordings, copies of which are available upon request. To provide a quotation we will need the following: - Name of Company - Nationalities of lives to be insured - Locations of lives to be insured - Gender - Occupations - Salary if salary related benefit - Cover required Once a quotation has been accepted, the employer will need to supply: 1 Completed and signed proposal forms. 2 Payment of the appropriate premium within 14 days of commencement date Under the Plan, Ace are responsible for the provision of Expatriate Health Cover for the duration of an employee s assignment.
8 HEALTH CARE APPENDIX BENEFIT & COMPENSATION TABLES The maximum amounts payable in any one period of insurance (less the deductible if applicable) is $1,000,000 per person per annum HOSPITAL IN PATIENT / DAY PATIENT In Hospital Medical Charges either Inpatient/Same Day Surgery as a private patient Full refund to policy limit Doctors, Anaesthetists, Nursing costs and Diagnostic procedures Full refund to policy limit Out of Hospital Medical Services Doctor s charges, Specialist charges and approved same day clinic charges Prescribed Medicines in Hospital Full refund to policy limit Full refund to policy limit PREGNANCY and CHILDBIRTH SERVICES Pregnancy to occur during period of insurance Routine Pre Natal, Delivery and Post Natal Charges for the care of Full refund to policy limit of $10,000 the Mother Emergency and/or Complicated Delivery Charges An additional $10,000 to Routine Care above Routine Maternity Care for the child from birth to 6 months of age at which time the new born child becomes an Insured dependent child under the policy) Full refund to policy limit of $10,000 ANCILLARY BENEFITS 100% reimbursement of actual costs, subject to annual sub limits shown below Acupuncture/Naturopathy/Hypnotherapy 100% to maximum $500 per person Chiropractic/Osteopathy $100 per visit to maximum. $1000 per person Dietician 100% to maximum $500 per person Optical 100% to maximum $500 per person Physiotherapy $100 per visit to maximum $1500 per person Podiatry 100% to maximum $500 per person Prescribed Medicines 100% to maximum $2,000 per person Prosthesis and Hearing Aids (one appliance every two years) Prosthesis means an artificial limb or total joint replacement and includes device designed and applied to improve function 100% to maximum $1,000 per person Speech Therapy 100% to maximum $500 per person DENTAL SERVICES General Dental Services means charges made by duly qualified oral surgeon or dentist for examinations, scaling, and cleaning, dental filling and restorations, diagnostic services, X-Rays, injections and extractions of teeth Special Dental Services means charges made by a duly qualified oral surgeon or dentist For root treatment, endodontic treatment, oral surgery, anaesthetic services, periodontal surgery, interceptive orthodontic services. Installation of and other specialist and orthodontic services and repairs to crowns and bridges, new dentures, dental repairs and remodelling 85% to maximum $1,200 85% to maximum $1,200
9 HEALTH CARE APPENDIX NURSING CARE SERVICES Home Nursing following an Injury or Sickness as defined by a qualified nurse who is not a relative $750 per week to maximum 4 weeks AMBULANCE Emergency transport expenses following an injury or sickness The maximum amount payable is $5,000 OTHER OUTPATIENT SERVICES Rehabilitation and Occupational Therapy 100% to a maximum of $10,000 Psychology and Psychiatry 100% to a maximum of $2,500 OTHER BENEFITS Compassionate Travel Additional Travel Expenses in the event of the unexpected death of the Insured Person s Spouse or Dependent Children or in the event of them becoming Very Seriously Ill, necessitating the Insured Person returning to his or her earlier country of residence, then subject to prior approval being obtained from Us and/or ACE Assistance, We will pay reasonable travel and accommodation expenses thereby incurred. The Maximum amount payable is $3,000 Repatriation of Mortal Remains The maximum amount payable is $10,000 Rehabilitation and Occupational Therapy The maximum amount payable is $10,000 Personnel Replacement In the event that an insured person, excluding spouse or dependent children, becomes very seriously ill the policy will pay reasonable travel and additional costs such as temporary accommodation expenses incurred by the insured for: The maximum amount payable is $10,000 the sending of a qualified replacement employee to Australia of the insured person to complete the unfinished business commitments of the insured person, or the return of the insured person to Australia after his or her recovery to complete those original business commitments Assistance company EMERGENCY TRAVEL ASSISTANCE The Plan incorporates benefits covering medical and / or emergency evacuation and will pay the actual, necessary and reasonable expenses incurred by the Insured person during a Policy Period for Medical Primary Care and Specialist Outpatient Care where such treatment is obtainable providing the Person contacts ACE Assistance and obtains a written certification by the attending Doctor stating that the Insured is Very Seriously Ill and it is necessary that the Insured Person obtains specialised treatment, surgery or postoperative attention which is unobtainable in the Country of Domicile Customer Care The maximum amount payable in respect of an Insured Person during any one Policy Period shall be $100,000 CORPORATE TRAVEL BENEFIT SUM INSURED (AUD$) Loss of Deposit $10,000 with excess of $50 per claim Luggage (limit any one item $1,000) $10,000 with $50 excess per claim Portable Business Equipment $5,000, with excess 10% min of $250 each claim Mislaid Luggage $3,000 Money $2,000 Missed Transport Connection $2,000 Rental Vehicle, Collision damage and Theft Excess $3,000 Cover Personal Liability $2,000,000 Authorised Business and related leisure Travel Insurance This cover applies only whilst you are travelling on authorised business travel including associated leisure travel. This particular travel insurance has been designed specifically to complement the Medical benefits you would already be receiving from the International Health Cover; hence this travel cover is at a reduced premium and allows for up to 10 trips per annum but with no single trip exceeding 60 days.
