3 Background Noridian Healthcare Solutions (NHS) Interactive Voice Response (IVR) system is a self-service option for providers inquiring on patient eligibility, claim status, check information and other general information. Please note that our customer service representatives (CSRs) continue to be available for claim specific and complex inquiries, but will only answer questions which cannot be answered by the IVR. The Centers for Medicare & Medicaid Services (CMS) Change Request (CR) 3376 states, With an increasing claims volume comes an increasing number of Medicare provider inquiries. One important way to successfully manage the workload is to increase and enhance the self-service technology tools available to Medicare providers and to require providers to use these tools when appropriate... Provider telephone CSRs are not intended to answer questions that can be answered on the IVR; they shall refer the callers to the IVR. Contractors shall identify and contact providers who repeatedly call CSRs for information that is available on the IVR to assist them to effectively use the IVR, including transferring providers back into the IVR. At a minimum, such education should happen at the time of the inquiry to the CSR, but may, in some cases, require post-call reinforcement. IVR Options Available States One of the following states needs to be selected in order to continue to the Main Menu on the IVR. California Nevada Hawaii (including American Samoa, Guam and Northern Mariana Islands Options Available The following options can be accessed from the Main Menu by keying or speaking the selection as below. The IVR will not provide the touch tone options during the initial prompt. 1 Eligibility 2 Claim Status 3 Financials 4 Procedure Code Pricing 5 Seminars 6 Phone Numbers 7 Addresses 8 Appeal Rights 0 Representative, Agent 1
4 The IVR is Easy to Use Have the following information available when calling the IVR: National Provider Identifier (NPI) Provider Transaction Access Number (PTAN) Last Five Digits of Tax Identification Number (TIN) If calling on a specific beneficiary or claim, have the following additional information available: Beneficiary Name Beneficiary Medicare Number Beneficiary Date of Birth Date of Service Helpful Hints Call from a quiet environment Be sure to have your information organized before calling Speak clearly and naturally into the telephone Bypass the upfront messaging by saying Main Menu or pressing the # key on the telephone keypad Selections may be keyed or spoken without waiting for the entire prompt to play Say Main Menu at any time to go back to the beginning of the call flow If the IVR is having difficulty with the information as spoken, try keying the information with the telephone keypad Enter the full Medicare number including alpha character(s) When speaking the date use the full format, for example, say July fifth, two thousand ten. In order to obtain accurate information, some Medicare numbers require you to verify the suffix each time, such as B as in Boy, D as in Dog, or T as in Tom. To eliminate repetition, include this information when giving the Medicare number B as in Boy IMPORTANT: Failure to provide the IVR with all the necessary information may result in inaccurate or incomplete results. Contacting a CSR Once at or past the Main Menu, indicate Representative or Agent to transfer to a representative. Press 0 at any point when the IVR is not actually waiting on a numeric response. Example: If the IVR is asking for the NPI, the IVR will register a 0 as part of the NPI rather than a request for a representative. The IVR will ask for the NPI, PTAN and TIN prior to transferring. If the NPI, PTAN, or TIN is not available, indicate Agent or Representative again to continue on to a representative. Using the Touch-Tone Feature The IVR is programmed to allow data entry using touch-tone in the event the user is unable to successfully speak to the IVR. When using the touch-tone feature, you must enter ALL components of the specific data via the telephone keypad. The system will not recognize a combination of speech and touch-tone entries within the same piece of data. 2
5 Using the Touch-Tone Feature to Enter Names The format for entering the beneficiary s name from 6 a.m. to 8 p.m. Central time is full first name, full last name. A single key entry is used for each letter of the name. Beneficiary Name (last name, first initial) Key Entry Doe, Joe D = 3, O = 6, E = 3, J = 5, O = 6, E = 3 When calling outside of 6 a.m. to 8 p.m. CST, the IVR will request the first letter of the beneficiary s first name. Once this is successfully obtained, it will request the first six letters of the beneficiary s last name. A three key combination must be used to key each letter. Below are some examples of how to key the name. Beneficiary Name (last name, first initial) Key Entry First Initial=J *51 J=*51 First six of last name= Doe *31*63*32 D=*31, O=*63, E=*32 In the example below, the beneficiary s last name consists of two names. An entry is not required for the space between hyphenated last names, etc. Enter the first six letters even if there is a space or hyphen between last names. Beneficiary Name (last name, first initial) Key Entry First Initial=J *51 J=*51 First six of last name= DoeMas *31*63*32*61*21*74 D=*31, O=*63, E=*32, M=*61, A=*21, S= *74 Note: During the times of 6 a.m. to 8 p.m. CST, the IVR may require users to enter the name using the three key combination. It will be identifiable because the IVR will specifically ask for the first initial and then the first six letters of the last name. The three key combination works as follows: Select the *(star) key Select the key on the telephone keypad which contains the letter Select the key which represents the position on the letter on the keypad Example: PTAN 55t551 Key Entry: 55*

Eligibility 1 Eligibility For beneficiary eligibility, key or speak the following information when requested: Patient Medicare number Patient gender Patient first and last name * Patient date of birth Date of service - The IVR cannot accept future dates of service while performing eligibility inquiries *After hours the IVR will require the first initial of the patient s first name and the first six letters of the patient s last name. The IVR will prompt for this information. The IVR application will provide the following information to the caller: 1. Part A and B effective and termination dates 2. Deductible information for the year of the date of service given The caller will then be prompted to choose another option: Managed Care (1), MSP (2), or Other (3). 1. Managed Care (Health Maintenance Organization (HMO)) information Whether or not there is an HMO HMO name HMO type Plan code number HMO address if available Phone number Effective date Termination date 4
7 2. Medicare Secondary Payer (MSP) information Whether or not there is an MSP on file MSP type MSP name MSP address if available MSP policy number Effective date Termination date 3. Other eligibility details Physical and Occupational Therapy limits Home Health Whether or not there is a home health care file Earliest date of home health Latest date of home health Hospice Whether or not there is a hospice file Earliest date of hospice Latest date of hospice Skilled Nursing Facility (SNF) Whether or not there is a Part A stay which may include SNF Earliest date of Part A stay Latest date of Part A stay Pneumococcal Vaccine Indicate if it s been received or next eligible date Date of Death Date of death on file if applicable The caller will be allowed to say another Medicare number for multiple patient eligibility requests. Note: In rare situations, the IVR may need to revert to a different system to obtain eligibility information. If this situation happens, the IVR will indicate that eligibility information will be limited. It will then be able to release Part A and B effective and termination dates, money applied to current and prior year Part B deductible, physical and occupational therapy caps, HMO information, and indicate if Medicare is secondary. Claim Status 2 Claim Status For claim status, key or speak the following information when requested: Patient Medicare Number Dates of Service - starting and ending dates The IVR will provide the following information to the caller: Claim Status (Processed, Denied, Pending, etc) Amount submitted Total Charges 5
8 Allowed Amount Paid Amount Check amount To request a duplicate remittance for the claim, say duplicate remittance. For additional claim information, say Claim Details to hear the following information: (if it is applicable to the claim) Finalized Date Processed Medicare Primary/Secondary Reason Code and Narrative Provider Reimbursement Deductible and Coinsurance Applied Total Non-covered Charges Check Number Document Control Number (DCN) ADR Issue Dates If multiple claims are available for the dates provided, you will be able to navigate through them to obtain claim details. At any time you may say: Repeat That, Next Claim, Previous Claim, or Claim Details. You can also change the Medicare number, NPI or PTAN numbers. Financials 3 Financials Once in the Financials menu there will be a submenu in which the caller with be prompted to choose another option: Checks (1), Offsets (2), Pending Approved (3) 1. Checks Information For check information, key or speak the following information when requested: Check date, check number or status The IVR will provide the following information to the caller: Check Issue Date Check Amount Check Number Note: It provides the dollar amount and not the number of claims. 2. Offsets Information For offset information, key or speak the following information when requested: FCN (Financial Control Number) The IVR will provide the following information to the caller: Patient s name Date of service 6

Payment Summary Information For pending approved information, key or speak the following information when requested: The IVR will provide the following information to the caller: Number of claims and dollar amount pending, month-to-date and year-to-date information and approved to pay amount Procedure Code Pricing 4 Procedure Code Pricing For procedure code pricing, key or speak the following information when requested: Procedure code Modifier (if applicable) Place of Service Zip Code Date of Service The IVR will provide the following information to the caller: The allowed amount Seminars 5 Seminars Visit us on the web at Phone Numbers 6 Phone Numbers Phone numbers available on the IVR: 2. Customer Service Representative 0 Representative, Agent When transferring to a representative, the IVR will request the provider authentication items. Key or speak the following information when requested: If the NPI, PTAN, and TIN are not available, indicate Representative or Agent to skip the provider authentication. 8
