2 The CTE-26, CTE/Tech Prep Program of Study (POS) and the CTE Assurance Page, referred to in this document as the CTE-26/POS, is an application designed specifically for secondary careertechnical program approval and renewal. It can be downloaded and printed from the Ohio Department of Education s website at SUBMISSION REQUIREMENTS 1. ORC specifies that a city, local or exempted village school district, community school, or STEM school that wishes to offer CTE programming must first seek approval of the lead district in the career technical planning district (CTPD) by submitting the CTE- 26/Tech Prep POS with signed assurance page by March 1 for program consideration. 2. A CTE-26/POS application is considered complete ONLY if it is submitted along with a signed Assurance Page by the deadline date. 3. The Lead CTPD Superintendent must approve or disapprove the program application by March 1. If a program is approved the lead district shall notify the Ohio Department of Education of its decision no later than March 15 for FINAL processing. If a program is disapproved, the lead district shall notify the submitting district or school of the disapproval or the decision to take no action by March 1. The submitting district has the opportunity to appeal this decision to the Ohio Department of Education no later than March The CTE-26 /Tech Prep POS and signed Assurance Page must be submitted electronically to odecte26@education.ohio.gov. The pathway consultants can assist with the completion of the template and information regarding program offerings including information related to career pathways within the career fields, content standards, professional development and resources. For general information on CTE- 26/POS application, contact Cyndi Brill cyndi.brill@education.ohio.gov or PORTAL PAGE INSTRUCTIONS The template for the CTE-26/POS application was developed in Adobe. A basic understanding of electronic applications would be beneficial when using this template. It is strongly suggested that you print out these instructions and view the CTE-26/POS application template as you review the instructions. Additional CTE-26 resources can be found on the Career Tech webpage by clicking here. If you are having difficulties in viewing the application, you will need to follow these steps: First, the application MUST be SAVED to your computer in order to work. If you are a Windows User: Right click on the link, select Save As or Save Target As. If you are an Apple/Mac User: Press and hold Control (Ctrl) and click the link. Select Save As. Second, if your PDF viewer is not able to display this type of document, you may need to upgrade to the latest version of Adobe 2
3 Reader for Windows, Mac, or Linux by visiting For more assistance with Adobe Reader, visit Once you have access to the template, you will be greeted with a welcome from the portal page. Through this portal page, an application will be developed that accommodates local district secondary and postsecondary course offerings. Active link buttons located down the left-hand side of the portal page allows the user to select the type of pathway programming application needed for preliminary approval of secondary career-technical education. Users will also be able to customize their program of study by selecting options that generate an application that reflects middle school programming and the primary type of post-secondary partnership the district will use for Tech-Prep articulation. To get started using the portal page, follow these easy steps: PORTAL STEP 1: Click on the button located on the left side of the portal page associated with the type of pathway programming the district will deliver. A CTE-26 application page will be produced for the selected button. The button will turn RED when selected. If you have made an incorrect page selection, just clear the application by selecting the last button Close All Pathway Pages and re-select your option. PORTAL STEP 2: Click on the Middle Level button if you will be delivering CTE courses at the middle level grades 7-8. A Program of Study reflecting middle school programming will be generated by selecting this option. PORTAL STEP 3: Select the primary type of post-secondary partnership the district will use in the Program of Study. 3
4 PORTAL STEP 4: Once you have made your selection, you are ready to start the application - just use your scroll down tool to find the first page of the generated application. OTHER PORTAL OPTIONS: For the user s convenience, additional action buttons for printing, reporting technical problems and for clearing the template have been added. NOTE: If you are experiencing technical difficulties with the template (i.e., a District IRN # does not pre-populate correctly on the application), please use the Report Application Technical Problems action button. By selecting this option, you are generating an support ticket. To receive timely assistance, clearly identify the problem you are experiencing in the body of the message. 4
5 INSTRUCTIONS FOR COMPLETING APPLICATION You are now ready to start the application - just use your scroll down tool to find the first page of the generated application. Information you will need when filling out CTE/26 WFD page of the application: District IRN Building IRN (where the CTE career field programing will be occurring). Contact Person (Should be the individual that has knowledge of the CTE program and the information that is being submitted on the application. This individual will be contacted if more information is needed to process application.) NOTE: Information on career fields, pathways, and Education Management Information System (EMIS) Career-Technical subject codes is also needed; these codes are at with the keyword search: CTE Workforce Development Matrix. STEP 1: Determine which type of application you will need under the Application Type dropdown box: o New WFD Program (Work Force Development) o WFD Single Course Offerings (This is a restricted option and can only be used with an established CTE program partnerships. Example: Single CTE course being offered in a middle school can show continuation of the career field pathway by establishing a partnership with an approved CTE program in the high school.) o Renewal of Existing Tech Prep (Refer to Program Renewal, Content Standards and Technical Test Development Schedule document for renewal deadlines.) o Addendum to Existing Application (Use to update an existing Approved CTE program that is not in a renewal year. Example: Add new middle level singleton course to an existing approved program or needed to establish a new Tech Prep Bi-Lateral Agreement to comply with Assurances.) 5
6 STEP 2: Type in your District IRN and the Building IRN where the CTE career field program will be occurring. NOTE: The template will populate associated fields in the document. List a contact person and their information. Verify that the contact s is correct. This will be used by the Ohio Department of Education to send Approval Verification letters. STEP 3: Select the two-digit CTE Pathway Program Code. Program Codes can be found on the CTE Secondary Workforce Development Career Field, CTE/Tech Prep Program, CTE Subject Crosswalk. Click here to find the document under Data and Accountability on the ODE website. Once the CTE Pathway Program Code has been chosen in the dropdown box, the associated CTE Career Field and CTE Career Pathway will be populated in the application fields. Additionally, the Subject Codes, Course Names, and Curriculum Code options will be populated for the selected Career Field and Pathway. You may use this list to select a sequence of courses to use on the Secondary Options/Program of Study page. 6
7 STEP 5: The last step on this page is to type in the total projected program hours for the CTE Hours in the Pathway. (Must be at least 450 hours unless otherwise stated on the Career Technical Education Workforce Development Program Matrix ) The Ohio College Tech Prep Regional Center points of contact information associated with your district s CTPD will auto fill from the District IRN and Building IRN that you supplied at the beginning of the application. College Tech Prep is facilitated in Ohio through six regional centers. The Ohio College Tech Prep Regional Center provides services for Programs of Study development. STEP 6: Continue the application by scrolling down to the Program of Study. In the cells provided, identify the English, mathematics, science, social studies and other courses that are recommended for a student in this program. It is preferable to include the name of the Recommended Electives. Recommended Electives need to be relevant to the pathway. Overall, the program of study must show how state and district graduation requirements/expectations are being met. Within that context, you are indicating the specific academic coursework you recommend for students interested in pursuing a particular career pathway. For two-year, career-technical programs located at a Career Center/Joint Vocational School (JVS), academics should be based on one selected or chosen associate school. Recommended electives must be identified by actual course name or title as opposed to being identified as Elective. For core areas such as mathematics, English, social sciences, foreign language, fine arts, Recommended Electives need to be relevant to the pathway. 7
8 Courses should be coded using the following initials related to their designation. The codes are case sensitive (i.e., use capital letters and parentheses) and when typed in to the action box, will turn the action box with the corresponding color code. For Example: CTE: Required Career-Technical Courses CC: College Credit Opportunities based on articulation or dual/concurrent enrollment agreements RA: Required Courses RE: Recommended Electives SC: Single WFD Courses Offerings (Incomplete pathway offerings) If a first course in the a pathway or single pathway course is offered as middle school programing at the associate school, it must be linked to an approved CTE/Tech Prep POS at the high school. If the first course in the pathway or single pathway course is being offered at the 9-10 grade level, it must be linked to an approved CTE/Tech Prep POS at career center. It is highly recommended that each associate school have a CTE/Tech Prep POS on file for each CTE program. Articulation/dual credit information on the postsecondary section of the template should correspond to that provided on the secondary section of this CTE/Tech Prep POS. This includes a clear designation as to what courses are eligible for articulated/dual credit as indicated on the secondary portion of the CTE/Tech Prep POS. Identify the number and the range of credits eligible for college credits in the career field. These are the potential credits a student can earn if s/he meets the postsecondary requirements for earning postsecondary credit. 8
9 INSTRUCTIONS FOR SECOND PAGE OF POS Section 1: Transfer Career-Technical Credit List the additional courses that are eligible for postsecondary articulated credit. Identify the credit granting institution, type of credit offered (use dropdown menu), and amount of credit to be earned. Types of articulated credit include: Bilateral Agreements, Statewide Career-Technical Assurance Credit (CTAN), Ohio Transfer Module (OTM), Dual, Advanced Placement or International Baccalaureate Credit. Section 2: Industry Certifications In the space provided, list industry credential opportunities. Such opportunities may include vendorbased or vendor-neutral certifications, industry or state licenses. EXAMPLES: NOTE: Recommended credentials and industry certificates are located on the FY2013 Career- Technical Education (CTE) Technical Assessment System Matrix. The list of credentialing opportunities is not comprehensive and is not intended to limit the credentials that students should strive to earn. Section 3: Supplementary Learning Opportunities Approval of new Career-Technical Education pathway program applications or applications for renewal of existing pathway programs is contingent upon complying with the POS assurances (listed on signature page). ASSURANCE 3j states: Students enrolled in the program will have access to a career-technical student organization(s)(ctsos). Check the CTSO s aligned to the pathway program and any supplementary learning opportunities accessible to the student enrolled in this pathway program. EXAMPLES: 9
10 Section 4: Career Options List the potential occupations students will be prepared for in this pathway and the required level of education and training needed. The Ohio Board of Regents maintains an information repository system that provides a means for information to be collected, organized, shared, searched and utilized. The resourses on this system include information for Ohio's public colleges, universities, and adult education programs. Additionally, career options can be resarched and quick links to school profiles, degree programs and career prospects can be accessed. INSTRUCTIONS FOR SIGNING AND SUBMITTING THE APPLICATION To facilitate the approval process for the CTE-26/POS, the Ohio Department of Education (ODE), Office of Career-Technical Education, designed the form to be submitted electronically. Once the PDF is signed and saved electronically, it can be attached to the to the Office of Career- Technical Education. A (CTE)/Tech Prep POS is not considered to be final unless it is submitted with a signed (electronic signatures) POS Assurance page. Note: A paper copy of the CTE-26 application will not be accepted by the Office of Career-Technical Education in FY15. Before you start the signing and submitting process of the CTE-26 application, you should first understand a few basic functions of the section. 1. The Submit to Ohio Department of Education action button will not be functional until the CTPD Superintendent s electronic signature is recognized. 2. The order or hierarchy in which the signatures should be obtained should follow: a) District Superintendent b) Post-Secondary College President or designee signature and/or Apprenticeship Sponsor or Adult Training Superintendent signature c) CTPD Superintendent Signature 3. The CTPD Superintendent will use the action buttons for APPROVAL/DISAPPROVED. 10
11 STEP 1: Create a self-signed digital ID A digital ID is like an electronic driver s license or passport that proves your identity. A digital ID usually contains your name and address, the name of the organization that issued it, a serial number, and an expiration date. Digital IDs are used for certificate security and digital signatures. 1. Do one of the following: Click in the Signature box and continue to Step 3 In Acrobat, choose View >Tools > Sign & Certify > More Sign & Certify > Security Settings. In Reader, choose Edit > Protection > Security Settings. 2. Select Digital IDs on the left, and then click the Add ID button 3. Select the option A New Digital ID I Want to Create Now, and click Next. 11
12 4. Specify where to store the digital ID, and click Next. New PKCS#12 Digital ID File: Stores the digital ID information in a file, which has the extension.pfx in Windows and.p12 in Mac OS. You can use the files interchangeably between operating systems. If you move a file from one operating system to another, Acrobat still recognizes it. Windows Certificate Store (Windows only): Stores the digital ID to a common location from where other Windows applications can also retrieve it. EXAMPLE: 5. Type a name, address, and other personal information for your digital ID. When you certify or sign a document, the name appears in the Signatures panel and in the Signature field. 12
13 6. (Optional) To use Unicode values for extended characters, select Enable Unicode Support, and then specify Unicode values in the appropriate boxes. 7. Choose an option from the Key Algorithm menu. The 2048-bit RSA option offers more security than 1024-bit RSA, but 1024-bit RSA is more universally compatible. 8. From the Use Digital ID For menu, choose whether you want to use the digital ID for signatures, data encryption, or both. 9. Type a password for the digital ID file. For each keystroke, the password strength meter evaluates your password and indicates the password strength using color patterns. Reconfirm your password. STEP 2: Signing the CTE-26 Application using your digital ID SECONDARY SCHOOL SIGNING INSTRUCTIONS 1. Once you have completed the CTE-26 form, type in the name of the District Superintendent, the name of the District and the Date. Next, click on the Superintendent Signature box to insert the digital ID signature. A set of prompts will follow. Enter your password and then click Sign. 13
14 2. Save (to your desktop) Please DO NOT change the file name. 3. Once you have completed the secondary section of the application and signed your portion of the form, send the application by to the post secondary contact for the College President s and/or Apprenticeship Sponsor/Adult Training Superintendent s signature. POST SECONDARY SCHOOL SIGNING INSTRUCTIONS 1. Complete your portion of the application and then select the corresponding Signature box to sign the document. 2. Once you enter your digital signature (for instructions on how to create a digital signature, refer to Step 1: Create a self-signed digital ID, page 10), it will ask you to save the document. Please do not change the file name. 3. Once you have completed the post secondary section of the application and signed your portion of the form, return the application by to the secondary school s contact address. Use the attachment tool on the ribbon bar to attach your saved application document. Click send. The post secondary portion of the application is now complete. SECONDARY SCHOOL: Submit the CTE-26 form electronically to the CTPD Superintendent. 1. Once the secondary and post secondary sections of the application are complete and signatures are received, the application will be forwarded to the CTPD Superintendent for approval. IMPORTANT The lead district in the CTPD shall approve or disapprove the program application by March 1st. If a program is approved the lead district shall notify the ODE of its decision no later than March 15th. If a program is disapproved, the lead district shall notify the district or school of the disapproval or the decision to take no action. The submitting district has the opportunity to appeal this decision to the ODE no later than March 15th. 14
15 LEAD DISTRICT/CTPD SUPERINTENDENT: Approving and submitting CTE-26 applications. 1. Once you have received the completed CTE-26 form, type in the CTPD Superintendent name, the name of the CTPD and the date. Next, click on the CTPD Superintendent Signature box to insert the digital ID signature. A set of prompts will follow. Enter your password and then click Sign. NOTE: For instructions on how to create a digital signature, refer to Step 1: Create a self-signed digital ID, page Next, the CTPD Superintendent will use the action buttons for APPROVAL/DISAPPROVED. 3. Once you have completed and signed your portion of the form, click on the Submit to Ohio Department of Education action button. NOTE: The Submit to Ohio Department of Education action button will not be functional until the CTPD Superintendent s electronic signature is recognized. 15
16 4. You will be given a prompt to select an option for sending . Select and click ok. 5. This button will take you to your account and automatically enter the address that the form needs to be sent to (odecte26@education.ohio.gov) and the subject line pulls from the District Name field from the PDF Form. Click Send. Following approval of a CTE/Tech Prep POS by the Ohio Department of Education, users are encouraged to adapt the CTE/Tech Prep POS template for other local uses to inform and advise students/parents/teachers and others. The template components can be used to inform and increase stakeholder awareness about opportunities the POS provides. 16
17 SUPPLEMENTAL INFORMATION CTE- 26 Technical Manual Completion of the JTC/CBI and FCS/GRADS templates are similar to the WFD Program Application template except they do not require the submission of a Program of Study showing articulated college credit. Complete identification information requested at the top of the template including institution name and specific program information in addition to the CTPD Superintendent s signature are required. Post Secondary signatures are not applicable. When applying for a FCS program, two program types are available in the drop down box: Family Consumer Science (FCS)-Middle School Only and Family Consumer Science (FCS) for high school delivery. A separate application is needed for each program type (option). Select the appropriate application for where programming is being delivered. Applications reflecting the (EO) Early Childhood Education and E1 Teaching Professions Career Field pathways require a CTAG Addendum for Introduction to Education. Additional digital signatures are required to complete this section and are embedded on the program information page of the application. These required signatures should be obtained before submission to Post Secondary College President and CTPD Superintendent for signing.
