Charges begin when the designated communication termination (s) is/are accessed and enabled thereby ( connected ) to receive the communication from the originating location on the network. Charges cease when the termination(s) is/are disconnected. 1.2 Distance Sensitivity The Company s charges are distance insensitive Dial-U~ Service Carrier s 1+ Dial-Up Service is a switched equal access outbound service using standard equal access dialing to place interexchange calls from customer premises to points located within Indiana, the rates of which are based on usage. Rates and charges for Carrier s 1+ Dial-Up Service are set forth in Section Callinq Card Service Carrier s Calling Card Service permits customers to make calls from any location within Wyoming to any other location by dialing I+800+NXX+XXXX, receiving a signal tone, then dialing in the customer s personal identification number (PIN) assigned by Carrier followed by the telephone number of the called party within Wyoming. Additional calls may be made without having to redial the 800 number or the PIN by pressing the key on touch-tone telephones for one second, receiving the signal tone and dialing the called party s number. Rates and charges for Carrier s Calling Card Service are set forth in. Section 2. Rates and charges for Carrier!s 800 Service are set forth in s4.3 following. 1.6 Connect Proqram Service Carrier s Connect Program Service offers switched outbound, inbound, and calling card services at rates that are distance insensitive, with time-of-day discounts applicable. Long term discounts are also available. 1.7 Dedicated Network Service Dedicated Network Service is available for outbound calling. Calls originate from customer-provided dedicated access lines. No minimum commitment is required. 1.8 Dedicated 800 Service Carrier s Dedicated 800 Service is available for incoming calls. Calls originate from any interstate or intrastate location over an 800 number and terminate to a customerprovided dedicated access line. No minimum commitment is required.

Billing is in one minute increments and no customer will be billed for an uncompleted call. This service includes the completion of collect, station to station, person to person, and credit card calls by PROCOM S operators and is separated into the following categories Operator services to end users pre-subscribed to the company. All operator services to end users who have presubscribed to PROCOM S services or who have a contractual or working relationship with the Company are charged the rates for operator assistance as outlined in Section Operator payphones, locations. services hotels, to local exchange company motels, and other transient Operator services to privately owned payphones.,!,..,,.,:

Usage begins when the called party picks up the receiver. When the called party picks up is determined by hardware answer supervision in which the local telephone company sends a signal to the switch or the software utilizing audio tone detection. A call is terminated when either party hangs up Calculation of Distance Usage charges for all mileage sensitive products are based on the airline distance between rate centers associated with the originating and terminating points of the call. The airline mileage between rate centers is determined by applying the formula below to the vertical and horizontal coordinates associated with the rate center involved. The Company uses the rate centers and associated vertical and horizontal coordinates that are produced by Bell Communications Research in their NPA-NXX V & H Coordinates Tape and Bell s NECA Tariff No. 4. Calling Card Rates are determined on the basis of six-second increments, by time of day, day of week and mileage basis as specified in the following rate table, plus the applicable surcharge. The minimum call duration for billing purposes is thirty (30) seconds.
