3 Baptist Health Plan Advantage HMO Plan Silver & Fit Directory Welcome to the Baptist Health Plan Advantage supplemental Silver & Fit fitness benefit. The fitness benefit allows you to join network fitness facilities at no cost to you. To get additional information about your fitness benefit and coverage, please see your Evidence of Coverage included in your Welcome Package or contact our Member Service Department at the numbers listed below. The fitness providers listed in this directory have agreed to provide you with fitness services. Please note, you must select participating fitness providers listed in this directory. If you obtain care from non-network providers, neither Medicare, nor Baptist Health Plan Advantage will be responsible for the costs. This directory provides a list of Baptist Health Plan Advantage s fitness providers. This directory is for the Central Region and Southern Region counties in Kentucky which include: Central Region: Bourbon, Boyd, Clark, Estill, Fayette, Garrard, Greenup, Jessamine, Larue, Lawrence, Madison, Nelson, Oldham, Scott, Shelby and Woodford. Southern Region: Allen, Barren, Butler, Calloway, Edmonson, Graves, Henderson, Hopkins, Logan, Marshall, McCracken, Muhlenberg, Pulaski, Simpson, Union, Warren, and Whitley. This directory is current as of June 15, Some fitness providers may have been added or removed from our network after this directory was printed. We do not guarantee that each provider is still accepting new members. To get the most up-to-date information about Baptist Health Plan Advantage s fitness providers in your area, you can visit or call our Member Service Department at or toll-free From October 1 through February 14, a representative will be able to speak with you from 8:00 a.m. to 8:00 p.m. in your local time zone, 7 days a week. Beginning February 15, through September 30, a representative will be available to speak with you from 8:00 a.m. to 8:00 p.m. in your local time zone, Monday through Friday.
