1 HOW DO I GET PAID PICK ONE on your Employee Information Sheet You can choose to be paid by Direct Deposit to your checking or savings account, or you can enroll in the ADP Visa Debit Card. Direct Deposit Complete attached form All wages will be directly deposited to the employee s bank account on payday. Complete the Direct Deposit form and submit to your Supervisor. Attach to the Direct Deposit form a voided check from your account or any document from your bank showing BOTH the bank s routing number AND your account number. If your bank account information changes, you must notify the park district and complete a new Direct Deposit form. OR ADP Debit Card YOU MUST BE 16 YEARS OF AGE TO ENROLL IN THE ADP DEBIT CARD If you select this option on your Employee Information Sheet, the Park District will enroll you in the Debit Card program. There are no additional forms to fill out. All wages will be directly deposited to the employee s ADP Visa Debit Card on payday. You will receive your personalized VISA card in the mail. Phone ADP-4321 to activate your card. 3 ways to access your money: o The ADP Visa Debit Card can be used at any retail store for purchases and cash back. o Money can be withdrawn at any VISA member bank (look for VISA sign in window of any bank) o Allpoint ATMs (located in most Walgreen s, Target, CVS, 7-11). One (1) ATM per pay periods is free. Each additional ATM withdrawal per pay period will incur a $3.00 fee. You can sign up for and/or text message alerts to get real-time balance updates by or text messages. You can download the ADP Mobile App to find ATMs and view transactions. The card will remain active until it has not been used for 12 consecutive months. This means you will use the same card from one summer season to the next. Don t lose it! 2 EMPLOYEE DIRECT DEPOSIT ENROLLMENT FORM To enroll in Direct Deposit, simply fill out this form and return it to your Supervisor. You must attach a voided check for each checking account not a deposit slip or bank statement. If you do not have checks, you can submit any document from your bank that includes your account number, bank routing number and type of account. Banks are very familiar with Direct Deposit; if you tell them you are setting up direct deposit with your employer, they will know exactly what to give you. Below is a sample check detailing where the information necessary to complete this form can be found. Important! Please read and sign before completing and submitting. I hereby authorize the Vernon Hills Park District (herein after the Park District) to deposit any amounts owed me by initiating credit entries to my accounts at the financial institutions (herein after the Bank ) indicated on this form. Further, I authorize the Bank to accept and to credit entries indicated by the Park District to my accounts. In the event that the Park District deposits funds erroneously into my account, I authorize the Park District to debit my account for an amount not to exceed the original amount of the erroneous credit. 5 HOW DO I SEE MY PAYCHECKS? Employee Services Portal To view your paychecks, go to The first time you go to the website, click on Enroll in the employee services portal. Enter your name, in ALL CAPS. Typically, this will be your FIRST NAME MIDDLE INITIAL LAST NAME with one space before and one space after your middle initial, and no punctuation. If you do not have a middle initial, it will be FIRST NAME LAST NAME with a space between the two. If you get an error message that tells you that there is no data for you, please contact Debi Minske at Complete the Payroll Information tab. Click NEXT when complete. Complete the Log In tab. See below for tips on creating your login and password. Click NEXT when complete. On Options tab, be sure the Personal Information receipt is checked. Click Finish. Log in using the log in and password you just created. Click on View Paychecks on the left. Select the paycheck you want to view. 3 years worth of paychecks will be retained on the Employee Services Portal. You can also view and print your W-2 forms from this website. Employee Services Portal FAQs Q: I can t get onto the blasted Employee Services Portal to see my paycheck. It keeps telling me there is no data for me. What do I do? A: You must type your name in ALL CAPS exactly as it appears in our payroll system. If you don t know how your name has been entered and haven t been successful seeing your data, you can call Debi Minske at and she will look it up for you. Q: What should I enter as my log in name? A: If you re a new user, enter any combination of letters and numbers you ll remember as your login name. This is case sensitive and must contain 6 to 12 characters, and should not contain any spaces or special characters (i.e. &, >, *, Q: What should I enter as my password? A: If you re a new user, enter a unique letter/number combination only you will know. Your password should be different from your login name. It is case sensitive and must contain 6 to 12 characters, and should not contain any spaces or special characters (i.e. &, >, *, Your password must contain at least one numeric character (i.e. penny1). Q: What if I forget my login name and/or password? A: Select the Forgot Login/Password link and enter your address. Your help question pops-up.
