National Healthcare Safety Network (NHSN) Introduction & Enrollment Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 1
Introduction to NHSN Secure, internet-based surveillance system Developed and managed by the Division of Healthcare Quality Promotion (DHQP) at CDC Free to participating facilities but requires signed agreement with CDC Enrollment on NHSN is separate from participation in the SHARP surveillance of HAI project using NHSN data Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 2
Purposes of NHSN Collect data from healthcare facilities: To permit valid estimation of magnitude of adverse events among patients To permit valid estimation of adherence to practices known to prevent HAIs To analyze and report collected data to permit recognition of trends To provide facilities with risk-adjusted data to be used for inter-facility & national comparison Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 3
NHSN Components Three basic components of NHSN: Patient Safety Device-Associated Procedure-Associated MDRO/CDAD Healthcare Personnel Safety Biovigilance Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 4
Patient Safety Component Modules Device-Associated Modules: Central line-associated bloodstream infection (CLABSI) Central line insertion practices adherence (CLIP) Ventilator-associated associated pneumonia (VAP( VAP) Catheter-associated associated urinary tract infection (CAUTI) Dialysis Event (DE( DE) Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 5
Patient Safety Component Modules (cont d) Procedure-associated Module options: Surgical site infection (SSI) Post-procedure pneumonia (PPP) Multidrug-Resistant Organism/C. difficile- associated Disease (MDRO/CDAD) Module Infection Surveillance Laboratory Identified (LabID( LabID) ) Events Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 6
NHSN Enrollment Process done through DHQP at CDC Visit NHSN website at Requirement for hospitals in CMS Hospital Inpatient Quality Reporting Program (effective January 2011) must report CLABSIs Additional CMS requirements in 2012 SSIs Currently does not apply to critical access hospitals Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 7
Checklist for Enrollment 1. Identify Facility Administrator 2. CDC training 3. Facility Administrator registration 4. Digital certificate 5. Facility enrollment 6. Facility activation 7. Facility set up 8. Monthly reporting plan 9. Confer rights to SHARP (optional) 10. Surveillance Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 8
Step-by by-step Enrollment Guide ippsrule_training.html Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 9
Step 1: Identify NHSN Facility Administrator Each facility can have only one Has the most administrative rights on the system Can appoint others with various administrative rights as backup Typically the primary user of NHSN: IP, hospital epidemiologist, etc. Only person who can join a group and confer rights Will enroll the facility in NHSN Should assign new NHSN Facility Admin before leaving the facility Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 10
Step 2: CDC Training Archived webcast with slide set format ~ hours for required modules Required by CDC before a NHSN Facility Administrator or user can enroll on the system Discusses data collection, numerators, denominators and data entry Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 11
Step 3: Facility Admin. Registration Review CDC s NHSN Facility Administrator Enrollment Guide for details on enrollment rollmentguidecurrent.pdf Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 12
Step 3: Facility Admin. Registration, cont d. Read and accept the NHSN Rules of Behavior Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 13
Step 3: Facility Admin. Registration, cont d. Complete registration information Ensure accurate e address You will need to enter: A facility identifier Date you completed CDC NHSN training Slide adapted with permission from Wisconsin Department of Health Services Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 14
Step 4: Digital Certificate Authenticates your access when logging onto CDC s s Secure Data Network Do not share your digital certificate; they are user-specific specific (including Facility Administrator) Digital certificate can be installed on multiple computers for the same user CDC will be switching to a user password Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 15
Step 4: Digital Certificate, cont d CDC registration confirmation will include the digital ID enrollment link to apply for a digital certificate If you already have a CDC digital certificate, do not register for another Follow the instructions in each CDC during the digital certificate process Follow CDC steps in order Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 16
Step 4: Digital Certificate, cont d Have an IT staff member at your facility install the digital certificate if possible Installation instructions are also in the Facility Administrator Enrollment Guide available at the NHSN website Create a backup copy of the certificate Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 17
Step 5: Facility Enrollment Access NHSN through the SDN You must use Internet Explorer with NHSN Select NHSN Enrollment in the upper left corner Print enrollment forms applicable to your facility (you will submit these online, but this will allow you to gather necessary information) Complete facility contact info and annual facility forms online Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 18
Step 6: Facility Activation Print the Agreement to Participate and Consent form Have a hospital administrator sign the form Return it to CDC in hard copy CDC will notify you after they have activated your hospital on NHSN You are now ready to log in and begin using NHSN Surveillance for Healthcare-Associated & Resistant Pathogens (SHARP) Unit 19
