2 Ambulance Community Care Plan The Pennine Acute Hospital Trust is working in partnership with the North West Ambulance Service and the Out of Hours GP service (GotoDoc), to implement the Yellow Ambulance Community Care Plan (CCP) This leaflet provides information, to you as the patient or carer, around the Ambulance Community Care Plan, how it works and how it could be beneficial to you. The North West Ambulance Service is forecasting to have 54, 000 CCPs in place by What is the Yellow Ambulance Community Care Plan (CCP)? The Yellow Ambulance CCP is a single yellow coloured A4 Document. The CCP is a simple care plan that is completed by you and your healthcare professional and details your health care information. The CCP will be kept at your home. A copy of the CCP will be provided to the North West Ambulance Service and stored on their secure data system (ERISS). This is to inform NWAS that there is a CCP at your home. If you need to contact an ambulance, the ambulance staff will be alerted to the CCP at your home. The ambulance service will assess your health needs and determine if you are able to be supported at home with the right community services. The ambulance service will contact the appropriate service and ensure that you are safe with the right response time from the community services. 2
3 Who will have an Ambulance Community Care Plan? The Ambulance Community Care Plans (CCPs) will be distributed by the Communtiy Services of Pennine Acute Hospital Trust and are for the people of North Manchester who have long term conditions and people who are prone to falls. How will the Ambulance Community Care Plan work? When you phone 999 and an ambulance is on its way, the ambulance crew/paramedic will be alerted to the CCP that is in your home. The CCP is bright yellow in colour so that it is easily identified by the paramedic. The ambulance crew have a specific assessment tool they use called 'the Paramedic Pathfinder. The Pathfinder is a range of triage assessment tools that are evidence based and clinically safe. On arrival at your home the paramedic will complete a face to face assessment. Through using the Paramedic Pathfinder for specific symptoms and your CCP they will determine the best care pathway for you. The CCP enables the Ambulance crew to identify when a referral to expert community teams may be the better option for you. There are a wide range of Specialist Community Services that are available to help support you at home. These include The Crisis Response Team, Community Matrons, Respiratory Nurse Specialists (ARAS), Falls Prevention Team, District Nurses and Social Care Workers. 3
4 There will be out of hours service provided by GotoDoc. If the ambulance crew feel that you need further support but you do not need to go to hospital, and it is after 8pm and before 9am then GotoDoc will provide the support you need. What are the benefits of the Ambulance Community Care Plan? The CCP will prevent an unnecessary admission to hospital, when this is not the best option for you. The care and support provided as a result of the CCP will be individualised to your specific health needs. The care provided through the CCP will improve your understanding and knowledge of the services that are available to support you at home. There is a strong evidence base showing that people cared for at home have a better recovery from ill health, and that care at home increases independence and provides a better quality of life. CCPs will improve the integration and working relationship of services, from different organisations to provide better care for you. The Pennine Acute Hospital Trust Community Services working alongside North West Ambulance Service enables the right care, in the right place, at the right time, by the right person. 4
