9 An Accountable Community for Health is a multipayer, multi-sector alliance of major health care systems, providers, and health plans, along with public health, key community and social services organizations, schools, and other partners serving a particular geographic area. An ACH is responsible for improving the health of the entire community, with particular attention to achieving greater health equity among its residents.

12 ACH Evaluation Framework Develop an evaluation framework for evaluating Accountable Communities for Health Work in close coordination with the foundation effort CHHS has contracted with The Blue Sky Consulting Group to develop the evaluation framework Challenge: developing an evaluation framework that captures each community s nuances
13 Data-Sharing and Toolkit Conduct literature review and environmental scan of current data and information sharing practices occurring within an Accountable Communities for Health (ACH) type of structure or collaborative Develop a toolkit that outlines the spectrum of data and information needs of an ACH Describe an optimal data and information sharing system Identify options for an ACH to create a data and information sharing capacity where none exists Identify options to improve upon and maximize an existing system
