Lessons Learned Benefits of having an exclusive COE provider Importance of the non-surgical benefit High-touch and collaborative culture Patient Provider Plan 11

Keys to Success: Benefit Plan Considerations Require employees to be treated by Centers of Excellence Through health plan (TPA) or ASMBS/SRC COE lists Cover bariatric surgery at a greater benefit level than other conditions Thoughtful co-pay, deductibles, etc. Incent patients for long-term compliance Co-pay reimbursement Pay for plastic surgery with compliance to follow-up physician/care team appointments Continued support through onsite weight loss & wellness programs Jim s Plate Onsite wellness coaches 12

The Direct Care Health Plan Conducted extensive research on best practices for health care delivery. Sought input from health care delivery thought leaders, and reviewed case studies of established programs. Every source lead back to the importance of a delivery system predicated upon the direction of care provided by the primary care physician. Created a plan called the Direct Care Health Plan, built upon the principles of a patient-centered medical home.

The Direct Care Health Plan Developed direct contractual relationships with 18 PCPs and spent countless hours discussing how best to adapt the principles of the medical home model to the unique challenges that Nevada s health care system presents. Patient access to medical care - Wait times delay timely identification and treatment of conditions Appropriate physician reimbursement - Value physicians for their time, which allows sufficient time to accurately diagnose, treat and answer patient questions. In short, we have built this plan to reimburse providers more for quality and outcomes. Patient engagement in and accountability of their health - Prevention/early detection/annual physical Use of technology - Physician collection and review of medical data to increase the efficiency of medical care provided and proactively manage the health of their patient population

The Direct Care Health Plan More than 2,500 employees enrolled in the Direct Care Plan, representing over 5,000 people After 8 months of experience, the early results are extremely encouraging. 97% of Plan members obtained annual physical 98% of employees 95% of spouses 90% of adult age dependents 3% who did not obtain represent 67 employees 115 dependents Success stories - We found patients who were lost in the system finally getting the help they needed to patients who were immediately admitted to the hospital upon arrival to their primary care physician s office for an annual wellness physical.

Healthy Eating. Healthy You. Commitment to remove and reduce barriers to making healthier eating decisions

Healthier Eating at Home Provide a solution for employees to eat healthier at home

Based on the published literature, the most cost-effective way to prevent obesity in youth is the Coordinated Approach to Child Health (CATCH), a comprehensive intervention to promote healthy eating and physical activity in elementary schools, which costs $900 per QALY saved CORNELL ECONOMIST JOHN CAWLEY Health Affairs, 2010

Surgical Out-of-Pocket Reimbursement Requirements 1 year post- surgery, program member must have attained 40% targeted weight loss, with an additional loss of 10% per year at the 2 year, 3 year, and 4 year surgical anniversary (a total of 70% at year 4). Weight loss target to be determined by the Surgery Center/Surgeon and agreed upon by program member prior to surgery. Weight loss will be monitored by the Surgery Center and will be reported to the Health Plan for consideration of reimbursement. Program member must maintain continuous Health Plan enrollment to be eligible to receive the reimbursement. Continuation of coverage under COBRA meets this requirement. The reimbursement will be in the form of a check issued by Health Plan s claim administrator and payable to program member. Out of pocket expenses include the surgery co-pay of $5,000 (prescription drug co-pays or over the counter supplements are not included). Reimbursement request forms are to be completed by both the Surgery Center and the program member attesting to adherence to program requirements.

Cosmetic Surgery Reimbursement Requirements Minimum of 2 years post-surgery, or when member has lost 70% of excess weight Weight loss will be monitored by the Surgery Center to be reported back to Health Plan to receive this incentive The total contribution for the plastic surgery incentive is $5,000 which can be used with a physician of the member s choice. Member must maintain active and continuous enrollment in health plan to receive this incentive Member must have participated in a minimum of 6 support groups per year in addition to the 4 mandatory psychological post-op consultations. The Surgery Center will determine if member meets criteria for cosmetic services prior to cosmetic procedure Cosmetic services must be performed and the member must submit for reimbursement, including receipt showing payment of $5,

Testimonial #1 It is said that we cannot enter the womb to be born again, but SWCC has done the next best thing. I have a new life thanks to the surgery, classes, and nutritional guidance provided at SWCC. Having lost 95 lbs over the last year has given me a new life. The energy levels are up, I am able to do some of the simplest things, like tying shoes, that I was not able to do before. Truly, I have been born anew. Thank you SWCC! - CC, Poker Room Manager at MGM

Testimonial #2 I came to one of the seminars Dr. Soong gave. I was weighing about 274 lbs. I m a 62 year-old man. I was done weighing this much I had the surgery in July 2011 and as of now I m at 214 lbs. I feel wonderful I no longer take any pills for anything. I go to the gym 3 or 4 times a week I would do this all over again if I had to. - FM, Supervisor at MGM Hotel

Testimonial #3 My experience with this entire program has been phenomenal. The Mind Over Eating classes were an amazing learning experience and an invaluable tool. I have had no complaints or problems after the surgery. I feel so much better, and have so much more energy. I have successfully eliminated all of my medications. The opportunity to participate in this program and have the surgery is truly a gift that I otherwise would not have been able to do and I am truly thankful. - DS, Cage Cashier at Aria

Testimonial #4 Since enrolling into MGM s Direct Care program, I feel as if the quality of my care improved tenfold. My Direct Care doctor, reminds me of the old-fashioned doctors that used to do house calls; patient, caring, attentive, and pays attention to every detail. I had only been a patient under the Direct Care program for one month before needing their services and never once was I denied services because of my short membership period. Every specialist I ve been referred to by my doctor has had the same professional attitude, concern for my well-being and ready to fix whatever was broken. - Employee

Testimonial #5 The Direct Care plan looked interesting for us because it encourages the medical practices to provide comprehensive care for patients while compensating providers with above average reimbursement rates, compared to other health plans, he said. For my patients, it encourages them to establish a relationship with me, which ultimately results in better overall health care for the patient. Direct Care Primary Care Physician

Direct Care Health Plan Physician Reimbursements Global Fee for Service Per office visit reimbursement is averaging $60 greater than PPO pricing (84% increase) Care Management Fee Projected Jan Dec 2012 total of $235k $6 PMPM Average per physician payment $12,900
