5 Incorporating Innovative Benefit Designs Metabolic Surgery for Comorbid Resolution Goal: To create a surgical weight loss alternative to those meeting specific qualifications which will ultimately help program members achieve a healthier status. Eligibility Employees are eligible after 91 days of employment Must be age 18 and over Maintain continuous enrollment in the MGM-sponsored Health Plan Includes COBRA members who maintain eligibility in the Health Plan 5
6 Program Requirements of the Patient Evaluation by a designated program physician to ensure all requirements are met Program criteria include the following. A body Mass Index (BMI) of 40 or greater; or A BMI between with at least one clinically significant health issue, including but not limited to, cardiovascular disease, Type 2 diabetes, hypertension, coronary artery disease or pulmonary hypertension. Participation in a medically supervised nutritional counseling 6 month educational process inclusive of 13 classes covering nutrition, exercise and weight management. Psychological Evaluation Participation in a pre-op/post-op program 6
7 Metabolic/ Bariatric Surgery Benefit Design Covered Procedures: Bypass Sleeve Gastrectomy Gastric Band $5,000 copay 20% coinsurance related to incidentals related to preoperative clearance tests, prescription co-pays $20 co-pay for office visits associated with the medically supervised weight loss program A stop loss benefit to minimize plan exposure for expenses related to complications. 7

9 The Results: Surgical Program 88 patients have had surgery Average weight loss during pre-surgery medically supervised weight management program was 18 pounds. Average weight loss was 55 pounds, up to 227 pounds for 14 months post-surgery. 56.6% reduction in medications taken post-surgery during the first 6 months of the program. 80.3% reduction in co-morbidities post-surgery during the first 6 months of the program. 13 The Direct Care Health Plan Conducted extensive research on best practices for health care delivery. Sought input from health care delivery thought leaders, and reviewed case studies of established programs. Every source lead back to the importance of a delivery system predicated upon the direction of care provided by the primary care physician. Created a plan called the Direct Care Health Plan, built upon the principles of a patient-centered medical home.
14 The Direct Care Health Plan Developed direct contractual relationships with 18 PCPs and spent countless hours discussing how best to adapt the principles of the medical home model to the unique challenges that Nevada s health care system presents. Patient access to medical care - Wait times delay timely identification and treatment of conditions Appropriate physician reimbursement - Value physicians for their time, which allows sufficient time to accurately diagnose, treat and answer patient questions. In short, we have built this plan to reimburse providers more for quality and outcomes. Patient engagement in and accountability of their health - Prevention/early detection/annual physical Use of technology - Physician collection and review of medical data to increase the efficiency of medical care provided and proactively manage the health of their patient population
15 The Direct Care Health Plan More than 2,500 employees enrolled in the Direct Care Plan, representing over 5,000 people After 8 months of experience, the early results are extremely encouraging. 97% of Plan members obtained annual physical 98% of employees 95% of spouses 90% of adult age dependents 3% who did not obtain represent 67 employees 115 dependents Success stories - We found patients who were lost in the system finally getting the help they needed to patients who were immediately admitted to the hospital upon arrival to their primary care physician s office for an annual wellness physical.

27 Surgical Out-of-Pocket Reimbursement Requirements 1 year post- surgery, program member must have attained 40% targeted weight loss, with an additional loss of 10% per year at the 2 year, 3 year, and 4 year surgical anniversary (a total of 70% at year 4). Weight loss target to be determined by the Surgery Center/Surgeon and agreed upon by program member prior to surgery. Weight loss will be monitored by the Surgery Center and will be reported to the Health Plan for consideration of reimbursement. Program member must maintain continuous Health Plan enrollment to be eligible to receive the reimbursement. Continuation of coverage under COBRA meets this requirement. The reimbursement will be in the form of a check issued by Health Plan s claim administrator and payable to program member. Out of pocket expenses include the surgery co-pay of $5,000 (prescription drug co-pays or over the counter supplements are not included). Reimbursement request forms are to be completed by both the Surgery Center and the program member attesting to adherence to program requirements.
28 Cosmetic Surgery Reimbursement Requirements Minimum of 2 years post-surgery, or when member has lost 70% of excess weight Weight loss will be monitored by the Surgery Center to be reported back to Health Plan to receive this incentive The total contribution for the plastic surgery incentive is $5,000 which can be used with a physician of the member s choice. Member must maintain active and continuous enrollment in health plan to receive this incentive Member must have participated in a minimum of 6 support groups per year in addition to the 4 mandatory psychological post-op consultations. The Surgery Center will determine if member meets criteria for cosmetic services prior to cosmetic procedure Cosmetic services must be performed and the member must submit for reimbursement, including receipt showing payment of $5,
