2 Social Care & Housing Services This directory only lists those care homes within the boundaries of, although we support residents in other care homes outside. Details of care homes in other areas are available from the relevant Local Authorities. A placement in Care Home can be funded by Social Care and Housing (including Direct Payments from April 2016), or privately funded by yourself. If you have a primary health need and come under Continuing Health Care (CHC), the Aneurin Bevan University Health Board will fund your placement in a care home. Giving up your own home and moving into a care home is a very big step for anyone to take. Like any important decision, it needs careful thought and you should not rush into it. All care homes in are run by independent/private organisations, and are regulated and inspected by the Care Social Services Inspectorate Wales (CSSIW). You may wish to view a home s latest inspection report to help you decide which home is right for you. These reports can be found on You may also be interested in viewing feedback from people who have stayed in a particular home by visiting and searching for homes in a particular area of. All residential and nursing homes have a complaints procedure, however, it s best to discuss any on-going problems with the home s manager to see if they can be resolved informally. If your concerns cannot be resolved we will support you in choosing an alternative setting. After a discussion with the home s manager, if you still feel dissatisfied, you have the right to pursue your complaint through the Council which can be done by calling All Care Homes are regularly monitored by Social Care and Housing, to ensure that they meet the necessary requirement to enable them to keep people safe and provide a quality service. We have produced a Factsheet entitled Choosing a Care Home that provides more information; this is available on the website, or speaking to your social worker or Customer Care on We have also produced a Factsheet entitled Paying for Residential Care that provides more information; this is available on the website, or speaking to your social worker or Customer Care on Both Factsheets should be read in conjunction with this document to help you decide what type of accommodation is right for you, as there are a range of accommodation options available: Residential Care This type of care and support is for people who have assessed eligible needs that mean that the support needed is best provided in a care home environment with their peers and 24 hour support on hand to help. People who live in residential care homes do not have any additional health needs requiring nursing support. 2

3 EMI Residential This type of accommodation is for people having assessed eligible needs for residential care, but who are also living with a form of moderate dementia or other forms of mental health issue Increasingly, Care staff in Residential with Dementia Care services have additional specialist training. Nursing This type of accommodation is for people having assessed eligible needs for care and support of a higher level for people who require more intensive physical support on a daily basis as well as funded nursing care. The homes in this category of care are expected to have a proportion of fully qualified and highly trained nursing staff on duty and have dedicated nursing equipment, such as specialist beds and assisted bathrooms. EMI Nursing This type of accommodation is for people having assessed eligible needs for care and support of a higher level for people who require more physical support as well as funded nursing care and who are also living with a form of dementia or other mental illness. Again, this type of service is expected to have a proportion of qualified nursing staff, on duty who will also have had specialist dementia training. Home Care / Support at Home Home Care, otherwise called Domiciliary Care or Home Based Care, is care provided to you in your own home, enabling you to remain as independent as possible in an environment you know and love. Extra Care Housing Extra Care housing (sometimes called Assisted Care or Supported Living) allows you to retain your independence by having your own front door, while knowing care is on hand if you need it. Supported Living Supported Living schemes are usually for people with learning disabilities or mental health problems, allowing you to remain independent by having your own front door, but to know a support worker is available should you need them. Respite Care / Short Term Care Respite care is typically offered in either a care home setting for a short period of time, or whilst an unpaid carer is on holiday. Respite stays can be from just a few hours/ days, up to 28 days in a year dependent on need. Intermediate Care / Reablement Care Intermediate care or reablement care is generally provided to support someone for up to 6 weeks after a hospital stay or to avoid a hospital admission. The intention of intermediate / reablement care is to rehabilitate someone to be able to return home independently or with a level of home care support. Intermediate / reablement care is normally offered in a care home setting, with services including physiotherapy or occupational therapy. 3
