13 ST ~ MEDHOST, Inc Carothers Parkway, Suite 100 Franklin, TN August 18, 2015 To whom it may concern, MEDHOST, Inc. uses a single, standard Quality Management System (QMS) in the development, testing, implementation and maintenance for its Business Intelligence (BI). module which is certified as both an Inpatient and Ambulatory Modular EHR in the 2014 Edition of meaningful use. MEDHOST software development is compliant to 21 CFR 820 with 2 exceptions. Our product for the Automated Threshold Calculations and Clinical Quality Measures criteria of the 2014 Edition of Meaningful Use is called Business Intelligence. BI is part ofthe product line which is offered through MEDHOST. Development, testing, implementation and maintenance of PL BI followed the HMS QMS defined below. This certification is for our Modular Inpatient Certification of Business Intelligence v4.0. Development, testing, implementation and maintenance of all criteria for which certification to the 2014 Edition of meaningful use follows the guidelines set forth in the 'Scope' statement of the Quality Management System Manual is presented below: Scope MEDHOST's Quality Management System was created in compliance to the Food and Drug Administration's Code offederal Regulations Title 21 Part 820 Quality System Regulation. MEDHOST applies this Quality Management System to its Electronic Health Record (EHR). Food and Drug Administration Code offederal Regulations Title 21 Parts 803, 806, and 821 are also incorporated in our Quality System, as applicable, for our medical device products. The QA Manual has two (2) exceptions to the 21 CFR 820 process for our EHR development that is separate from that required for our Class 2 Medical Device software. 1. Our Medical Device software complies with all CAP A requirements of 21 CFR 820 while the rest of our EHR software complies with the CAP A requirements for Processes only. 2. Design Risk Analysis is conducted for our EHR software and Medical device products, but only is documented on the Risk Analysis tool for our Medical Device software.
