1 Cyberbullying and its effects (Please DO NOT write your name anywhere on this booklet)
2 Information & Instructions We are asking lots of young people aged between years old to answer these questions about cyberbullying and its effects. This is so we can get a better picture of the problem and how it affects people. This is so that schools & colleges can develop better ways to fight bullying. Do not write your name on the survey this is so that each survey stays anonymous. All the answers will be put into a computer so there will be no way to pick your answers out from the results. Your parent(s) have said it is ok for you to fill in this survey but you can now choose if you want to. If you don t want to take part please turn to the attached page & fill in your age & gender and then just leave it on your desk. If you do want to fill it in then start at Question 1 and work your way through to the end. Try not to miss out any questions, but it s ok if there are some you don t want to answer just leave them blank & go on to the next one. If you don t know the answer just write DK. When you ve finished, all the surveys will be collected and put into a sealed envelope and returned to the researchers no-one at the school will be able to see your answers and there are no names on the survey. Everyone in your year group who fills in the survey will be entered in a Prize Draw for a 25 voucher (e.g., ITunes, HMV etc), so remember to keep your Prize Draw raffle ticket safe! When we have all the results we ll come back to your School & give a presentation so that you, your parents and teachers can see what we found. We have provided a leaflet with this survey (that you can keep) that has some tips on how to cope with bullying & where to get more information about what to do if you are being bullied or want to talk to someone about bullying.
