2 how it works Welcome to PATHWorks! The PATHWorks program is a voluntary program open to all housing choice voucher holders. You will work towards reaching your goals, working one-on-one with your own personal client services coordinator (coach), by participating in workshops and completing selected tasks. The program can last up to 5 years. However, you may graduate early. The only requirement for participation in the program is your desire to actively work towards your goals and to seek and maintain suitable employment prior to your graduation. We believe it takes a holistic approach to see lasting change. By looking into five key elements; Goals, Finances, Employment, Education and Personal Development, and providing coaching, workshops and resources in these areas, you will be equipped to achieve your dreams. In the following pages you will find a variety of workshops, resources and tasks that you can choose to attend/complete based on your needs and goals. Each workshop or task has a credit assigned to it that you can use as a fun way to track your progress. Each workshop or task includes a description and contact information that you will find on the description page. This information will help you plan a course of action with workshops/tasks that work best for your individual situation and goals. The PATHWorks program is designed to support your goals and dreams. Your enthusiastic engaged participation is critical to your success. You will meet, help and inspire others with your progress and success along the way. The program looks to you for input and feedback about what services would best support your journey. We encourage teamwork and support to each other throughout the program and beyond. 4 goals PATHWorks orientation credits 1 Orientation is the participants first informational meeting to explore the roles of the program participant and client services coordinator (coach). This orientation can be accomplished with a one-on one meeting with your coach or by attending one of the regularly scheduled orientation events. goals workshop credits 1 Goals workshops are scheduled throughout the year and consist of a 1 hour hands on workshop that will help you identify and develop both your short and long-term goals. vision boarding credits 1 This hands on workshop is a great way to visualize your goals. You will create a collage of positive images and phrases that speak to you. By placing this board in a prominent place in your home you have a constant visual reminder of your goals. finances S.T.E.P.S. credits 4 The Pacific Institute s STEPS to personal success is a 12 hour workshop that will help you understand how your mind works and how to make meaningful change in your life. The NCRC helps you find jobs that fit your skills and interests. It enables you to rank above other job applicants who cannot demonstrate their skills, and also confirms to the employer that your skills will meet their needs. Complete at your local WorkSource office. or imatch skills credits 2 Spend a few minutes registering and entering your skills then find the great jobs that are waiting for you at the click of a mouse. imatchskills improves the quality of the matching process by going beyond resumes and job titles - straight to your job skills. up your resume credits 1 Learn the importance of what should and should not be in a resume and why. Regular meetings offer time to brain storm, seek out tools and receive referrals. 9 agreement PATHWorks Coaching Collaboration Agreement The PATHWorks program has adopted a coaching based approach. Coaching happens when a person is ready to learn more about themselves, their relationships to others and their surroundings. In a coaching relationship, participants set their agenda and make their own decisions with the support and accountability of a coach. Coaching is similar to Counseling; however a counselor often creates a plan for the client based on what they interpret to be the best course of action. The coaching relationship is a designed alliance. Coaches work with participants to move towards their goals. This process will be driven by the goals of the participant; the coaches offer tools, accountability and support. Coaching is a combination of neutral 3 rd party information, tools, accountability and space for participants to define their own goals; it is a process. Participants and coaches are expected to be clear and honest about what works and what doesn t and why, about how they prefer to communicate and about what their barriers are. PATH Works offers support as participants work towards employment, educational and financial goals on their path to long term economic self-sustainability. Please review and discuss the following guidelines to ensure shared expectations between the participant and coach. Participants agree to the following: 1. Maintain quarterly contact with my coach through face-to-face meetings, phone, or mail. 2. Respect myself and my coach s time by being on time and notifying my coach as soon as possible if needing to reschedule any appointments. 3. Assume total responsibility for the decisions I make about my goals and I will not hold Housing Works responsible for any consequences resulting from my actions. 4. Be open to hearing new ideas and accessing resources presented in this process. 5. Comply with all HUD Contract of Participation requirements. 6. Actively participate and work toward achieving my goals at all times.
10 agreement The Coach agrees to the following: 1. Maintain confidentiality as required by HUD guidelines, sharing information with partner agencies for the benefit of the client as outlined in the release of information disclosure and privacy agreement. 2. Refer participants to the appropriate professionals as needed as I am not a tax professional, lawyer, or licensed financial planner. 3. I will not make decisions for participants; taking actions outlined in educational workshops and meetings is the participants responsibility. 4. Help clarify financial topics, present options, promote financial wellness, agree on action steps and encourage participants to pursue their goals. 5. Offer participants with information, education guidance and referrals in order to be able to able to make informed decisions. 6. Develop relationships with community partners to provide adequate referrals. I understand the approach of the PATHWorks team, have been given an opportunity to ask questions and agree to the above terms of participation. This agreement is supplemental to the HUD Contract of Participation. Participant Coach
