50 A practice is on its way to meet medication reconciliation criteria because its patient portal enables post-op access to a clinical summary of care including a medication reconciliation list. Does this meet meaningful use criteria? NO! WHY NOT? MU criteria specifically addresses transitions of care thus providers have to be trained to use a specific code to identify a post-op visit as a transition of care event. This is an EMR awareness, a workflow issue, and a data flow issue.
