3 The Physicians Network Working with doctors Rebecca Barbato, Rebecca Turk and Karen Walker Ward, the Collaboration launched the Physicians Network in July 2008 to promote a stronger connection between the early childhood community and health practitioners, and to increase their awareness of early childhood developmental issues and services. Dr. Barbato is a pediatrician in practice at Loyola Center for Health at North Riverside and is assistant professor in Pediatrics at Loyola University in Chicago Stritch School of Medicine. Dr. Turk is a pediatrician at Pedios Ltd. in Oak Park. She previously worked on a grant at Loyola Medical Center to implement developmental screening at Loyola s nine satellite clinics. Dr. Walker practices pediatrics at Pediagroup in Oak Park and is the medical director of The Children s Clinic sponsored by the Oak Park/River Forest Infant Welfare Society. She serves on the Executive Committee of Illinois Chapter of the American Academy of Pediatrics. The Physicians Network Breakfast Meetings The Physicians Network sponsors two breakfast seminars each year focusing on key issues in children s healthy development. Topics have included screening for developmental delays during well-child visits and the referral process, identifying red flags for learning disabilities, roots of childhood obesity and what to do about it, perinatal/postpartum depression screening, and promoting oral health. When feasible, physicians receive Continuing Medical Education (CME) credits for attending the meeting.

4 Collaboration for Early Childhood Care & Education During , all six of Oak Park s governmental agencies joined to initiate and promote the Collaboration for Early Childhood Care & Education. Established as a not-for-profit organization, the Collaboration became a unique public/private partnership in which all of the governmental agencies contribute financially and participate through staff and/or board involvement. Now, with more than 40 organizations participating, the Collaboration works to overcome the fragmentation and scarcity of services endemic to the early childhood field by leveraging and integrating all of our community resources to better meet the needs of the youngest children and their families. Our key initiatives include the advancement of High Quality Preschool for All Children, Professional Development, Parent Information and Support, and Developmental Screening. The Collaboration is a voice for parents and children regarding issues of early learning both locally and at state and national levels. It works with all elected officials to promote child-friendly policies and programs that support very young children and their families. The Collaboration has received state and local recognition, serving as the model for the Illinois Early Learning Council. President Obama has called for a federal early learning council based on the Illinois model to coordinate policy and resources. The Collaboration has also received local awards from the United Way of Oak Park, River Forest and Forest Park, and the Oak Park Education Foundation, in recognition of its innovative structure, programs and impact on the well being of children and families. In April 2010, the Collaboration received the Dr. William Fitzsimmons Public Health Award for its Hearing and Vision Screening Program.

6 Making a Referral: Early Intervention Newborn to 36 Months The Illinois Department of Human Services Early Intervention Bureau coordinates services for children under 36 months of age through a network of Child and Family Connections Offices (CFC). CFC 7 serves children living in Central and West Suburban Cook County. The CFC arranges for evaluations and assessments of children to ascertain delays in one or more of the following areas of development: physical development, cognitive development, speech and language development, social or emotional development, and self-help skills. Families with eligible children receive an Individualized Family Service Plan (IFSP). The IFSP lists the services and supports that must be made available to the family. Early Intervention services are paid for with a combination of government and family resources. The cost of some services are paid by the program and provided to families at no cost. These services include evaluation, assessment, development of a service plan, and service coordination. Services recommended in an IFSP may include, but are not limited to: physical therapy, occupational therapy, speech/language therapy, developmental therapy, psychological services, and social work services. On-going Early Intervention services are paid for by the families health insurance, government insurance (All Kids/Medicaid), and program funds. Families contribute to the cost of services by paying fees based on a sliding scale. Physicians Network of the Collaboration for Early Childhood Care & Education 5

7 CFC 7 Referral and Communication Process Below is a description of an effective referral and communication process between a referring physician and Child Family Connections 7 (CFC) serving Central and West Suburban Cook County. 1. The physician should complete the Illinois Standardized Referral Form with the parent or guardian of the child. If the physician is interested in receiving information about the outcome of the referral, be sure to check ( ) the appropriate boxes in Section 4 and have the parent or guardian complete and sign Section Fax the completed Illinois Standardized Referral Form to the CFC 7 office. Fax #: (708) CFC 7 will assign the family a service coordinator who must contact the family (usually by phone) within 2 business days. Please share this information with the parent or guardian and let them know to expect a phone call. 4. CFC 7 will send the referring physician a completed Illinois Early Intervention Program Referral Fax Back Form Part I within 2 weeks. This section will inform the physician if: a. The family has been scheduled for an evaluation; b. CFC 7 has been unable to contact the family and would like the physician to assist them in making a connection; and c. The parent or guardian has asked that the physician contact him or her directly for results. 5. When an Intake is scheduled, CFC 7 will coordinate an evaluation of the child to determine eligibility for services and what services are recommended. 6 Physicians Network of the Collaboration for Early Childhood Care & Education

8 6. After eligibility is determined, the service coordinator will complete Part II of the Referral Fax Back Form and send it to the referring physician. Other information may include: a. The Individualized Family Service Plan (IFSP) Summary will be sent to the referring physician if the parent or guardian has authorized the release of this information by completing Section 6 of the Illinois Standardized Referral Form. Note: Physicians who would like to receive a FULL COPY of the Individualized Family Service Plan should contact the service coordinator listed in Part I of the Referral Fax Back Form. b. The service coordinator will also send the referring physician a request for any needed prescriptions to implement the plan. 10 Insights for Healthcare Providers: Lessons from Practice Parents whose primary care physicians routinely inquire about their child s development and use a validated screening tool for all domains of development report a high level of respect, satisfaction and appreciation. Their experience of this attention to their child s development is likely to increase follow-through on referrals. Ongoing communication between physicians and parents when their children are receiving services contributes to children s health and well-being. When parents feel positively about their physician s investment in their child s development, especially when physicians inquire about ongoing services, parents are more proactive in the communication loop. Parents who are given developmentally oriented activities by physicians while waiting for developmental evaluations find them helpful and this may contribute to their competence and confidence as collaborators in the communication loop. When there are positive relationships, cooperation and coordination of efforts, families, physicians and service providers express satisfaction and gratification regarding their efforts on behalf of infants and young children s development. Physician offices that have bought into and made the transition to routinely administering developmental screening tools communicate pride and satisfaction with their attention to this aspect of health promotion, prevention and intervention. Families and service providers share in their praise of medical offices which have adopted these practices and consider communication to be superior in the offices that support developmental screenings. 11 Making a Referral: Early Childhood Special Education 3 to 5 Years Early childhood special education services for children, three through five years of age and their families, are provided through local school districts and special education cooperatives. Professionals with training and expertise in special education services implement the federal Individuals with Disabilities Education Act (IDEA), Part B, by supporting the educational needs of young children and families. Early childhood special education professionals and related services personnel provide specialized educational services to children with disabilities in a variety of settings: early childhood special education classrooms, preschools, child-care centers, Preschool for All programs, Head Start programs and other early childhood settings to meet the developmental learning needs of these children. In addition, families and early childhood providers may request information about appropriate expectations for children s development. Referral and Communication Process The first step in determining if a child needs special education services to meet developmental learning needs is for the parent to request an evaluation. If the physician or the parent is concerned about a child but is not certain that an evaluation is indicated, it is always worth a phone call to the local school district s early childhood special education personnel to seek information and guidance (see school district listings). When a physician has concerns about a child s development that he or she believes may benefit from specialized educational services, the physician should encourage the parent to contact the child s local school district to request an evaluation. If a physician has significant concerns and is not certain that a parent will make a request for evaluation, the physician should contact the child s local school district for information and guidance. 12 Initial Eligibility Step 1: Request for Evaluation A request for an evaluation is made by the parent to determine the child s eligibility for special education. It is best practice to make the request in writing. A phone call may not be sufficient to guarantee consideration of an evaluation. 14 School Days Step 2: Decision to Proceed At this stage the school district meets with the parent to determine whether evaluation should proceed. If yes, the district needs to determine what domains will be evaluated and will request the parent s consent to do the evaluation. Timeline begins when parent signs consent Step 3: Consent to Evaluate The evaluation cannot proceed until the parent has provided informed written consent for the district to begin the evaluation. Timelines for completing the evaluation do not start until consent has been given by the parent. Step 4: Evaluation During a period of up to 60 school days, district personnel can take steps needed to complete evaluation. If any part of the evaluation is written in final form, it may be shared with the parent. 60 school days Step 5 must occur by the 60th day Step 5: Eligibility Conference and IEP By the end of the 60th day, the evaluation team must meet with the parent to decide if the child is eligible for special education. If yes, an Individual Education Plan (IEP) team must then meet with the parent to develop the IEP for the student. Initial Placement does not occur until the parent signs consent Step 6: Consent for Placement Before special education services begin, the parent must provide written informed consent to allow the district to proceed with placement. Placement may begin no sooner than 10 days after the parent consents unless the parent gives permission for it to start sooner.
