4 INTRODUCTION Choosing your Primary Care Physician (PCP) When you enroll with Care1st Health Plan, you will select a Primary Care Physician, ( PCP ); a PCP is a doctor who will be your personal doctor. Your PCP will be the doctor you will see when you are sick and he/she will send you to a specialist and coordinate your health care whenever you need it. You must always see your PCP each time you need medical care unless it is an emergency. Each family member may pick the same or a different PCP/doctor or clinic. Remember that all family members must choose a PCP/doctor. INTRODUCTION The doctors listed in this directory are by the city they work in. You can also go to the back of the directory to find a list of the doctors by their last names. The doctor s provider number is a very important number (this number is located directly under the doctor s name). Please make sure to write this number on your enrollment form. If your current doctor is not listed, call Care1st Health Plan. You may need to choose a new doctor. Types of PCP s/doctors The list below will help you learn what each specialty /type of PCP means so you can choose the best doctor for you and your family: DOCTOR SPECIALTY Family Practice General Practice Internal Medicine Obstetrician (OB/GYN) Pediatrician MEMBERS DOCTOR USUALLY SEE For the whole family For the whole family Adults Women Children To pick a PCP: Look in this book for a doctor in the city where you live Pick a doctor for you and your family members Write the Doctor ID (Identification) Number or the name of the doctor (s) you choose for you and your family on the Medi-Cal Health Plan Choice Form. Sign your name on the Medi-Cal Health Plan Choice Form Check to make sure you filled out the Medi-Cal Health Plan Choice Form completely. Mail your completed Medi-Cal Health Plan Choice Form in the self-addressed, stamped envelope included with your packet. Call Care1st Member Services at , if you are already a member and want to change your doctor, or if you want help to become a member. The PCP and Medical /IPA you choose, decide which specialists you can go to and where you will get other specialty services like vision and hospital care. To find out which Medical /IPA your doctor is with, check under the doctor s name in the directory. EXPLANATION OF SYMBOLS If this symbol (*) is shown next to a doctor s name, this means the doctor cannot take any new member at this time. However, if this doctor is already your doctor, please call Care1st at IPA (Independent Physician Association) is a group of PCPs, specialists, and other health care providers that work together. PCP (Primary Care Provider) is a personal doctor. The PCP takes care of health care needs and works with members to keep them healthy. The PCP will also make specialty referrals when medically necessary. A-3

5 INTRODUCTION Some hospitals and other providers do not provide one or more of the following services that may be covered under your plan and that you or your family member might need: family planning; contraceptive services, including emergency contraception; sterilization including tubal-ligation at the time of labor and delivery; infertility treatments; or abortion. Call Care1st Member Services at to ensure that you can get the health care services that you need. For help in locating a Nurse Midwife in your area, please contact the American College of Nurse Midwives electronic Practice Locator Service at MIDWIFE ( ). You will be asked to enter your ZIP code, and you will be provided with the name, address and telephone number of the Nurse Midwife (or Midwives) closest to your area. MEMBER IDENTIFICATION CARD As a Member of Care1st Health Plan you will receive a Member Identification Card like the one pictured below. You will need to show this card each time you see your doctor, get your prescriptions (medicine), use the emergency room, or see your eye doctor. Keep this card with you at all times. When you get your ID Card, please make sure that it is correct. If not, call Care1st Health Plan Member Services Department at Monday through Friday 8 AM to 6 PM and we will make the changes and send you a new card. DO NOT THROW YOUR MEDI-CAL (BIC) CARD AWAY You will need to use your Medi-Cal card to see your Medi-Cal dentist and to get other services that are not covered by Care1st Health Plan. Care1st Health Plan Member Service Department Care1st Member Service Department is here for you when you have questions about your health plan, want to change your doctor, or have a complaint about your doctor or the plan. The Member Service Department is open Monday through Friday from 8:00 a.m. to 6:00 p.m. After-Hours or Urgent Care Sometimes you or your child gets ill after hours. You are not sure if you can wait until your doctor s office opens. Urgent care services are available when you are within or outside of Care1st Service Area. Urgent Care Services are those services needed to prevent the serious deterioration of one s health from an unforeseen medical condition or injury. For urgent care, or after-hours, follow these steps: STEP 1: Call your PCP. Another doctor may answer your call if your PCP is not available. A doctor is available by phone 24 hours a day, 7 days a week. STEP 2: Tell the person who answers the phone that you are a Care1st Health Plan member. STEP 3: Ask to speak to your PCP or the doctor on-call. Tell the doctor what has happened and follow his/her instructions. Call Care1st Health Plan at anytime if you cannot contact your PCP. Remember: If you have a life-threatening emergency, call 911 for emergency help or go immediately to the closest emergency room. A-4
