3 No lines = No Halos, Sparkles or Glare! How Rx 2 Works Implant the first lens into the dominant eye or according to your usual protocol. It will have a 0 or 1 in the box by the lens Diopter. If you used a 0 after implantation refract the patient and record the reading acuity. If NO adjustment to reading is needed, implant another 0. If an adjustment of +0.2D is needed you simply add 2 to the first lens and the 2nd implant will have a 2 in the box. If +0.3D is needed 0+3 = 3 so you would implant a 3. Research shows that the recessive eye will take over and your patient should be happy & spectacle free. 1st Eye 2nd Eye Optical Zone Dilemma Because the Zones on the standard Multifocal lens reduce the light to the retina by 20%, IOL companies make the lenses Aspheric. 4 DIFFRACx Micro Yesterdays standards for visual outcomes are no longer considered adequate for the active life-styles of today. Imperial Medical Technologies, for more than 10 years has been quietly and confidently working on new optical systems that we believe will set the standards for others to follow. Now, after the introduction of PRESBIOPTICx the World s first Progressive Multifocal IOL using refractive optics, IMT is once again leading the way with DIFFRACx Micro the World s first Progressive Multifocal IOL using diffractive optics with NO lines which can mean NO halos and glare for patients. How It Works It s well known that for decades diffractive technology has been used for optical systems in many medical fields, scientific fields and space exploration. Now, nano-technology in manufacturing has advanced enough to create a diffractive intraocular lens perfect enough to make a real difference. IMT uses the laws of optical physics combined with state of the lens nano-technology, on the world s finest high tech equipment, to create an optical system that can provide emmetropia to patients as never before. As light travels through the multiple zones of a multifocal lens they are focused on the retina which feeds a myriad of signals to the brain. Neuro-Surgeons, Psychologists and Ophthalmologists know that the brain is capable of filtering and sorting multiple images and then centering the attention of a precise image. DIFFRACx uses the brains superior awareness to detail to its advantage. The theory behind the normal diffractive optical system is to supply sharp, crisp images for the brain to sort, thus allowing for good distant ision, good mid-range vision and good close vision. Unfortunately, the standard multifocal lens offers concentric circles or lines that dilute the image and blur the true crispness of the image. Imperial Medical invented the progressive IOL and those images can now be viewed without loss of light and without the image being blurred by concentric lines, separating the dioptric zones. This is done through more than 20 zones allowing for distance, mid vision and near vision no matter what the pupil size. More Light Unlike its competition, IMT modified the DIFFRACx Micro to include its technology from its ASPHERICx IOL line and its progressive multifocal PRESBIOPTICx intraocular lens line. Because of that, DIFFRACx Micro lenses are also progressive, aspheric and aberration controlled meaning that there are clearer focal zones with NO LINES to eat up valuable light needed at twilight or dusk. Dr. Gianluca Rubiolini, Como, Italy, reported in a formal presentation at the ASCRS that for the first time, he was able to perform Diabetic Retinopothy surgery without shadows on the retina and without fear of the laser bean hitting the rings of the lens because with a progressive multifocal THERE ARE NO RINGS. How Its Made The EOEMA material is carefully and methodically lathe cut on precision air bearing lathes and once milled, carefully tumble polished to remove any free radicals. Each lens is carefully hand inspected on a wave-front analyser for power and quality before being called an Imperial Medical intraocular lens.

5 Clinical Data Imperial Medical s quality process, combined with superior plastics, have proven to provide superior visual acuity and unquestionable safety. Urminský, PhD of the Ophthalmological Clinic, Medical Faculty of the Charles University & University Hospital in the Czech Republic presented finding from his 135 patient study in 2004 that IMT lenses from EOEMA presented significantly lower PCO rates than other lenses tested. 1 In addition, Dr. Gianluca Rubiolini reported in a 2011 study, released at the ASCRS that more than 90% of all patients implanted with DIFFRACx Mirco were free of spectacles and the other 10% were less than 0.25 diopters off two months post op. Indeed, studies indicate that when properly used, implantation of diffractive intraocular lenses can provide excellent long term visual acuity without adverse reaction and without visual misadventures for the patients. 2 The Archives of Ophthalmology in 1999 published a study called Bilateral Implantation of Asymmetrical Diffractive Multifocal Intraocular Lenses, which detailed that eighty percent (80%) of the patients in the study reported no use of spectacles postoperatively. However, studies also indicate that the No. 1 Complaint of all multifocal lenses is halos, glare, sparkles and starbursts. Surgical Features DIFFRACx Micro IOLs combine many features that will allow the surgeon to have a quality surgical experience and positive surgical outcome when using DIFFRACx for their patient s visual needs. Micro Incision Technology Because of its unique features, DIFFRACx Micro intraocular lenses can be implanted through 1.6mm to 1.8mm cartridges up to and including diopters. However, surgical experience should dictate the size of incision and the size of cartridge to be used. IMT always recommends using a silicone cushion when implanting and we recommend that from to you can use a 1.6mm to 1.8mm cartridge. From to you can use a 1.8mm to 2.2mm cartridge and above it is recommended surgeons use a 2.4mm to 2.8mm cartridge. Caution should always be used when injecting an intraocular lens into the capsular bag and inexperienced surgeons should seek proper training before attempting micro incision lens injection. Lens Positioning DIFFRACx Micro was designed to be implanted in the capsular bag with the diffractive surface anterior. Surgeons will note a small position indicator (hole) on the Top Right and Bottom Left of the wings or haptic. Care should be taken to always implant the lens with the small position indicator to the Top Right or Bottom Left or the 1:00 o clock and 7:00 o clock positions. 12 The Procedure The system comes complete with detailed instructions that closely follow the standard surgical techniques used for Keratoconus, High Myopic, Astigmatism, Ectasia corneal congenital or post op of refractive surgery. Once the visual axis is located, the cornea is marked and the site for the incision is formalized. Then, the incision is made to the prescribed depth, usually about 75% of the stromal depth which is indicated in the pachymetry. If the Vejarano Diamond Knife is used, the base of the tunnel will be easier to separate. If not, use a standard RK knife and carefully make the incision to the prescribed depth. Next, the tissue is gently separated with the Vejarano pre-delaminator. Once the small cavity is started, introduce the Vejarano Y Spatula and start delaminating in a clockwise and counter clockwise directions. Once the tunnels are prepared the Patent Pending V Glide Automatic Vacuum System can be gently placed on the cornea. Vejarano V - Glide System The Secret is the Patent Pending V-Glide Positioning System which maintains an even and continuous vacuum. After the disposable tubing and attached Luer locks are assembled and connected to the Vacuum, carefully place the Vejarano V Glide instrument onto the surface of the Cornea aligned with the original markings. Press the Low Vacuum (50mmHg) switch or the High Vacuum (65mm Hg) switch. (Note: Amount of vacuum used is indicated by the surgeon s preference). The Vejarano V-Glide gently fixates the alignment ring onto the cornea and maintains the prescribed vacuum. The digital readout indicates both the amount of vacuum and the time the patient is under vacuum in order to provide safety for you and your patients. Safely Create the Tunnel The Vejarano V Glide positioning tube allows for the surgeon to rotate the delaminator smoothly and easily thereby reducing the potential for a perforated cornea. Once the ring is fixated on the cornea, the Vejarano V Spatula is inserted in either a clockwise or counterclockwise direction and the appropriate Vejarano delaminator blade tip is placed below the spatula in order to introduce the tip of the Vejarano delaminator into the opening of the tunnel. Gently remove the Vejarano V Spatula leaving the Vejarano delaminator blade in the entry to the tunnel. Next carefully rotate the Vejarano delaminator 180 creating a smooth and even tunnel. Reverse the rotation and extract the Vejarano delaminator from the Vejarano V Glide positioning tube. Follow the same procedure in the opposite direction and use the same technique. Extract the Vejarano delaminator from the Vejarano V Glide positioning tube and turn OFF the vacuum. This will allow for an easy removal of the Vejarano V Glide System from the cornea. Gently irrigate the eye if the protocol dictates and you are ready to insert KERATACx, the World s Finest, Patent Pending Corneal Segments. 15 KUSTOM LENS TM The newest lens system, still in R&D, is KUSTOM LENS TM which uses OphtaTOP TM to measure the topography, anterior chamber and retina for patients who have had LASIK or other corneal procedure.
