We have multilingual doctors and staff, and we offer interpretation services by phone in 140+ languages. When you have a doctor you connect with, it s easier to stay healthy. Choose your doctor and change anytime Browse our online doctor profiles at kp.org/ searchdoctors to see your options. You ll find information on a wide range of highly skilled doctors, including their education, credentials, and specialties (in fact, only 1 in 10 doctors is selected to work for Kaiser Permanente). Then choose the one who s right for you. You can choose your doctor from: Adult medicine/internal medicine Family medicine Pediatrics/family medicine (for children up to 18) Choose 1 doctor for your whole family or a different doctor for each family member. You can also change your doctor anytime. See specialists often without a referral You don t need a referral for obstetricsgynecology, optometry, psychiatry, chemical dependency, or addiction medicine. For other specialties, your doctor can easily refer you. Your entire care team works together often under one roof sharing information to give you the care you need. Choose how you get and manage your care In person You can always make an appointment to see your doctor in person. And because most of our locations offer many services under one roof, you can see your doctor, get lab services and X-rays, and pick up your prescriptions all in one trip. By phone Save a trip to the doctor s office. Where telephone appointments are available, you can schedule one to talk to your doctor by phone. If you re not sure what kind of care you need, you can also call our advice nurses 24/7. By Connect with your doctor anytime. You can your doctor s office with nonurgent questions and get a reply within 48 hours, often sooner. By video Save time with a video appointment. For certain conditions or symptoms, you may be able to see your doctor using your computer or mobile device. Online Stay on top of your care 24/7. Once you ve registered on kp.org, you can use our website to choose your doctor, schedule routine appointments, view most lab test results, refill most prescriptions, print vaccination records, and manage a family member s health. Help to quit smoking Let us help you succeed in quitting. You pay nothing for classes, telephone coaching, online programs, and prescriptions, including prescribed over-the-counter tobacco-cessation drugs. Ask your doctor about your treatment options. * The rewards program runs from January 1 through December 31, 2016, and is open to all Kaiser Permanente FEHB subscribers and their enrolled spouses 18 and older. You can take the Total Health Assessment as often as you would like and as many healthy lifestyle programs as you want, but you can only earn rewards for them once during the reward period. You must complete the activities during the calendar year. Reward cards expire 12 months from date issued. 5

Easy access for easier care With convenient hours and locations, it s simple to get the care you and your family need. Many of our locations offer same-day or next-day, after-hours, and weekend services, along with obstetrics-gynecology, pediatrics, and other specialty departments. Many services under one roof Most of our facilities offer a wide variety of care and services, so you can take care of several health care needs in one visit. You can see your doctor or specialist, get a lab test or an X-ray, and pick up your medications all without leaving the building. Finding convenient locations Choosing the right location is as easy as hopping online or pulling out your smartphone. Visit kp.org/kpfacilities to search by ZIP code, keyword, or the service you want. To search with your smartphone or mobile device, just use our location finder on the Kaiser Permanente app.* Getting care anywhere With so many locations to choose from, you re sure to find one near your home or work whatever s convenient for you. You can also see different doctors at different locations, either for yourself or your family. We can mail most prescription drugs to you within 10 days. You don t pay for U.S. mail delivery. And when you order refills by mail, you get a 100-day supply and pay only two 30-day supply plan pharmacy copays. Chiropractic without a referral You can receive up to 20 chiropractic visits per year, without a referral, with any American Specialty Health (ASH) Plans of California, Inc., participating provider. Key benefit changes to High and Standard Options include: Added a specialty drug tier. Specialty drugs are often for treatment of complex diseases and may be high-cost. Our FEHB Drug Formulary contains a list of our formulary and non-formulary drugs and the tier your drug is in, so you can determine the cost you ll pay. For more information go to kp.org/feds. No. You can choose the new enrollment type or the Self and Family coverage. What s the cost difference between the Self Plus One and Self and Family coverage? Self and Family is less expensive than Self Plus One, so you would save money by enrolling in Self and Family. 15 Kaiser Permanente Senior Advantage (HMO) for Federal Members Why Senior Advantage for Federal Members is right for you If you decide to enroll in Senior Advantage for Federal Members, you ll get better benefits than with your FEHB coverage alone. You ll get all of your FEHB benefits plus: Lower copayments for office visits and for most other services, such as inpatient hospital visits, most prescription drugs, and many other benefits Up to a 100-day supply of prescription drugs at plan pharmacies for 1 copayment (High Option only) No deductibles (Standard and Basic Options only) Extra credit toward the purchase of eyewear, including eyeglasses Extra coverage for preventive dental care through DeltaCare USA at no extra cost (High and Standard Options only) What s new for current members of Senior Advantage for Federal Members? The 2016 FEHB benefit changes listed on page 8 apply to you. Self Plus One is a new enrollment type available in If you cover only you and 1 other person (either your spouse or a dependent), you may choose between enrolling in the Self Plus One or Self and Family enrollment type. See page 11 for more information on Self Plus One. You can change your enrollment type, but remain in the same Kaiser Permanente FEHB option you have now. For example, if you re a Kaiser Permanente High Option member, you may remain in the Senior Advantage for Federal Members High Option. Be sure to choose the right enrollment code on page 11. To enroll in our Senior Advantage for Federal Members, you must first enroll in Kaiser Permanente s FEHB plan (either the High, Standard, or Basic Option), plus Medicare Parts A and B (or Part B only). After that, you can apply for Senior Advantage for Federal Members. We can help you make an easy transition. For questions or help with the enrollment process or to have information sent to you, please call Member Services at (TTY 711), 7 days a week, 8 a.m. to 8 p.m. Kaiser Permanente is an HMO plan with a Medicare contract. Enrollment in Kaiser Permanente depends on contract renewal. To join a Kaiser Permanente Medicare Health Plan, you must reside in the Kaiser Permanente Senior Advantage service area in which you enroll. Benefits, premiums and/or copayments/coinsurance may change on January 1 of each year and at other times in accord with your group s contract with us. This information is not a complete description of benefits. Contact the plan for more information. Limitations, copayments, and restrictions may apply. You must pay the Medicare Part B premium to join a Kaiser Permanente Medicare health plan. Enrolling in Senior Advantage for Federal Members will not affect the benefits you receive through the Federal Employees Health Benefits (FEHB) Program, although the network and other rules of Medicare may apply. 13
