2 Request For Proposal Specifications Publicly Available on 12/16/2014 I. INTRODUCTION Champaign County Nursing Home (CCNH) seek proposals from qualified vendors to furnish long-term care pharmaceutical services and long-term care pharmaceutical consulting as detailed in this proposal specification document. CCNH, owned and operated by Champaign County Government, is a 243-bed skilled care facility located at 500 Bartell Road, Urbana, Illinois II. SPECIFICATIONS A. Services Champaign County seeks competitively-priced pharmaceutical and consulting services. The selected pharmacy provider will provide services directly to those Champaign County residents who are private pay, covered by the Illinois Medicaid program, Medicare A, MMAI plans (Health Alliance, Molina Healthcare) and Medicare replacement plans like Medicare Advantage, HMO, and VA in a 7day packaging format. Selected pharmacy will provide equipment necessary to dispense medication format for all units on the facility in sufficient size and quantity to service Champaign County Nursing Home. The selected pharmacy will provide education on items such as IVs, medication administration as an example and annually evaluate each Champaign County Nursing Home nurse on competencies in all aspects of medication administration i.e. oral, topically, etc. The selected pharmacy provider will participate in monthly Quality Assurance Performance Improvement meeting and projects i.e. psychotropic reduction project participation. The selected Pharmacy providers consultation by RPH would ensure all residents are reviewed monthly and written reports and recommendations to MDs provided. The selected pharmacy will be available 24/7 for consultation and a daily delivery is expected with additional availability of medication procurement within 2 hours as the need arises. B. Selection Method Selection will be based upon several factors, including the competitiveness of the pricing methodology. Pricing for house stock items will be a consideration in evaluation. Administrative and clinical support capabilities for dealing with medication delivery under the Medicare PPS are vital elements for evaluating the capabilities of the successful pharmacy provider. Medicare Part D is a significant factor for all skilled nursing facilities. The pharmacy provider s scope and depth in its panel of approved Medicare D drug plans is critical for smooth resident pharmacy service.

3 Availability of medications including pharmacy flexibility during emergency supply considerations is important to Champaign County. Selection will consider the quality and depth of clinical and administrative support that the pharmacy vendor will provide to Champaign County. Given the burden and scope of regulation in skilled nursing care, the qualitative considerations represented by clinical and administrative support are not value-added activities. Rather, they are integral to the delivery of reliable, routine pharmacy services and will receive as much consideration as the cost of pharmaceutical services. Once Champaign County has selected a primary pharmacy provider, it reserves the right to negotiate the terms and conditions including the ultimate Medicare A pricing plan, the parameters of service and delivery, and the clinical and administrative support criteria. In the event that a successful agreement cannot be reached, Champaign County retains the right to end negotiations and to select another pharmacy provider. C. Evaluation Criteria The proposals for this solicitation shall be evaluated based upon a 100-point scoring system according to the following criteria and point values for each area. All supporting documentation that the pharmacy provider wishes to be considered in the evaluation process shall be submitted with the proposal. REFERENCES AND RESULTS (0-20 POINTS): Pharmacy providers will receive points based on references that are able to substantiate the provider s successful performance. Champaign County is interested in a stable pharmacy program and is interested in working with a pharmacy partner that has proven results. Results refers to your firm s demonstrated history of maintaining positive relationships in Nursing Home settings and in being able to respond to the needs of clinical staff, physicians included. Provide us with verifiable examples of your results and list references. Describe your method of ensuring delivery of routine and emergent medications, and the expected lead time on receiving routine and non-routine medications. PHARMACEUTICAL PRICE AND NET COST TO CHAMPAIGN COUNTY (0-20 POINTS): The proposals shall be rated on the proposed cost for the contract. All providers are encouraged to submit pricing information in several elements pricing for Medicare Part A; pricing for house stock items; pricing for monthly consultancy services including a review of resident drug regimens consistent with IL regulations; pricing for medication exchange/stocking per episode; and pricing for medications that will be separately billed to private paying residents. Pricing for medications should be easily reconcilable by the facility to ensure ongoing pricing matches the contract. Providers should be able to explain how the facility can audit the medication invoice to the contract.

4 ADMINISTRATIVE SUPPORT (0-15 POINTS): Pharmacy providers will be scored on their capabilities to complement Champaign County staff in medication administration, in their flexibility to handle emergency orders and substitutions, in their ability to work with physicians to select the most appropriate medication regimen, in their ability to provide clear and succinct billing support and data entry, and in the overall ease of using the pharmacy provider s systems, and in any training programs that the pharmacy will provide to Champaign County staff emphasizing compliance with State and Federal requirements. PHARMACEUTICAL CONSULTING (0-45 POINTS): Pharmaceutical consulting is a pivotal part of the overall pharmaceutical service given the recent OIG focus on the use of antipsychotic medications and total medication use. The OIG recently reported that 99% of providers are not in compliance with the use of antipsychotic medications as a component of the overall plan of care, as described in this report In addition, the OIG found that 51% of Medicare claims for antipsychotic drugs contained errors, as detailed in this report Champaign County seeks a consulting partner that can help achieve and maintain medication use compliance and minimize the use of medications in the interest of providing a person-centered approach to medical care. Bidders should demonstrate past success and a plan for working with Champaign County area physicians in minimizing medication use and achieving compliance in the management of medication regimes for the elderly. D. Contract Terms The pharmacy provider selected in this proposal process will enter into a contract with Champaign County Nursing Home. The contract will specify the services and systems support provided, the pricing structure, the reconciliation process for the Medicare A pricing plan, the process for resolving disputes, the termination provisions, and the process for amendment of the contract. Champaign County is not interested in a long-term contract with burdensome cancellation provisions. Champaign County will negotiate a flexible contractual agreement that rewards service, pricing flexibility, and support capability. E. Software Requirement Prospective pharmacy providers are encouraged to include state-of-the-art software applications for medication administration and delivery as part of their proposals. CCNH is planning on converting to electronic health records and are utilizing Achieve Matrix Software currently. At this time, a software platform has not been identified. Please include interface capabilities with nursing home EHR software.

5 F. Pricing Pricing shall be for the Medicare A plan plus house stock. Proposals shall include pricing that is consistent with SMAC levels. All proposers shall identify their pricing structures for Medicaid, private pay, and other payer sources. Champaign County will have available its listing of medications used in the past 60 days. G. Returning Products Pharmacy represents that all medications and related supplies will be first quality. Champaign County will receive full credit for any items returned to pharmacy that have been damaged in shipping, mis-labeled, or otherwise unusable through no fault of Champaign County. H. Account Representative A qualified account representative shall be assigned to the Champaign County/Champaign County Nursing Home to assist in ordering, handling service problems and providing timely information concerning the pharmaceutical services. III. PROPOSAL SUBMITTAL A. Due Date Proposals must arrive at the Champaign County Nursing Home Administrator s office on or before January 15, 2015 at 2:00 P.M., whether mailed or hand-delivered. Bids will be opened at 2:00pm on January 15, 2015 at the Champaign County Nursing Home. Interested bidders are encouraged to notify Karen Noffke, Administrator, of their intent to bid at the or address listed below by 4:30 p.m. on December 30, 2014 B. Questions Any questions regarding the requirements in this Request for Proposal must be submitted in writing. In order to guarantee sufficient time to provide vendors with a response, all questions must be received by the Administrator's Office by December 22, 2014 at 4:30 P.M.
