2 Section I: Purpose The Fayetteville Public School District is seeking proposals from experienced substitute teacher employment service firms that can provide expertise for practical, cost-effective, and integrated solutions to the substitute challenges facing the District in the next fiscal year and beyond. Section II: General Information The Fayetteville Public School District currently includes nine (9) elementary schools, three (3) middle schools, two (2) junior high schools, and one (1) high school. The District has a separate administrative office building, District maintenance building, transportation garage, warehouse, and food service building. The District has a student enrollment of 9,142, and employs approximately 710 certified staff and 590 support staff. Section III: Scope of Work It is the intention of the Fayetteville Public School District to purchase consulting services for practical, cost-effective and integrated solutions to the substitute teacher challenges facing the District in the next fiscal year and beyond. Contractor shall: Recruit, interview, screen, employ and train substitute teachers prior to placement. Place substitutes daily as needed in the District. Be responsible for all related employment services including record keeping and any required state, federal, FPS, and legal court reporting. Insure that substitute teachers meet all District requirements for working as a substitute teacher. Provide contingent workforce when required. Provide District with software management capability for substitute tracking and employee attendance. Comply with Arkansas Public School Computer Network Financial Management System Coding requirements. Software capability to run historical reports on substitutes. Possess a thorough understanding of the requirements of statutes, policies, and procedures school districts must follow, including Criminal Records Checks. Vendor to provide at least ninety-five percent (95%) fill rate for requested services (substitute teachers and substitute paraprofessionals). Capacity to invoice District for services provided on an individual basis. Provide District all needed information for Arkansas Department of Education reporting. Items to Provide Post RFP Award Notification By Winning Vendor Only Winning vendor to provide appropriate applicable, desired insurance coverage required of District. Winning vendor to provide the District full disclosure of bidder s legal liability. 2

3 Winning vendor to provide Documented proof that during the course of the services agreement there will be continuity in the assignment of staff and documented proof that staff assigned have the appropriate certifications/qualifications. Section IV: Submittals A Substitute Teacher RFP Committee will be established to review submitted RFPs. To allow the evaluation team to fairly judge the merits of each proposal, six (6) copies and one (1) marked, unbound original of responses to this RFP shall be submitted. Each copy shall include the information listed below. Cover Sheet: Include the completed cover sheet that is part of this RFP signed by a person authorized to act on behalf of the company submitting the proposal. Executive Summary: Provide a short summary that demonstrates your understanding of the scope of services required by the School District and why your firm is best able to provide that scope of service. Experience and Qualifications: Provide details of these items in regards to projects of similar nature. Overview of Company : Address, telephone, fax, ; whether company is local, regional, or national; if a multi-office firm, which office will provide the services; areas of expertise and types of consulting performed by the company. Project Team: Provide names, titles, addresses, telephone numbers and s of the person(s) who will be authorized to represent the company. Specify the names, titles, addresses, telephone numbers and s of the person(s) who will be directly working with FPS. If available, provide a copy of the company structure, names and credentials of those who service this account. Experience: Provide for each member of the project team a description of their experience and education, time with the company, and how their experience suits their role on the team. Cost: Provide a cost breakout for all service/product costs that FPS may wish to pursue with the company. References: Provide a minimum of three references from entities of similar size and nature of business as the Fayetteville Public School District for which a service has been conducted within the past three years. For each reference, supply the name, contact person, their title, telephone number and address. Describe the contract (scope, length, and dollar value) for each reference. Contract Form If you intend to propose a form of professional services contract between you or your company and the School District, please provide a sample copy of such contract form. If you prefer that the School District generate such contract, please so indicate. Litigation Record: Have you been involved in litigation with any of your clients within the last five (5) years? If so, please give the name, date, and jurisdiction of each such matter, and a brief description of the nature of the dispute and the outcome. Additional Information: Should you feel that additional information would help us more properly evaluate your proposal, please feel free to address such matters under this heading. 3

Any questions or requests for clarification regarding this RFP should be directed to the District by prior to May 31, ed questions will be the only accepted form of question submittal besides those that are brought up in the vendors conference. It is the responsibility of the vendor to ensure that Greg Mones is in receipt of all questions via . Section V: General Conditions The School District reserves the right to waive, modify, or otherwise vary the conditions or requirements herein and may accept or reject any and all proposals it deems to be in the best interest of the School District. The School District may, notwithstanding the terms herein, negotiate the terms of any response to this RFP. The proposal award recommendation will be offered after the evaluation by the Substitute Teacher Services RFP Committee. 4
