Additionally, to complete the Application form you will need to disclose: The current value of any personal assets and liabilities of all business partners and directors. Balances of any existing business loans/lines of credit. After you submit your application We will contact you shortly to advise on the progress of your application. If your business is a company, a Director s Guarantee may be required. For your convenience, the Application form which follows can be printed and completed in black or blue pen or alternatively completed by typing the details into the relevant fields (you can save your progress if you are unable to complete the entire form at once) then printing and signing. 1

Section J Privacy Protection of Information Sharing and handling your personal information If you go ahead with this application, we can give some of your personal information to credit reporting bodies and others and get information about you from them to help us assess your credit worthiness. This includes how you can: access and correct your information; make a complaint about how we manage your information; and contact the credit reporting bodies we deal with if you have queries about the information they hold. If I am the nominated Points Earner, then NAB may give to the Program Provider and vice versa, my membership account information (including membership number and full name) to confirm program membership and facilitate the crediting of points to my Qantas Frequent Flyer account. NAB may give information to guarantors and proposed guarantors NAB may give personal information about me to a potential or existing guarantor,or their authorised legal representative, (including a person or entity nominated as a guarantor in Section E) to assist them: To consider whether to act as a guarantor or to offer property as security and to inform them about: this application, the credit guaranteed or to be guaranteed; my credit worthiness, credit capacity or credit history; and any other matter NAB decides is relevant to a potential guarantor or guarantor. The information that NAB may give to a guarantor or potential guarantor may include credit eligibility information (that is information NAB obtained from a credit reporting body or based on information obtained from a credit reporting body). Information will only be given to a potential guarantor to enable them to consider whether to act as a guarantor or to offer property as security. Non-Individuals Authority to disclose confidential information to guarantors and proposed guarantors NAB may give to potential or exiting guarantor, or their authorised representative (including a person or entity nominated as guarantor in Section E) confidential information about our creditworthiness, credit standing, credit history, credit capacity or eligibility for credit. Such confidential information may include, but is not limited to, copies of this loan application or loan contract, statements of our assets and liabilities, copies of mortgage documents or equitable charge, or other information considered necessary by NAB to assist them to: (a) consider the risk of acting as a guarantor in or of offering property as security for, personal or commercial credit provided, or to be provided, to us by NAB; or (b) make disclosures where that person or entity is a guarantor of, or has NAB to us; or (c) make disclosures where that person or entity is a guarantor of, or has NAB to us and the amount owing under the personal or commercial credit is, or may be, increased; or (d) make disclosures where that person or entity is a guarantor of, or has NAB to us and at the request of the guarantor, a copy of the latest loan account statement provided to us; or (e) make disclosures where that person or entity is a guarantor of, or has NAB to us, a copy of demand relating to the personal or commercial credit, the subject of the guarantee. 5

Where I have provided any identification documentation (e.g. passport, driver s licence) to NAB in connection with my application, NAB may contact the authority that issued the document to verify the status of and any information contained in the document.
