2 In-Network Premier Plan (Culinary Network Dentist) Deductible: None Maximum Yearly Benefit: No Maximum Co-pays: Depending on the service Dental services must be from a Culinary Network dentist in the Nevada Dental Benefits (NDB) group. There are no co-pays for examinations, basic cleanings, x-rays and silver fillings. Orthodontics (braces) are covered for you and your eligible dependents. Dual Culinary Coverage: If you and your spouse/ domestic partner both have Culinary dental benefits, you and your eligible dependents will not have any co-pays. Anesthesia for some extractions are covered. Orthodontics (braces): In- Network (Culinary Network Dentist) Only Benefit eligible dependents (under 19): For regular 24-month treatments*, the co-pay is $850 ($300 at the beginning of treatment and then $50 for 11 months). Benefit for you and your eligible dependents (age 19 and over): The co-pay is $3,100 ($900 at the beginning of treatment and then $50 every month for 36 months, plus a maximum of $400 at the end of active treatment). You can find this sign hanging in all of our In-Network Premier Dentists offices! *For eligible dependents (under 19): Dental treatment depends on diagnosis and if additional appliances (a device that is used to move or position teeth during treatment) are needed or if treatment will be longer than 24 months.

3 Out-of-Network Basic Plan (Non-Culinary Network Dentist) Deductible: None Maximum Yearly Benefit: $1,500 for you and your eligible dependents Co-pay: Depending on the service Amount Payable (out-of-pocket): You are responsible for all charges beyond the Plan Schedule of Allowables. You may go to any dentist of your choice. Nevada Dental Benefits will pay for charges that are covered up to the total listed in our Schedule of Allowables. You will pay the difference between what NDB pays and what the Non-Culinary Network dentist charges. Dual Culinary Coverage: If you and your Spouse/ Domestic Partner both have Culinary dental benefits, you may still have co-pays and extra costs. Dental Emergencies (applies to both plans) If you have a dental emergency, call your dentist. If your dentist is unable to see you within 24 hours, contact NDB at NOT ALL DENTISTS WHO SAY CULINARY IS ACCEPTED HERE ARE IN OUR NETWORK.
